    as someone that’s coming up to the 1 year PO mark for a full quad rupture on my right leg i can tell this is a very honest video from these docs ! This surgery takes A LONG TIME to heal properly and if you do something ‘excessive’ too early you will pay the heavy price like these good docs explain

    Hi Dr's Thanks for your well wishes. I am 7 weeks post op now and progressing nicely following my Surgeons protocol. Strength and mobility is returning with about 80 degrees flexibility now. There IS life after this injury! Slow and steady with some solid conditioning will get me back to previous activities. I am a very active 62 year old and I'm missing those activities big time! Patience and careful execution of the exercises is the key! Keep coming with the solid advice. You DO speak the truth! Thanks Guys!

    I drive an 18 wheeler and had this surgery done a month ago, due to a work relatedaccident. I'm afraid to go behind the wheel again and from experience from a previous ACL surgery to my other knee I know first hand how your life changes. I had a great surgical team but you're never quite the same after surgery no matter how successful it is. Take care everyone. Thanks for the video Docs. Excellent.

    I've now done this twice, my left leg 11 years ago on holiday slipped on a path, unfortunately not diagnosed in A&E and had to wait 4 weeks until repaired. Fortunately worked hard at rehab and the knee function returned to what it was. Would really recommend going backwards on a cross trainer, seemed to work to stimulate the vastus medialis that had the most wasting. The second time was my right leg only 5 weeks ago, slipped on some grass while walking to work. Fortunately recognised (made the diagnosis myself before got to A&E) and repaired within a week. The one thing I would say to anaesthetists out there is that for the first 24 hours post repair it was EXCRUCIATINGLY painful. I don't know if it's the drilling through the patella or the re-stretching of the muscle but I was in absolute agony BOTH times. I wasn't prescribed enough post op pain relief, but after 24 hours it settled down and I was just on some paracetamol and codeine at night. I was 49 and 60 with no other risk factors but I did have some left patella soreness for a few years before the first one which made me switch from running to cycling.

    I am coming up on 6 weeks from surgery to repair bilateral ruptured quadriceps tendons. I was rushing down our mud room steps and landed on my left foot with a flexed knee and felt my knee give out, then I guess I tried to catch myself on my right leg and the right knee went also. It was terrible pain while my legs were bent, but once I straightened them with my arms I was pain free. I actually saw a strongman at the Arnold’s classic rupture both of his quadriceps tendons doing a log lift, so I assumed I did the same minus the 420# log. So my daughter called 911 and I had surgery the next day. I’ve been in leg braces locked at zero degrees since, but at 5 weeks I was allowed to sleep without the braces and I start physical therapy this Friday.

    Had a total knee replaced, three weeks in I was doing great....then 6 am missed the last step. Tore my quad. Surgeon said it was the worse he has seen. I am on day fifty of recovery. Pt says I am doing better than expected. Trying to get my strength and stability back . I am 63 and concerned about my future. I have not been ready to read or research this because of the sheer trauma of the event, but now am doing some research. I do not find much out lthere on this kind of injury after total knee. Anyway its been an adventure!

    Thanks for sharing this video with all my knee problems on my left leg. When gardening I have to keep it straight when I sit down because of my cyst and for kneeling I don't try to make it a habit for that. I had the patella tendon repaired for my acl long time ago that was my second surgery. I use the walker instead of the crutches when I had it done.

    I can say personally that riding a bicycle regularly in my later fifties has removed almost completely any knee discomforts. However, there has never been any traumatic injury to this area of my body... unless you consider aging physically traumatic in some way.

    Such great info. I ruptured my quad tendon almost 2 years ago after I fell during a hike. It was an extremely painful injury and thank goodness I was with people as I was only half way through our hike and had another 1-1.5 miles to go. I made it out with help. As long as I kept my leg straight I could walk. I went to the emergency room and they thought it was a tear and I went to the orthopedic the next day and they did the straight leg left which I could not do, they scheduled my surgery 3 days later. And then told me after surgery it was a complete rupture. There is a specific protocol for rehabbing this injury and I followed all of the rules and was told I was pretty much healed within 4 months. I was lucky since they told me the standard recovery time was 9-12 months. I haven’t hiked since but I feel like my quad is back to pre injury shape. Crazy thing is now my other knee is the one that gets sore when working out. I was only 44 when I was injured. I basically slipped and fell and landed on my quad with my foot under my butt.

    Hi to my favorite drs I just saw this video and it sounds very painful and I'll try to be careful as I dont want that. Unfortunately I have cancelled my back surgery as I went for my preop and they did a e k g and found that I have Afib and now on heart meds and blood thinner 😕 I was in the hospital 2 days and nights and home now . I'm disappointed but I'll eventually get it done and want to say thank you for your good luck wishes you sent me I really appreciate and love you both as your such caring and great drs.     I was in a motorcycle accident when I was 50. Tore the quadriceps but they didn't catch it at the hospital. I could walk but if I bent the leg at all it would collapse. Went back for a checkup and they sent me to a orthopedic surgeon. He poked around and asked if it hurt a couple of inches above the knee. I said yes and he told me what was wrong. He said at my age it didn't have to be fixed. I was still playing competitive basketball and wanted it fixed. A few days later he fixed it. I was walking with crutches but not using them 3/4 mile to my gym in a few days. Also had several busted ribs and generally beat up. That was in February. That fall I tried to play basketball but after one time it hurt really bad to climb stairs so I took a year off. The next year I was just as strong as ever. I played until I was 62. Playing at those ages of course your knees hurt some but that one never hurt as much as the one that was never injured. I will always believe that my willingness to work it was why I recovered from it so well.

      @@mayamoon6819 I put 5 lb ankle weights on and did leg lifts. Seemed to help. Soon as the surgeon authorized it. I started working on being able to bend the knee.

    I'm currently recovering from a complete quadriceps tendon rupture, that was treated conservatively. Leg was immobilised immediately for an extended period (9 weeks total) and I've been doing PT to regain ROM. I can SLR and I've full use of the extensor mechanism. I do have 10° of extensor lag. Working hard to hopefully fully recover!

    Thank you for an excellent video. Fifteen years ago at age 47 I completely torn both of my quad tendons when tripping over our dogs and landing with bent knees on the hardwood floor. Both knees were surgically repaired the following day, followed by a couple of months of PT. Have always been an avid exerciser (weights and running). No longer run but still do resistance training. Is there any reason, in your opinion, to avoid performing barbell squats with heavy weights provided form is very good (in excess of bodyweight)? I do not do leg extensions, sissy squats or other exercises that put lots of stress on the knee joint. Thank you.

    My left quad tendon has ruptured 3x since 12/21 following a left full knee replacement 11/21. Ruptured 12/31/21, 5/30/22, 8/19/22. I'm waiting to consult with an Ortho specialist in a couple days who was referred to me by my ortho doctor. He said my repair requires someone with more expertise. I pray the upcoming 3rd surgery will resolve this issue once and for all. The pain is excruciating whenever a rupture occurs. I don't understand why this keeps happening. Praying 3x's is a charm with the 3rd surgery.

    Had never heard of this injury. Really good information on the how's of it happening, what to expect to experience, what you do, then what we need to do and what to expect in the future. Is there any physical therapy that usually goes with this? In your practice, how many times a month (ballpark range) do you see this? Thanks for another great vudeo!

      PT started after the 7 week mark for me after being cleared by my Dr. Slow and steady PT is required for the first few months just to regain some range of movement and balance. Hardest let is breaking up the scar tissue before you even can start any type of strength training

    Yep, misjudged the height of a kerb as I stepped off the sidewalk. Also suffered a complete fracture of one of the bones in my leg below the knee. I need a cane to walk and never fully recovered full use/stability of my leg. About 9 months after the surgery, I fell again in the street. The sutures and repair held up, a reassuring outcome of my surgeon's handiwork.

      @@TalkingWithDocs thanks for getting back. I've had my PCL injuries since the 80s. I was then told there was an operation but it doesn't always work. So have lived with it since. Most of the time I can manage but occasionally I have use a knee support.

    Hi guys. This video came up in my work computer feed. Amazing how that happened because I was not searching for it BUT I was really curious about ROM changes after full quad tendon repair. This video was really useful because so few people seem to know about this surgery. thanks for putting the content out there.
    In the video, you talk about a period of recovery of 6 months to 1 year. I am curious about flexion ROM changes over time for average folks. There is lots of info online on ROM over time for TKR but not for quad-tendon repair. Based on the comments below it seems many folks are below 90 degrees at the 6-8 week mark. Do you have a sense for reasonable target ROM for your patients as they recover at different points in time (e.g., 6 weeks, 12 weeks/3 months, 6 months, 1 year)? I read that one might expect to gain almost the full ROM (120-130) if they had that preoperatively, but those were from sources that were checking back in with the patient more than 1 year after, that article suggested that as long as a patient continues to progress all is OK: slow and steady wins the race. Is that the way you two look at it for your patients? Do you expect that over time close to the full ROM might be recovered if the patient continues with PT and stretching and strength?

      Yes usually close to full range and strength. Sometimes a small quads lack. Usually most motion is obtained by 3-4 months. Strength often a year

    Thanks so much for this video. When you said this injury should be detected early, it brought tears to my eyes. My knee has been hurting me for months. I have a large dent above my knee as well. My rheumatologist said it appeared normal (never had a dent before). The next month I advocated for myself and got an MRI. Results: Partial tear medial quadriceps musculotendinous junction, complex Focal chondral tear media patellar facet, chondral flap, mild fibrosis posterior margin Hoffa's fat pad, and a 5.4 mm intercondylar roof ganglion. Still waiting to hear back from the doc. Since this is a partial tear, do you think it's too late for me to have good results? Or maybe this is just a PT outcome???

      Hey! I am currrently recovering from a partial quad tendon tear. I ended up doing Physical Therapy after getting checked out and seeing that everything was aligned. I did Astym therapy that has helped put the healing back into play and helped a ton. I’m a cyclist so I have had a hard time with this and have had to reduce my endurance training and pretty much just spin on my bike for now. Also I started taking magnesium and collegan peptides that seem to be helping recovery.

    I just found your channel it’s great I was worried I did it again
    tore my quad rehabbing from a TTT about 20 yrs ago doing a wall slide with an exercise ball between my legs. I was medicated for rehab so I really didn’t feel anything
    Lol walk it off well I did for about 2 years and then
    The doctor thought I needed my patella removed
    They actually went in and found out I ruptured my quad and it pulled all the way up to my pocket
    I worked in the kitchen golfed played sports my ortho looked at me in the hospital room and shook his head at me.
    At this point in my life it is what it is good days all the way to terrible days and Arnica gel is one of my best friends
    Oh well that’s my story

    I recently torn my quadricep, I'm currently waiting on surgey. I watched your video and noticed you guys mention a few times you can't straighten your leg. In my case it's extremely painfull when I bend my knee. What's the difference in my tear . That causes the tear to only feel, better when my leg is straight??

    I partially tore my right quad tendon about 13 years ago when I was teaching incarcerated youth. One of the youths jumped on my back unbeknownst to me. I pushed him backwards up against a wall and I felt my quad give like an over stretched rubber band. Earlier that year I had been on Cipro for a lung infection. I think the Cipro made me tear it.

    really informative. I fully ruptured my QT 19 months ago at age 48. Fit and competitively athletic. It has been a devastating injury and recovery. Followed the post op instructions to a T. Did the PT till 6 months post op. But it never felt quite right but was told to give it 12 months. Just before the 1 year anniversary it swelled and became stiff and painful after a light jog. Went for a second opinion and new doc noticed a small dimple over the scar. Ordered an MRI and revealed a partial re-tear 30-50%. Second doc said I had good strength and function and wanted to avoid a new surgery. I was referred for PRP and more PT which hasn’t helped much at all. My questions are 1) what criteria or how do you determine whether a partial tear should be operated on? 2) Do you think stem cells treatment can’t help someone with a partial tear like mine? After the PRP I was told stem cells taken from my hip is another option to help strengthen the tendon and improve things. Lastly, my second doc said he is against keeping the leg fully locked for six weeks. Said leg should be locked when moving about but passive bending and PT should be started sooner to prevent scar tissue which he thinks actually adds to the risk of re-tear once bending is finally started 6 weeks PO. Thoughts at that other line of thinking? Thanks

      Hi M. So it is hard to say without examining you etc. so if you do not have full extension that is often an indication for surgery. If you have significant pain or instability that would be another reason. PRP maybe worth a try. Not a lot of evidence at this point. The risk would be time and financial.       @@TalkingWithDocs Thanks. I have full extension, no lag, good strength, no real instability. I am able to cycle, and do elliptical, take long walks on most days . I have moderate pain that comes and goes and lots of popping in the knee. Good days and bad days. Biggest issue is daily tightness and a feeling like there is a tight band just to the upper lateral side of the knee which looks a bit like a slight knot which is not there on the good leg. I do a lot of daily stretching but that tightness/discomfort is sort of always there. I have to wear a neoprene sleeve to exercise and when I’m on my feet a lot (which despite being very thin makes a huge difference and reduces that tightness sensation). So I would say good strength and stability, moderate discomfort that is intermittent, but I get around fine just can’t do the running and sports I used to love to do before the injury. I guess the problem with this injury is even with a second surgery (given the success rates and as you point in the video - what a significant injury this is) there’s no guarantee that a second surgery to re-repair would result in a better outcome or mean that I could return to running and the other sports I did pre-injury.

    Very interesting, May I ask you a question regarding the quadriceps tendon? I'm not after any medical advice; I want to know if any type of treatment is available for a thickened and irregular quadriceps tendon. In 2017 at 52, I had a total knee replacement. I only had pain at that point because I was born on bone immediately after the operation. In the following weeks up until now, it was evident I had a problem with my tendon. Still, nobody would take any notice of it now. Five years down the line, I am in constant pain, and I have the aforementioned lads didn't above my knee Going into my thigh.
    It would be fantastic if I could get rid of this pain in the joint itself is perfect, just the pain from the tendon. Thanks, docs.

    Once upon a time , September 2015, I was taking out the garbage on a crisp fall morning. The sky was very clear and dark as the moon had already set. As I was carrying my trash and recycle bin I could not help but look up into the sky and marvel at the beauty of all the stars that were visible. Surely, I had to be at the end of my driveway, so I turn to my right, take a step and plummet into my ditch. I experience a sharp pain in my knee, but I am a pretty tough guy (so I think) and dust myself off and head to work. Once at work I realize that I can't straighten my leg and head downstairs to the ED ( I used to work at a medical center blood bank). And sure enough a quad tendon repair is diagnosed and surgically repaired the next day. And it was an arduous task to recover. My surgeon kept me in straight knee brace for 10 weeks and then the rehab begun. It was probably one year before I felt like I was whole again. Now I am 63 years old and retired and have gotten back to running with a brace, and have almost the same range of motion. Thank you for your excellent and entertaining disbursement of information.

    About a year and a half ago I fell walking up my LONG driveway - and ruptured my Left Quad Tendon. Due to other problems it was three weeks before the first surgery was performed. And it completely failed 8 weeks later. Second surgery also failed. Third surgery included use of a Graft and unfortunately is has also partially failed although they will not do another surgery because it is A small tear in the middle of the tendon. Worse - six weeks after that failure I had a major Staph infection that required a clean out, 3 months of Anti-biotic infusions - and according to the Infectious disease doc I might be taking Anti-biotics for life. I was told I would never reach 90 Degrees of flexion - but I managed to get over 100 to about 105. But the problem is - the knee area - even without the braces - still feels like that is brace holding it together and some times that reduces my range of motion. I was told that is "not unusual" and it eventually would abate but almost a year later it has not. I still get swelling in that leg as well. (I wear compression socks for that).

    Thanks for this video. I'm in UK and currently off my feet due partial tear to my quad tendon that's has pulled off part of a bipartite patella. I also have lateral and medial ligaments. It's pretty painful. I'm under a knee specialist who has put me in a 30 degree Leg brace for 7 weeks before commencing physio.
    Is there anything I can do to aid recovery? Thanks.

    Hey Docs about two months ago my knees got weak due to overuse from biking and getting back to early etc. After taking a needed break I could still put weight on both my knees despite a bit of leg atrophy and got back to walking carefully with knee bands. I unfortunately took what i assume to be one wrong step and suffered a sharp pain in my right knee but didn’t fall. Afterwards there was no visible swelling, redness or bruising but I haven’t able to put any weight on my right knee for over a month know and have to use crouches. Putting too much weight on my knee causes an “empty feeling” inside with some pain but no throbbing the pain is hard to localise but if anything its in the front or inside the knee. I experience no from of locking but my knee feels vulnerable when i try to extend it fully and therefore always lay with a pillow. I assume this is because of their hyper mobility. After initial rest i have been doing isometric physiotherapy with no pain, on my doctors order for a couple of weeks now which keeps improving my left knee which I am still able to put weight on, however my right knee doesn’t seem to improve at all. My doctor wont give me a referral to a orthopedic surgeon before trying more physiotherapy. I have a hard time finding out what’s wrong with my knee as I don’t recognize any of the symptoms. Hope you can help me understand what is wrong. Sorry for the essay and thanks in advance.

    My rupture occurred after knee replacement. I tore it a little in the hospital after surgery, plus the full length knee brace was heavy so trying to lift my leg to roll over in bed it completely tore off with a loud "pop" and excruciating pain. that was almost two years ago and I don't have strength in my leg and still have pain specially above the knee.

    Victor Oladipo had this happen to him in a game and he had a gap vibisle just above his knee cap. NBA athletes are always injuring something. I don't think there is even one player that did not hurt himself badly in his career.

    I came home from having a total knee replacement and 10 minutes later fell in my bedroom and tore my quadriceps tendon. I'm having surgery in a couple of days. I can't imagine what this new injury is going to do to my knee replacement recovery. I'm so bummed out.

    YOU NEVER MENTIONED WHAT A PARTAL TEAR DOES. I FELL 2 YRS AGO AN HARDWOOD FLOOR WENT FLYING FORWARD TRYING TO KEEP MYSELF UP BUT TO NO AVAIL. HOWEVER MY LEG WAS BLACK FROM TOES TO KNEE AND A HEMATOMA AT SHIN JUST BELOW KNEE. I WAS SENT FOR XRAY AND DOBBLER.
    I HAVE BEEN I PAIN MORE AT NIGHT AND WHEN MY LEG MOVED A CERTAIN WAY I WOULD YELL BUT ONLY LASTED MINUTES. I FELT I TORE SOMETHING BECAUSE INSTEAD OF GETTING BETTER IT WAS WORSE! I WANTED A MUSCULOSKELETAL ULTRA SOUND BUT WAS NEVER TOLD TO GET ONE. MY KNEE
    STILL HURTS AND HARD TO GO DOWN STAIRS. SHOULD I STILL GET THAT ULTRASOUND OR WOULD
    IT BE TOO LATE ANYWAY? SINCE 2YR COVID AND SORE KNEE I HAVE NO MUSCLES LEFT AND HARD TO GET UP OFF CHAIR!! Yikes! I WATCH ALL YOUR VIDEOS AND LIKE LEARNING MORE . REGARDS FROM MONTREAL

    Fell down some stairs 10 days ago. Got diagnosed 7 days ago- full tear on my left leg. Got the surgery done a 6 hours ago… 😭 I really needed to get some of this insight to really understand the gravity of what happened and also communicate this to people who were still thinking I could walk or physio it off.
    Back burner superficial me is also thinking lordt what am I gonna do with this scar and will I ever be able to walk in heels again? 😭 FYI I’m 37yo.

      Thanks for reply. Went to my ortho surgeon today and learned that my CPPD has moved to my left knee. Previously, this infuriating causer of pain had been limited to my right knee. Now, I am on Day 13 of excruciating pain bending and straightening the knees. It isn’t weight bearing. I had spent about 90 minutes kneeling or sitting while scrubbing a wood floor for the holidays. The next more, upon waking, I was in the worst knee pain of my life. I was diagnosed with CPPD in right knee nearly 19 years ago. A year ago, left knee clear in Xray. Today brought “such digital joy” I. The name of CPPD.
      My pain is that I can’t bend legs. Ince bent, I can stay there for a bit. I am now hearing clicks and pops above the knee cap…

    My quadriceps tendon is deteriorating, went to surgery and he removed the "dead" part of it, that was 2 years ago and now I feel like my quadriceps tendon is trying to completely tear apart, it hurts to walk, sit and lay down. 2 weeks ago it started and it hurt to sit and lay down, now it also hurts to walk, I feel like my knee cap is shaking and can also feel the quadriceps tendon, I am trying to be careful before I go to my doctor but how careful do I need to be ? because he told me after the surgery it was very possible for it to tear.

    4:44 - football in mud slipping while carrying a defender on you back and another dives at the legs will do it also…. Wish my specialist didn’t clear me after 2 weeks and actually performed an MRI instead of just looking at the knee. Can’t afford to take 6mo off from work as an adult now 🫠😂
