The Associated Press news agency is quoting three US officials as saying that a preliminary assessment suggests the missile that hit Poland was fired by Ukrainian forces at an incoming Russian missile.

Earlier on Tuesday President Biden said it was "unlikely" the missile that hit Poland was fired from Russia.

With the benefit of hindsight, I sincerely hope that lessons will be learnt this time and Polish air defence procedures will be reviewed thoroughly and vigorously to ensure that this doesn't occur again. Prayers, vigils, resolve and meaningful dialogue.
Russia and (many of) Ukraine's unguided missiles make it even harder to guess where they're going to land, as they don't have a predictable projectory.
With Russia's/Ukraine's inaccuracy and Poland's proximity to Ukraine, it wouldn't be until the last second in which they realize it's going to strike them.

Even if this was an accident (and most probably is), NATO must use this opportunity to provide their most advanced missile defense systems as well as long range missiles to hit Russian military targets from land or sea. Also, the time is now for a long awaited no fly zone and a clear "red line" when it comes to bombing civilians and infrastructure.

Poland has cool air defense, which allows a rocket to fly 10 kilometers calmly in its own country and let the inhabitants of Poland be disgraced. Cool NATO military experts who, knowing the model of the rocket, knowing the maximum distance of its flight, and at the same time cannot determine from which country it could fly and which not. Radars are also cool, which can not determine the route of the rocket.

I've got an idea.
Lock Joe Biden, Vladimir Putin and Xi Jinping in a room and tell them they've got four days to rach a peace agreement and also agree to scrap their nukes and call their forces off. If they exceed the deadline. Take them to another room make them wear bulky vests and tell them that they're wearing suicide vests with control's attached and telll them that each control is connected to a different vest and the buttons set off the explosives an one of them will get blown up so the oteher two'd have to do the same (there aren't any and the 'detonators' are just fake but they don't know that) and see how willing they are to push the button then.
