    *So many people have abnormalities on MRI and no pain - the MRI only means anything if the findings correlate with your symptoms. My advice if you have pain is to treat your shoulder function - I found that exercise in **_Bulletproof Your Shoulder_** very helpful.*

    Thanks for this video. I am in the medical field and have been sitting up all night trying to determine if I have a RTC tear. I compared my 3D CD with yours and see that there is an impingement. Thanks again for posting this video!!

    Excellent explanation of the shoulder MRI. The only thing more I wish you would have pointed out would be for people with Labrum (SLAP) tears which are also pretty common. I have a RC tear so this was perfect.

    Wow...awesome. Had MRI’s on both. Right one was repaired with anchors 7 yrs ago and has new lateral tear, arthritis and bursitis. Left one doc told me was “in bad shape”...left bicep detached, cuff torn(grinding, popping & crunching) and he never addressed what appears to feel like a pinched nerve underneath shoulder blade. That pain is bad, besides other problems...but that nerve pain is almost debilitating. Could you possibly explain what might be happening. 50 yr old active male. Thank you so much for your time and videos

    Does 30 - 40% supraspinatus tear require surgery? Can it regenerate itself if left without a surgery? Are there any patients of a young age who had it grow back on its own?
    Or it never grows back because it’s impossible from the anatomy and biology standpoint and with special exercises we only make what is still attached to the bone stronger and the torn part remains torn forever?

    Hi Dr Chris, thanks for the great video! Hope you can give an opinion on this please. I am based in New Zealand & I am felling very disillusioned about what to do. I am getting conflicting advice from different Dr's here about what i should do. I am a 35 year old male. I have received my MRI report and it reads; The right shoulder scan demonstrates a relatively large partial thickness articular surface tear of the anterior aspect of the footprint of the supraspinatus tear 5 x 8 x 3 mm with a few intact bursal surface fibres present. Mild right subacromial subdeltoid bursitis. If you had a patient like this would you recommend surgery or conservative treatment like physio etc.? I would appreciate any feedback on this matter please :) I don't suffer a lot of pain, only for certain overhead movements & throwing a ball hard will i experience loss of power & some pain. I may have had this injury for a while too. Thank you.

      Maverick, Always start with most conservative measures first. So try physio. If that doesn't work, next would be regenerative procedures to heal the tendon in place, which would be advised as shoulder surgery often creates more problems than it solves.     Thank you!! I just finished up with MRI's for both shoulders and was trying to make some sense out of the images :) I know more than I care to about MRI's for spine issues with 3 surgeries behind me...I now have to be fused from my neck all the way down :( I've been complaining about muscle tearing in both shoulders for 3+ years now. It is to the point that there isn't much keeping it all together, so my shoulders pop in and out of the joints while doing simple tasks. I was hoping to be able to see the muscles/tendons...but I'm having a hard time distinguishing one from the other. These are 3TMRI's - whatever that means! Again, thank you for taking the time to help educate us!

      Glad they were helpful. 3T refers to the Magnet strength, and 3T MRI's have better resolution. There are better solutions than fusion in most cases.       It's too bad I can't reply with a pic of my spine....then you might
      understand how bad it is. I have rotational scoliosis that was caused
      by the first surgeon. I had surgery #1 to fuse L4-S1. I came out in way
      worse pain than before and I knew something wasn't quite right. If
      Fentanyl patches and 50mg Oxy's don't even touch the pain...then you
      know it is serious! He wanted to go right back into surgery to see what
      the problem was, but I kept after him to pinpoint what the problem was
      first. CT was done and according to him, he couldn't see anything that
      might be causing me the kind of pain I was in (a Neuro surgeon looked at
      those same CT's and was able to point out exactly where the problem was
      in under 60 seconds) Then came the EMG, which def showed a nerve
      impingement at L5-S1. Surgery #2 was 6 wks after the first, he took the
      hardware out on my left side so he could retrieve the chunk of disc and
      bone fragments that he left behind(this got lodged against my sciatic
      nerve and I am guessing that the bone framents chewed at it for 6wks!).
      For whatever reason...he did NOT put the left side hardware back
      in...at 6wks post op, he felt it had fused enough to give me the support
      that I would need. Yea...right! He fused me straight..with no natural
      curve left in my lower spine. I think that combined with instability
      is what has led me to have flat back syndrome.
      I will go read your links...but unless my insurance covers those kinds of treatments, then I am at square 1 ....again :(

      So sorry to hear all of that! While it may be hard to believe, we hear fusion nightmares from patients very often. Some parts of treatment are covered, like the exam if you're in network, the majority of the cost is not covered by insurance, yet.

      @@centenohome can you cover how much of the muscle bellies the rotator cuffs muscles show on mri. Like the initial skeleton diagram you showed shows all the scapula and musculature but the coronal images on the mri don't look like they go all the way to infraspinatus or teres origin sites.

    I was told I have extremely weak muscles in my left shoulder blade, and there's a gap in my humeral head and the socket due to my instability. They even taped my left shoulder with therapy tape to support my shoulder blade and make it feel "normal". They said I need an MRI, and hopefully, it will show what my next step should be since my current PT isn't helping yet.

      The most effective solution to lax ligaments is generally is generally precise image guided injections of your own stem cells or platelets. An MRI is important, however it can't diagnose instability as it is a static image. To diagnose instability you need test called stress ultrasound.     Thank you so much for sharing. I've been chronically ill for years and been in pain every day for 5 years now. I know I have two neck prolapses (C4/C5, C5/C6) and I'm struggling with my left side in general, all the way down to my elbow and sometimes fingers. Bought my shoulder MRI pics now and it doesn't look good. I was wondering if there should be so much white around. Thank you so much.

    Thanks Dr. Chris for your well presented video on shoulder MRI. However, I would like to know whether all MRIs are read by more than one MRI expert to ensure the accuracy of the findings. From my experience all X-rays on Tuberculosis patients are read by 3 independent radiologists

      No, they are usually not read by more than one radiologist. However, this brings up a critical point. The doctor you see should be reviewing his own interpretation of the actual MRI images with you in the office. If that's not happening (i.e. they are just reading the report), then that reduces the eyeballs looking for stuff and increases the likelihood of missed findings

    hi mr. centeno. do you have any mri's of a before and after disc procedure showing any form of healing with degenerative disc disease? and if you don't do you files of any success in office that shows regeneration of a disc with ddd.

    Excellent video but i have a questionMy radiologist says i have fluid builup in shoulderbut the mri report says that all the four rotator cuff muscles appears normal and the final verdict was " no detectable abnormalities found"Then what was that "fluid buildup" the radiologist was referring?Thanks

      It depends where the fluid buildup is. It could be an inflammed bursa, (bursitis). The bursa is a fluid filled sac that helps to reduce friction in your shoulder spaces.

    Is there any way you could take a look at my scans? The doctor said there was no change in the muscle or the bone structure, yet the shoulder still hurts even after a long period without training.

    Hi doctor, my mra reads that i have a slap tear, minimal partial articular sided tear of infraspinatus and a mildly patulous capsule and posterior/superior migration of humeral head suggesting laxity. Ive tried prp i ts been 2 weeeks and the same :/

    PT student here, and I may be mistaken, but aren't most of these side-by-side images different types of MRIs? T1 vs T2 MRIs? My understanding is that water shows dark on T1 images and white on T2 images, which would explain the color differences between the bones, muscles, and cartilage in the left vs the right images. I'm still trying to get a grasp on how to read these, and confused how you came to some of the conclusions about tears/soundness of tendons and presence of healthy vs arthritic cartilage by comparing their color in these images. Could you help clear this up for me?

    What type of machine do you need to read an mri. My computer will not read mine. According to the report there is a 16mm retraction. Is that already to large of a distance to be repaired?

      eric Millward, No, the Labrum is like a gasket that helps to hold the arm bone in the shoulder joint through its range of motion.     i have tried to MRI my shoulder twice! I cant get through the tube or the high field open systems. I am trying this upright MRI in a few days, my understanding is that the upright style machine is fairly new, my question is, is it still going to show my shoulder and tell me if I have a torn rc?

      While 3T MRI's offer the clearest images, stand up MRI's are a good option when for whatever reason the other options aren't possible. A good technician "should" be able to get the needed images.

    Is a bicep tendon tear(Popeye Syndrome) different from a Supraspinatus Tendon tear? Can they be related? Supposed to be a bicep tendon tear but the MRI said Supraspinatus Tendon tear (not a complete tear)

    Hello, i have just did an MRI for my left shoulder and the only note my doctor told me is that i have a mild lateral downsloping of the acromion.
    How can i fix that ?

      Hi Ysf, The important question there, is Why do you have a mild lateral sloping of the acromion? , and to address that problem without surgery.     Hello Dr Centeno,
    I am an active weight lifter . Since some months, my left shoulder is popping with some pain and i stopped lifting for sometime hoping that the pain would subside.Movements like bench presses are making my shoulder rigid.I had an MRI and the report came out as "Patchy hypertense area in supraspinatus tendon likely Tendenosis with Partial tear". I am currently undergoing physiotherapy for some weeks now but i dont think it is working out. Do i need to get a surgery or continue the therapy ? i would really appreciate your comments on this. Thanks!!

    How about enhansing image contrast with so called shadow I.v. Boluses? And even though this is all about nuclear magnetic imaging, why not change to paramagnetic or ferro or ferri or diamsgnetic imaging. Small amounts, because technic is so sensible. So toxicity, teratogenity, accumulation etc. Is negligible cause we all die and so on you know? Fun ain’t huh…

    I find this very complicated!! I have been told we all have wear and tear in our bodies. So how can you tell if it's pretty bad except for the symptoms that one has. Even in that case isn't it possible that with the right physical therapy, stretching and strengthening the muscles that this will support the skeletal system? I guess unless it is a tear in a place that holds the bones in place. Is the rotator cuff a muscle? It looks like one, but I think that it is taken for granted that it is a muscle.

      The rotator cuff is a complex series of muscles and tendons around the shoulder that help lift the arm and stabilize the ball in the socket (the shoulder joint). A tear is exactly what it sounds like-a tear in the tissue. While some rotator cuff tears result from trauma, the vast majority of rotator cuff tears happen from simple wear and tear as we age. The most common scenario for a tear to occur is simply a slightly more demanding movement than usual. How could that happen? It’s been known for a considerable amount of time that the rotator cuff doesn’t have the best blood supply, but blood supply varies to many areas of the body. The problem is likely a lack of healthy progenitor cells and stem cells to repair the damage as it occurs over time, and at some point with little provocation, the unhealthy tissue simply fails and a tear occurs.
      Most conservative measures like physical therapy should be tried first. If that doesn't help, Regenerative therapy, like precise image guided injections of your own platelets and stem cells can fix most tears that are currently undergoing surgery.       Thanks. I have a frozen shoulder, so my orthopedic surgeon says. He diagnosed me right away with just taking an x-ray. No MRI. I just took an MRI yesterday because I have been in physical therapy for three months. I have improved in the movements of my arm, but the pain is still there. I have heard all kinds of stories from people who either know people who have had this or they have it. The one thing that seems to be agreed upon is the amount of time it takes for this to heal and go away. Two years!!! I can sort of understand this because one time I stopped a fall with my right hand. The pain was not at all like what I am experiencing with this frozen shoulder, but it was there for about a year. All by itself it went away. Now every expert tells me you have to have physical therapy with this. I find it painful and depressing that I have to go through this everyday. My orthopedic surgeon had told me, about a month ago, that he could perform this procedure (in the office) and relieve the pain that I am feeling, but I don't want to do anything like that. I was just looking on here to see what the MRI would look like and yes, how to read it. At least you did clear up for me that the rotator cuff is a group of muscles. I kept thinking that it was part of the bones. I thank you for that. However, when you are looking straight in there, from the side, it seems that there are no muscles. All you do see is bone. Shouldn't you see muscle? Or does the MRI go through the muscle in certain shots. Like I said it is complicated to me. You are use to looking at these. I find it fascinating, but quite unnerving that you can see so much damage in these pictures. That you know exactly what's wrong from them .You must know these things from people's symptoms as well. Yesterday I was talking to someone who told me his friend had a frozen shoulder. He went to Malaysia to have acupuncture (why he had to go to Malaysia for that is beyond me) and was healed. I've also heard that it could be psychosematic. I am trying to find an answer to all this. I also fell off my bike in September and was in the hospital getting checked. I had no broken bones or head injury. My O.S. told me it had nothing to do with my problem. This all started in February. Anyway, thanks for responding. You did teach me something!!!

      By the way, I had the MRI this past Thursday, but have not gotten the results back. They went to my Orthopedic surgeon and my cousin, a doctor who is a pain specialist. My cousin feels that I tore my rotator cuff, but of course that is speculation. I think I shared with you that I fell off my bike in September. I did end up in the hospital. It was a pretty nasty fall for someone who is 62. I think doctors need to take alot of information on people who have frozen shoulders so that we can see if there are certain way of living that might cause it. You mentioned diabetes. My blood work is pretty good. I do have a lot of honey in my tea. I have been trying to leave out foods that cause inflamation, like grains. I wonder if honey is on that list? Guardians of the Galaxy just started. I'm going to watch it. Thanks so much for responding!!!

An MRI is a snapshot while your shoulder is still. For some shoulder issues, a dynamic ultrasound, which is an ultrasound done while the shoulder is moving is a better diagnostic tool.
