    So the "moral of the story" for an asthmatic is to try to be less anxious during a flare-up - and to take measures to avoid those things that trigger asthma. Each asthmatic should learn what his triggers are, and to take the medication his physician recommends at the right times and the right dosages.

    Thank you Dr. S. after watching your video part one I started trying to breathe out more. I have had to take steroids for this and can't stand them. Yet, they usually decrease the inflammation and open it up. This is a bit too technical for me but it was interesting. Thanks again.

    Pre bipap/intubation heli-ox(helium and oxygen mix) sometimes helps with air movement. Post intubation anesthesia vent/gases sometimes helps to relax airways when other measures are not working.

    There are a few parts of status asthmatics that are topics for advanced management. For regions in which ECMO availability is scarce, it's not always an option to ship these patients off. Then the question becomes how much driving pressure is 'tolerable', what pH is 'bearable', and whether the auto peep can be overcome sufficiently by increasing fluid volume. It gets individualized to the bedside numbers. In rural medicine I've had to allow peaks of 90+, pHs of 7.0, and paralytics, but with patience we've always been able to get the person through the storm without exception.

    This helps me understand my asthma exasperation. I have has ongoing bronchitis and asthma that is being exasperated by people I cannot control smoking around me. I am stuck but trying to control it. I was just emergency referred to a lung and respiratory specialist.😢 I hope they can help. I wake up a couple of times nightly coughing not able to breathe. 😢 it's pretty terrifying. My attacks sometimes last up to 15 minutes.

      Make sure you check for down feathers in your pillows and comforter, pets in your bedroom, sulfites in processed food, and allergies. GERD may also play a role.

    What do you think about using breathing tools to increase the lung capacity? You can do it 5-10 min per day.

    The only difference between hand bagging and a mechanical ventilator is accuracy and you don’t have a machine telling you how high your pressures are and how big or small volume you’re giving the patient. A ventilator is simply a $30,000 manual resuscitator that alarms when you give volumes, pressure or minute ventilation higher or lower than what you set in the alarms.

      @@MarkShinnick My point was you can do anything on the vent that you can do with an ambu bag. If you have to bag a patient, the only thing you’re doing is giving larger volumes and much higher pressures, possibly damaging to the lungs, numbers that you would be shocked to see if measured on the vent. During transport you set modes on the vent or if you’re bagging just go easy. That bag is 1.5 - 2 liters.

      @@Idiotsincarshere This is off-topic, but your point rings true for bagging desating pts that are on high oxygen and pressure requirements. You can "bag" a pt on the vent. You take them off the circuit and lose any possible recruitment you may have had. But this happens all the time.

    Great lecture. What do you do with a patient who has a right paralyzed diaphragm with severe asthma with a complication of vocal cord disfunction. I live this nightmare but they started to use respiratory epi along with everything.

      Speech therapy for the VC dysfunction. Escalate Ics, labs, lama, singular for asthma. Need to be followed by a HCP.

      Many viruses can cause post infection asthma exacerbation. I speak from experience.

    By increasing flow rate you will be also increasing peak inspiratory pressure.

    Dimentional analysis doesn't work on your equation you cannot got from L to s units without canceling the L and adding the s. Stick to being Dr and not mathematician.

      Check you math again 😉It works perfectly. Both L and minutes cancel leaving seconds (s).
