    I wonder if doing hrt with Mirena is the EASIEST way of taking a progestogen for menopausal therapy. Other forms of progesterone are trail and error for many people I know. I think it would be more simplistic to use a Mirena iud and some form of estrogen (femring from what I understand is the most stable and simple)

      @@heatherhirschMD I have a female friend who is post meno
      She shared with me her hormonal experiences and she only felt good about 2 weeks a month and during pregnancy... And she is worried about possible mood swings
      I actually suggested to her this idea a few days ago
      I do personally in my humble opinion think bioidentical progesterone is safer and better
      However if it comes between no hormones or (some) synthetic I think hrt is better
      And tbh I don't like the notion of stopping hormones at a certain age... Just my 2 cents worth

      I'm confused my gynaecologist wants me to have the mirena coil (iud) fitted as I had my ovaries removed and can't take progesterone as it causes pain, but I'm also taking no seperate eostrgen?? She said I won't need estrogen when the IUD is in, I sthat correct?

      @@boplop5406 no because I have endometriosis and even the smallest amount of eostrgen gives me severe pain to the point I nearly throw up and can't function, I'm completely pain free without.but thankyou for stating that in a very accusatory way. 🙄🙄🙄👍

    I started hrt 2 years ago , feel fantastic , had had constant bleeding , now have the mirina coil inserted and now just use the estrogel . Feel great. My gyno recommended the mirena coil to protect my uterus and every thing is wonderful . My mood , health, has improved dramatically. HRT is a life saver. Thank you for the info on the Mirena coil. The hrt cured everything ,mood swings, hot flashes, sore joints, skin and hair ,energy all came back to normal using hrt.it’s amazing .

    I am 45 and have been bleeding for 12 days now I am over it and have an appointment to talk to the doctor about getting this IUD. The benefits from everything I have seen and read are great.

    Reassuring information! I had the Mirena IUD placed in Feb this year (I get Estrogen through a patch). Before that I was taking it by pill and had lots of issues - but have had zero issues since having the IUD placed. Wish I would have known to get the IUD instead of messing with the pills in the first place.

      I love my mirena iud! oral progesterone comes with side effects that u negate with the iud ! Plus u never have to think about your progesterone component of your hormone therapy plan. A total win win !

    I’m 46 and I have the Skyla IUD, it has been a heaven sent for me. I had a Mirena IUD at first and it was horrible 😞

    Mirena was a nightmare for me, not only could I feel it, I became so angry.
    I’m absolutely lost now & have hypothyroid, fibromyalgia, and Raynaud’s disease.
    My hormones are so off causing misery.
    I don’t have irregular periods nor do I have dryness.
    Depression & muscle burning & mood swings are what I’m dealing with😞

      Sorry to hear you had a bad experience. A big cause of this can be a low lying IUD that abuts into the cervix or uterine wall and is displaced. It could be PMDD or perimenopause, keep journaling and see if you can find any cyclic pattern or find a NAMS doc near you!

      I have a low lying mirena it was seen on an ultra sound. My doctor said it was fine as long as it was “in there”. I feel like I can feel it. I ache after having intercourse. Like period pains and just uncomfortable and my partner can’t go too deep during sex as feels uncomfortable. I don’t know whether to get it just removed or replaced. I’ve tried it for 6 months now and it hasn’t got better. It hasn’t lightened my periods either yet the previous coils i have had I only had very very light periods. Any advice would be much appreciated.

    I had to get the IUD finally after bleeding like a River all the time!!! Got it at 44. Doctor couldn’t figure out what was wrong except it was from stress. He ran ever single test otherwise he had too and thankfully all were negative. The Mirena saved me. Very painful insertion though. I have to get it out and put back in in 2 years and I am scared!!! I now have no period and am so happy.

    I had to have copper non hormonal iud to prevent pregnancy in my peri menopausal stage. I have no thyroid and found that the pill significantly lessened the effects of my hypothyroid meds. Life is completely different for people with auto immune issues. What works for most doesn’t work for me. I am looking so forward to a hysterectomy (uterus and fibroids).

    Hi Dr Hirsch.
    Once again, I had a question about HRT and thought I would check your channel (I'm also subscribed!), and found this video!
    I am nervous but motivated to get an IUD for the progesterone aspect of my HRT regimen.
    I think you had an excellent point about one of the few good findings of the WHI study: estrogen only lowers risk of breast cancer, and if you still have a uterus, the only way to attempt to mimic estrogen-only would be with use of IUD for progesterone staying in the uterus, instead of oral progesterone. I am going to keep that in mind when I go in for my fitting in 2 weeks (nervous but this is the right decision!).

      Hi Candace, I'm curious to know if you went ahead and got the IUD? If so what was your experience, was it painful, what did you do before and after the insertion to help with the pain? I have been post menopausel for about 5 years and is scheduled for to get and IUD on Thursday, but scared of all the horrifying stories I been reading about. Hope to hear from you!

      @@lourdeslayton332 Hi Lourdes! I did get the IUD - I was determined to despite my nerves so I requested 1 Ativan tablet before the procedure. It did help with my anxiety, but didn't obviously do anything for the pain factor.
      I hope I don't dissuade you when I say that yes, it was quite painful. But keep in mind that the intense pain lasts for about 2 minutes and the protection of the IUD lasts for 5 years.
      I didn't have either of my children through vaginal delivery so apparently that can increase risk of pain.
      Incidentally, my 18 year old daughter decided to get an IUD (with my approval!) and said she didn't know what I was talking about and didn't find it very painful at all!
      So please keep all of that in mind when you go in for your appointment, and please let me know how you do!

      PS After the procedure, I went home, sat in front of the TV with a hot water bottle and babied myself - although by then the cramping was very tolerable.

      May I ask how you are liking the IUD now? I’m seriously contemplating switching from the oral micronized progesterone to the IUD. I have some water retention and moodiness with the oral route. I’m also on the estrogen .025 patch. Are you glad you did it? Do you have any side effects? Thank you so much!!!

    i am 54. I tried the compounding cream of estradiol and progesterone and I had many many issues with it… I finally stopped the cream after continued bleeding, acne, no sleep and I was getting very very irritable over everything which is not like me at all. I have been off the cream about 2 months. Pretty much the irritability has gone. But hot flashes have returned slightly, sleep could be better, acne calmed down for the most part and blah mood. I have been doing research because I do want to feel better… I have a non hormonal iud right now but have been considering switching it out with mirena iud with progesterone and an estradiol patch or gel but am concerned with returned irritability and acne…. What are your thoughts??

    Thank you so much for this great video! Question. I have been post menopausal for 6 years and strongly considering HRT with IUD as my progesterone. My question is, I had a prior h/o prolapse fibroid that was removed. Can I still do the IUD, if yes would you recommend having an ultra sound first to assure I don't have any more fibroids?     I have been on Mirena since 2023 March and I had blood work in July 2023, My TSH is abnormal range. 1 year later which is last week I had another blood work done. TSH level got higher. I am wondering if Mirena causes this.. Does anyone have same problem? I have had any Thyroid problems before IUD. I love Mirena so I do not want to remove and switch to oral progesterone.

    I just found your channel today!! My lmp was 2/13/24 but I have had irregular periods exactly as described in this video, sleep problems, mood changes, for a year and a half. Now hot flashes, no sweating for 2 months. I believe I’m in perimenopause and I’m 50, is it too soon to start hormone therapy? My Obgyn recommended Mirena along with low dose estrogen I believe but I was nervous until now after watching this. I’m very new into doing my own research but I do not want to go with out hormones my body has lived with all my life. I have no family hx of cancer, no medical problems, and do not take any medication. What is the safest low dose u can take with Mirena? Thank you for doing this. I want a healthy quality of life.

    thank you for these videos. I have always struggled with PMS/PMDD. when I was younger, I tried low-dose birth control to help the symptoms, but it made me nutsy. I am perimenopausal and am researching my options, and I’m worried that this nutsiness might happen with HRT. Is this something you have any insight into? Maybe you could consider this for a video? 🙂

    What premedication do you recommend since insertion can be more painful since not having periods anymore? And what are side effects do you commonly hear in post menopausal women ( read about mood/ weight gain/anxiety so nervous) Thank you!

    I have the mirena iud and I am so happy about it because I used to have really heavy painful periods and even the contraceptive injection made it worse (I bleed every day for the first two months after every single 3 month shot until I gave up on getting it) so I was really surprised to just have light spotting with the mirena and not even any actual periods and no pain, so I'm looking forward to just sticking with mirena through the menopause when that starts which I guess is in about 10 years or so instead and not need any other menopause treatment, I guess I just stay with IUD for the rest of my life now is that how it works?

    I hope you respond Dr. Dr prescribed pop pill 0.35, bp went up, headaches. I have an endometrioma and trrrible periods. I will be 52 and now sleeping is off too. I have no tubes and one ovary left. Continuous bcp isnt hoing yo eork. Had futch test done om estrogen dominate. Shoukd i get an iud and which one?

    My periods stopped for one and a half year. I am 55 years. But suddenly I had bleeding and quite heavy. I ran thru transvaginal ultrasound and pipel. Everything was fine. To stop the bleeding my doctor suggested mirena. I had it. I just wanted to know that is it good to have mirena in my situation and does it protect against cervical cancer or there might be a risk of it. How frequently I need to check for developing any cancer in the uterus

    If I’m menopausal and my uterine lining is thickened (no cancer) and I am not on hormonal replacement. Will an iud help me shed the thickened lining?

      I had a thickened uterus with some unusual cells. Last year I had my second DC and hysteroscopy and the results came back normal. My uterus started to return to normal. I believe it was because I've been working with a certified Naturapath and nutritionist for the past several months. She addressed all my normal imbalances as well as thyroid support. It's been life changing.

    I have been on progesterone only contraceptive pull since 2015 and not getting a period so it’s difficult to know when I’m starting the menopause. I’m 46 now so I guess I’m in the peri menopause phase but have no symptoms yet. I have fibroids in the uterus and cysts so not sure if IUD would be a better option for me?

    how much of the mirana IUD gets into bloodstream? And does it mainly stay in Uterius.

      The progesterone released mainly stays in the uterus and does not go systemically. many women who did not like the oral progesterone do excellent on the IUD

      Thank you! I am almost 48 and I have a copper IUD. This is my second copper and having problems with this one, I think it’s my age? Heavy periods spotting second half of my hellish cycle. My gynecologist gave me uterine biopsy and pap and ultrasound all negative. She suggested this progesterone iud. I’ll have to get this. Hoping it helps my spotting? It’s ruining my sex life! Lol!
