If I'm not mistaken, the song's opening lines about swallowing needles are a reference to Japanese culture, specifically a children's rhyme that goes: "Pinky promise, if I lie, I will drink 1000 needles and cut my pinky." which shows how much they place value in keeping and honoring one's promises to others. So glad that you guys are covering reactions to this song ^w^
Japanese language is really attractive, even though it does not follow the usual English convention (S-V-O) but gosh, when they get proper translated you will see how their words put together can be so poetic and symbolic

Though this song came out a while back, It is becoming more popular due to a 2020 live performance that has just recently been shown on TH-cam. People claim this live performance was "better than the original" and if you like the song, I highly recommend that you listen to it!

In Japanese, there’s a phrase called yubikiri genman uso tsuitara harisenbon nonasu yubikitta （指切りげんまん嘘ついたら針千本飲ます指切った）
This is a phrase you say when you cross each others pinky and swear you’ll keep the promise.
The phase means, I will swallow thousand needles if I break the promise.
This is why the lyrics has needles in it.
