    Man. I was once given too high a dose of nitrous, and while it's not remotely comparable to a heroin overdose, I experienced the panic-inducing sensation of having to manually force myself to breathe, because my body was too drowsy/out of it to do it automatically. Very scary. Wouldn't wish that on anyone.

      I only give enough Narcan to improve respiratory effort. I'm not going to fight some mother funker on the way to the ER. Most of the OD's I saw were of prescription drugs. Very little heroin in our area I'm not sad to say. I work on a semi-rural peninsula now. Very tranquil most of the time. A far different (more self reliant and have their shit together) demographic than the city I retired from.

    • @ElGeetaro8929
      @ElGeetaro8929 4 ปีที่แล้ว +1

      You didn't ruin the high. You induced withdrawals, its shocking this isn't obvious to more providers. There is a reason your protocol also probably reads something like " give them enough to restore respiratory drive". Not wake them completely up. You make your own problems when you wake them up too. Give them 0.5mg IV titrated to effect and i promise they dont wake up to be mad at you anymore and you quietly pick them up and put them on your stretcher and transport without any problems. Yes you have the time to put on a NRB or place a BVM and assist their respiratory depression while you get an IV. Their only life threatening problem is respiratory depression, you can fix that without narcan. And before you even try the narcan, seriously try this. Its old school......PAINFUL STIMULI. If they start breathing on their own in response, why are you giving narcan?

    I'm in Australia...My brother broke his leg at home and we just chucked him in the car and took him to the hospital ourselves...he sat in A and E for 4 hours before he was seen....if we had called an ambulance he would have been seen to sooner...but it wasn't life threatening and he could feel and move his toes, so a hop hop hop to the car ...he had to have a plate put in his leg and then he got a staph infection....it took loads of strong antibiotics, 3 further surgeries and 8 months to heal...but it was all free -thank fully.
    My point is some people need to just toughen up..you don't call an ambulance unless it is life threatening or you are incapacitated (like the elderly on the floor)... If we want free medical we need to not be abusing the system...

      It's a misconception that an ambulance will get you in faster. We radio in a report to the hospital and they'll triage them based on our report and more often than not, they will tell us to put them in the waiting room, if they deem it not urgent.

      You've missed the point that this man is not only homeless, but has serious issues with alcoholism, which will provoke and exacerbate his physical condition. He can't, of course, look after himself, but however many times he will be treated in hospital and receive some sort of social care, his circumstances will eventually lead to his death. Would this be from fear of arrest, or do your laws protect those seeking medical help even if they are there for illegal activity such as taking drugs?

    • @niallconneely5645
      @niallconneely5645 5 ปีที่แล้ว +4

      Hi Michelle, Irish lad here to answer your question yes many drug users who end up in ambulances often don't want to go to hospital as they are scared of arrest. However this doesn't happen. Medical staff can only inform the patient about the support services available to them and encourage them to get help but the patients do not always know this. As far as I know the only time they report it to the police is when a violent crime has occured like a shooting or if they think that the patient is a danger to themselves or others.

I was actually in the hospital and it idiot doctor exploded the bulb inside of my stomach even though I was in intense pain the hospital release me I got home before I realized what it happened I called the ambulance to my house to take me back to a different hospital I had exactly 60 minutes to get there where I would have to go to mediate surgery. I run to the ambulance tell them what happened it takes them a few minutes to understand then they go to the hospital without sirens the hospital is over 45 minutes away from my house I could not believe these people I'm on the phone with the Doctor Who is meeting me in the emergency room and yet they still don't see the urgency of the problem. I get to the hospital with a few minutes to spare unfortunately for me because the ambulance took so long and bringing me to the hospital the doctor informed me that they have no choice but to basically redo the procedure in the emergency room and they don't have time to even give me a pain shot they simply rammed the thing back into my abdomen and hold me as I'm screwing it wasn't the best time . I know this is going to sound awful, but when the paramedics are attending a druggie they are not available for other emergencies, the time, money and effort that is expended on druggies is ridiculous, a total waste. If they refuse to go to hospital, leave them, if they relapse and the paramedics are not able to attend or attend timely, then so be it, eventually the over dose calls will taper off and the community will be better for it. Yeah, harsh I know, my it's just my opinion. This disgusts me so much I am not going to finish watching.

      Meh we like to make it seem like there are other things going on. Usually all the other calls on the board are just as dumb so its not really wasting time (doesn't mean something serious couldn't come up but most calls are not what i would define as an emergency). And its worth the time to get the dude to go the hospital the first time so you don't have to come back and deal with him a second time that shift.

    They need to change the law to declare that anyone under the influence of Heroin is not competent to make medical decisions; so they can be compelled to go to the hospital. They should be declared incompetent until cleared by a rehab facility.

      you really have not thought that through have you? you immediately just invalidated all murder convictions where someone was intoxicated because they cant make decisions. no thanks to your logic. Not to mention as a medic, its already a big deal as is to do things against your will and you must demonstrate inability to comprehend person, place, time, event. If you can answer "who is the president?", "how many quarters in a dollar?", "whats your name?", and "what is my name?", you're clear to make your own decisions and in America its kind of a huge deal to infringe on people's freedoms especially when they consumed their drugs and now the police have no evidence to prosecute with because they consumed the evidence.

      @@ElGeetaro8929 nope. The same legal doctrine applies to criminal acts. They were sober when they decided to drink. They should have had a designated driver. Another option is to take a taxi/Uber/Lyft TO the bar, then they don't have anything to drive in drunk. Incapacity for medical decision making can and does have different standards than for legal culpability.

      Yes, such a waste. What really bothers me though are the people who call us because they "need" an ambulance and "need" to go to the hospital because they are out of their pain meds. Now, don't get me wrong. I suffer from chronic intractable pain for which I take opioid medications. However, most people with chronic pain do not have intractable pain. For them, opioid withdrawal is uncomfortable, but it will not kill them. For someone with intractable pain however, not having the opioids the person is used to taking can literally be a life or death kind of thing... Not because the person doesn't have the opioids per se, but because the intractable pain is untreated. Intractable pain can damage the cardiac and respiratory systems, liver, kidneys, etc...so it's a big thing for people like me/those with intractable pain. However, those of us that truly cannot ever be without our opioids are the ones that no better than to let our meds run out in the first place, We know to get our refills in the proper manner at the proper time. I'm sure there have been cases where this hasn't happened, like weather, doctor out sick, etc, but overall, the ones who really need the hospital when they don't have their meds are the ones who aren't going to run out of the meds in the first place.

      depends on how your system is set up. In America we have different provider levels. If both are paramedics then we would just swap who was the lead provider from call to call. If I am working with an EMT i take all the advanced calls and they take all the basic calls. Sometimes they take all calls that day, sometimes I do. Sometimes I take calls for my partner because they just really don't like someone. And if you tell me you cant take a call because you are not comfortable dealing with pediatrics for instance, guess who is going to run that call for sure now. You're going to be able to operate without me one day. I'm not going to baby my partner when we have to baby every patient all day.

Even if it was a good full thickness bite🤦🏽‍♀️🤷🏽‍♀️ why are they not having someone drive them to the hospital? In the USA I know we pay for our ambulance or our insurance does. 2 years ago I spent a month in the hospital and 3 ambulance rides later...... I was seriously ill and one Hospital transferred me to a larger hospital. This happened twice and I was mad i was wasting someone else’s serious medical attention. I almost died and family came in to pay final respects and I don’t remember my children or brothers even being there!!!!! I had mrsa in my heart from a surgical infection to a med port placement and once I was released home 2 days later my lung filled up and I had severe pneumonia while still trying to heal from the mrsa. I then went into renal failure because of the antibiotics. I was 41 at the time. It was just over 2 years ago!!!! Very scary!!!! I don’t even remember the ambulance rides just when they got me loaded up with the pneumonia.

The ER was literally across the street and someone else had driven them to our clinic in the first place. She tried it again a second time at a later visit but thankfully the doc told the patient it wasn't gonna happen. We can't just call an ambulance to act as a transport service. The ambulance is for emergencies... Maybe the ambulances should be renamed the Trauma Service, so people stop calling for mental health and drug overdoses, too. There should be different groups to deal with that.

    Don’t send an ambulance to any OD ! My eldest son OD’d twice when he was a student nurse ! He then got married and after a couple of years they were expecting their first baby ! He was ‘clean’ for a year, then went to see a GP in another town, who not knowing him gave him a whole load of opioids and Benzodiazepines ! He tried to dissolve and inject very strong opioids and died from an air embolus ! 5 days later his wife gave birth to a baby boy ! 5 months later I found my husband dead on the bedroom floor from an asthma attack ! So I lot both my eldest son and my husband in under 6 months ! I was 49, my husband 50, my son 25 ! The baby was dedicated (christened) at his father’s funereal ! My daughter in law stayed living with me for the next 20 years ! I tried to be a father, grandfather and grandmother as I supported both mother and child ! That baby boy is now 23 and has just made me a great grandma 👵 which is wonderful ! But my daughter in law (the only relative who lives locally ) she wants nothing to do with me (she has also disowned her birth mother, who was my friend ) I’m now disabled and live alone ! I’m blessed to have 2 other sons and their wives and children, but they all live away ! Both sons are ordained Baptist ministers and so is one daughter in law ! I have 4 grandchildren, as well as my great granddaughter ! I’m here for some reason known only by God, and by His gift of grace I depend on Christ and the Holy Spirit within ! Without Jesus Christ, crucified and risen, life would not be worth living ! This is my testimony to you all ! My prayer as (if) is if you’re not a believer you have a choice, Heaven or Hell when you die (as we all must one day ) so if you’ve read this far, I beg you at least to think about it ! Ask questions and get someone you can trust, ask them to help ! Blessings to you all xx 😘
