Is it ever explained how The Joker became such an infamous figure in the criminal underworld of Gotham? Usually it just seems like he created his own empire out of thin air, henchmen and all. How did he get this far?

It's true, and also remembering that he is a Master Manipulator and Extremely Charismatic being able to easily read people and Psychologically Manipulate them to do what he wants them to do.

I think John Dimaggio's performance in Under the Red Hood does not get the credit it deserves. It took me a couple of watches to put Hamil's Joker aside and judge John's as a stand alone, and I think he should at least get a thumbs up.

But the joker doesn't have schizophrenia. He's actually entirely sane. He has no diagnosable mental illness. He's just crazy. He does what he does because he knows he's a comic character and he knows it's his job to do it. He's even said he strives to exceed his fans expectations. He has a job to do. And he really loves his job.
