    Can't Vlaakith just be like 'I wish everybody rebelling against me was dead"
    also how does she not have crippling back pain from just sitting like that on the throne forever

      Long story short, she can't...Wish isn't a "be all and end all" type of spell, if your wish passes a certain point where a 9th level spell wouldn't be enough to fix your issue a dire consequence is inevitable. Nature of this consequence is unpredictable by nature so no one dares to cast a wish spell of that degree.

      I don't really understand power scaling in DnD I just remember that Vlaakith wishes your whole party dead if you piss her off

      The Wish spell is not a perfect spell. Only true gods like Mystra, Shar, Selune, and Tymora (and other gods) can cast the Wish Spell without fear. Vlaakith is a powerful lich, but even she isn't immune to the dangers of the spell.
      The Wish Spell acts like a miracle, and there's only so much a miracle can achieve or be able to do. It's unpredictable; it's limited, and costly to the caster. Vlaakith wishing to end the rebellion would not do her any good. It'd leave her crippled and weak, and the Netherbrain's crown could very well push the limits of what the Wish Spell can counteract.
