  • Eric Topol, a graduate of UCSF’s medicine residency, has published over 1,300 peer-reviewed articles, is a member of the National Academy of Medicine, and is one of the top 10 most-cited researchers in medicine with more than 300,000 citations. For decades an international leader in cardiology, precision medicine, and digital health, during Covid he emerged as one of the world’s most trusted sources of information on the scientific and policy aspects of the pandemic. In this week’s Medical Grand Rounds, Eric will be in conversation with Bob Wachter, discussing the latest updates in Covid, as well as some of the new developments in artificial intelligence and digital transformation of health care.

    Eric Topol, MD, founder and director of the Scripps Research Translational Institute, is a world-renowned cardiologist and geneticist. His principal scientific focus has been on the use of genomic and digital data, along with artificial intelligence, to individualize medicine. He has written three bestseller books on the future of medicine: The Creative Destruction of Medicine, The Patient Will See You Now, and Deep Medicine: How Artificial Intelligence Can Make Healthcare Human Again. He is the principal investigator to two large NIH grants, the All of Us Research Program that supports precision medicine and a Clinical and Translational Science (CTSA) Award that promotes innovation in medicine. Eric was the founder of a new medical school at Cleveland Clinic (Lerner College of Medicine), was commissioned by the UK to lead a review of their National Health Service, and is active clinically as a cardiologist. Additionally, he is editor-in-chief of Medscape, publishes the Substack newsletter Ground Truths, and maintains a strong presence on social media on Twitter (@erictopol) with over 675,000 followers.

    You both have been such a vital source of information throughout this pandemic, I can't express enough appreciation for everything you've done, especially for people like me who don't work in medicine but are desperate for reasonable, experienced perspectives. Thank you so much for everything you do! While you don't owe us your mental health by any means, please know there are far more of us who are so deeply thankful than there are trolls!

    This was incredibly helpful to me as I find it harder and harder to find trustworthy information as to where we are in terms of variants, vaccines, and how to access risk. I have shut down my Twitter account but check every few days, looking specifically to see if the two of you (and a number of other medical experts) have posted anything new. This discussion has truly brought me up to speed. I thank you both for continuing to try to cut through the misinformation.

    I really appreciate what you do. I follow you on Twitter but now primarily read your substack because I don't want to give Musk the views. I refer to you so many times when trying to help my family and friends navigate the pandemic, especially now trying to understand the new studies coming out about Long Covid and how it impacts our behavioral choices moving forward. Just wanted to thank you!

    The mask issue is why doctors are not scientists or engineers and should not be in charge of public health.
    As a safety engineer for asbestos removal crews in schools I had to send workers home constantly for facemask violations. I loved smoke testing hotshot guys "thinking" instead of following protocol. Masking is hard. Touching your mask or adjusting it is a fail. Facial hair is a fail. Looseness is a fail. These guys took a 3 day course on masking with hazmat and dangerous materials in mind and they still needed me to keep on them daily. I never saw one person during the whole pandemic that wore an adequate mask correctly. From an industrial viewpoint it was theater at best.

    The Cochrane report describes population effects of masking, not individual effects. How anyone fails to see the difference is hard to understand. Any direct medical provider knows that we care about our patient's individual risk more than population risk. We know that masking as a populace is impossible to control. Protecting the vulnerable individual means taking direct action, and unfortunately that means that these individuals need to mask, and the people that interact with them need to mask.

      Population effects should allow you to make policy. I think the Cochrane report shows that masking policy was garbage. Fauci knew this and then went 180. Go ahead and wear a mask; as far as I know it's not illegal.

    Thank you for this talk! And to echo everyone else, please Dr Topol - don't quit! Just block a bunch of them or just ignore. These trolls are the worst. I don't understand any of it, nor do I understand what is the malfunction with Vinay Prasad. What is the matter with that insane little demon? What a horrible thing he is.     Did crioglobuline like antybody occur after SARS COV2 infection?
    If crioglobuline occur,that can increase blood viscousity at low blood temperature.
    To decrease blood viscousity may be cysteine compounds can work by decreasing S-S bindings?(with secondary effects of overdosage)

      I just asked this question of Anne Sosin - a public health practitioner at Dartmouth College. I know many, many people who have had Covid and never reported it to the state, so that number does not exist. Where can we go to find out hospitalizations and death rates for our state (numbers that should tell us something)? I want to know these numbers to make informed decisions.

    37:36 why not suggest working on whatever is the immune system problem? Could it be from taking another drug that is causing a poor immune system? That's a side effect of so many drugs.
    No mention of that at all, how do you get away with giving medical advice without seeing the patient? I've seen many doctors prefacing their comments online about doing not doing that.
    Not one mention here about nutrition.

    Keep up the good work, you both. I haven’t gotten Covid either and have been masking for the last three years. The information is not just useful. It also helps justify why I’m still masking and taking precautions when dealing with others who think Covid is over.

    A sane and informative discussion. Excellent information. As with regard to vaccine and boosters, I too have had 5 shots. I am happy to have had them as an option; but I am suspicious of the long-term effects or unreported side effects. While there are many reasons I have various health issues, I do have some reason to wonder about the vaccine effect on kidney function and perhaps liver as well, and the nature of the vaccine on exasperating dormant or small issues. I have noted that drugs such as repatha have indeed impacted my LDL, but not without raising ALT, taking a Repatha hiatus the drop in the ALT to well into normal was revealing. Couple that with the no LDL is too low crap that cardiologists seem to love, like an 11 is fine, is quite frankly nuts. So, it is complex, and we have not had full honesty, nor has the government required enough questions to be answered relative to the vaccines, specifically, the mRNA ones. Further, the availability of data for older persons as to efficacy is kind of really really hard to find, IF it really exists at all. It is always suspiciously buried and hard to get. There is seemingly zero out there about people who actually get covid and vaccine outcomes as opposed to BS measures per 100K of population. Further treatment options other than Paxlovid are nearly non-existent. Why no better treatment options? Why no improved vaccines? Why no prescribed testing cadence for non-presidents? Guidance is all half-assed at best. Hell, I never heard of a testing cadence until the uber geriatric got elected.
    I congratulate the two gentlemen in the video for having a real discussion and being honest. Great work, despite the hordes with pitchforks and torches, albeit there are indeed some Frankensteins out there, so there is that, these gentlemen should not be the mob target, not that there should be a mob at all.

    In the world of data driven, data trained systems, data cleanliness is critical. Many times the possible new science or intriguing findings (not replicable) is being driven by dirty data or mistakes in data process/processing.

    You're both extremely helpful, offering tremendous benefit. All the observations about Twitter and alternatives are completely accurate unfortunately. I hope you don't stop, but I get your substack letters, Dr. Topol, and have found little use for the Twitter workings since the new owner made changes to enhance his own warped perception of scientific information to favor politically motivated conspiracies instead. Thank you for the guidance. The comments on the Cochrane study on mask were very on target as it does defy common sense itself and will of course, do harm.

    Per Dr. Topol's suggestion, I tried to get a second bivalent booster at CVS and then at Walgreens. I explained that it's been almost six months since my first one, I'm 75, and have numerous comorbidities. Neither would give me one because the CDC has not authorized a second bivalent booster for anyone. I think it's unfortunate that this video is raising people's hopes of getting a second bivalent booster when this is in fact not possible, as far as I can tell.

    Almost nothing about long covid. Millions of people are disabled. People should be informed about severe consequences of COVID infection. More research is urgently needed. We need cure for long covid. Please, work on mitochondria dysfunction.
