NASA has had failures, and kept going. Debris from one explosion, in which astronaut lives were lost, landed all around my area. I do think NASA took a pause before launching again to find the mistakes and fix them, and FAA had nothing to do with that, but SpaceX being civilian, FAA has to make the grounding call official. But they shouldn't stop learning and getting better.

SpaceX/Musk business culture: Arrogant and Reckless. FAA should have imposed tighter restrictions on SpaceX after they almost destroyed their own launchpad on the first launch and jeopardized down-range safety. Tesla and SpaceX only successful when Musk is banned from the factory or from micro-managing operations. He has no business being involved in US government, much less evaluating its efficiency. USA/NASA should improve the technology used by Spaceship One, Virgin Galactic, at least to get astronauts to and from the ISS. No need for dangerous complex extremely expensive boosters when a vehicle can be launched from the underside of a 747, Airbus, or specially designed plane.

I seem to recall that SpaceX had a number of failures due to fires in the engine bay on previous launches of their rockets.
One thing bugs me about Starship: it's designed to hold many passengers but has not launch escape system. I wonder if they are relying on Starships engines itself to get itself out of trouble if there's a malfunction of the Falcon Heavy booster.
