A question sir, patient has malignant lump fixed to skin, which makes it T4 (locally advanced), would not it be preferable to do neoadjuvant first so that the possibility of leaving tumour at skin edges may be minimised? Regards
Yes, chemotherapy to downstage the Tumor, 2, to reduce the bulk of the Tumor, 3, to treat micrometastasis, 4, systemic therapy is a must, 5, to check the response

Judging from the point of view from western medical colleges and their norms on case presentation, the case presented by this candidate falls short on various levels. The case presentation is a systematic procedure that touches on many aspects starting from chief complaint, past medical history which is subdivided into surgical, medical (further subdivided) etc, to psycho-social history. Although it may feel as if though that some components are not relevant, they are routine and should not be undermined before going to clinical examination. However, from my point of view, it is odd that the initial case presentation did not achieve this but rather rushed to investigations. In addition, some aspects of investigation were also left out including molecular/cytogenetic studies which may have aided in hormonal therapy. Although the case presentation is short, it is not complete.
Definitely Yes .... But they have presented this as a short Case. If it's a Long Case then we can follow the proper format whatever chronology is there.
This is for a short case. Which entitles you spending 5 min with the patient and do a specific clinical examination as the examiner commands and do any associated examination. What you are talking about is a long case where the candidate has 30 mins to elicit a complete history, do a complete examination and then present the case just like you pointed out.
