They went through all of that roleplay to make him remember something he didn't want to know. It was good intent by the doctors, but they were blinded by their egos, and the outcome is a horrible reality for DeCaprio. It's not electric shock treatment, but the pain and suffering it causes someone is psychological torture over a lifetime.
I get why he wanted to be lobotomized, but had he chose different, a lot of patients could have been helped after proving the doctor's methods right. So his ending line and thought process was all the more ironic.

watch the start, when he wakes up on the boat, there's handcuffs hanging from the celling of the cabin, He's always been Andrew, at the end he accepted that his life is destroyed and meaningless, hinse the symbolism of the dream where his wife and home turned to dust.
WHAT?! Um…no. It all happened like he finally remembered. His wife killed their kids and he killed her and went insane. He made up an elaborate reality to cope with the pain and finally decides to give on and just kill his brain in the end. He can’t deal with being a monster. Watch it again. You completely missed the story.
I find it interesting that some people who watch this movie don't get that he knew what he was choosing at the end.
