      The score you gave at the very end: 3, 2, 1... is that 1 being the easiest/best prognosis or 3 being the easiest/best prognosis? I am a little confused because 3 wall defect is easiest to tx with bone graft, but Miller Class I for recession is one of the easiest treat overall right?

      Thank you so much! I'm glad you enjoyed it. Unfortunately I am not familiar with the PSR system so I cannot confidently speak on it, but the good news is it's not covered on the board exam to my knowledge. At a glance it does appear the PSR screening tool applies many of the concepts we talked about in the first video like BOP and PPD but using a different probe and different measuring system. Instead of writing down every finding from six sites of every tooth, you quickly measure bleeding, calculus, and probing depth but only record the worst reading in each sextant of the mouth. The idea is to save some time, but the drawback is you have limited information to refer to back in the future. Hope that helps!

    Hi, Ryan. There is a discrepancy between this video and the perio practice questions. In this presentation you say that PDL cells are the slowest populators of the wound but in the practice questions you say that the bone cells are. Please clarify.

      Hi, Kerry. I think I understand where the confusion is stemming from. The order of population in wound formation is epithelial --> CT --> PDL --> bone. I have them listed in the correct order in this video from fastest to slowest. The numbers (1-4) are referring to how they are labelled in the image, not the order they populate the wound. Hope that helps!

    Thank you for posting these Dr. Ryan! I am a bit confused on some discrepancies. I would love it if you could help clear these up.
    - I have read that periodontal dressing/packs DO enhance healing.
    - Correction of inadequate zone of attached tissue - CT vs Laterally positioned sliding flap
    - Are Ultrasonic scalers more effective in sub or supra calc?
    - Does gingivectomy result in retention of all or most of the attached gingiva?
    - Does calculus have cytotoxic bacterial products even though its always covered by plaque?

    Have two questions regarding perio:
    1) when do we start flap surgery, if when pocket depth is greater than 5 mm Or pocket depth is 5 mm, does bleeding on probing have any role here?
    2) what will happen if we prescribe topical or systemic steroid to NUG or acute viral infections, what is the reason of being contraindicated?

      Hi, I'm not sure about (1), but here's what I think about (2):
      Steroid is anti-inflammatory. We need our immune system to be working to prevent the uncontrolled bacterial/viral spread. Thus by prescribing steroid, we suppress our immune system from protecting us against infection.
      Steroid in oral cases is prescribed when the immune is hyper-responding, or in autoimmune cases

      niether pocket depth nor BOP give as much info as CAL does. this is why CAL is most important information do decide treatment plan.
      1)pocket depth: 5mm could be true clinical attachment loss, in which resective osseous surgery is indicated, or could be 5mm with a psuedopocket from hyperplasic gingiva (ei. if pt on phenytoin/nephedipine) in which case perio surgery is NOT indicated, rather a gingival surgery (gingivectomy)
      2)BOP: good indication of inflammation but not always the case. if a pt is a smoker, the gums may not bleed due to vasoC that nicotine causes in gingiva. so even if CAL is 6/7mm, they may not BOP if they smoke.

      A “four wall defect” is another name for an extraction site where all walls are maintained. Yes, it is the ideal site for regeneration. And that is correct, they are interchangeable.

    If Gtr is an example of new attachment what SRP is an example for regeneration?

      GTR is more consistent with regeneration and new attachment, whereas SRP alone can result in repair and reattachment.
