Worked *mostly* as expected. It didn't pull the first image on the post every time - sometimes the last, sometimes one from the middle of a number of images on that post. But overall it did what I needed. Every post that I checked except a few now has a Featured Image.
A little more feedback: It appears that you have to reload *every page* of your blog (or blog archive) in order to have it set the featured image for that post. The instructions say to reload your site, but it apparently takes more than that. So if you have 2000 posts you may want to set your blog archive/list to temporarily show more than the default 10 posts in order to reduce the number of clicks needed.

Yes, it may takes more time for the process to complete, It depends on the size of website, Number of pages and the server speed

You can use a code manager plugin like wpcode.

You need to hide featured image in actual post using theme customizer, it only shows to the archive page.
