Even after he was poisoned with such a high dosage of wisteria poison(70 times more the amount required to kill a normal demon), he was just weakened and was not able to regenerate. Even in this state, Kanao and Inosuke struggled to behead him. The sad thing is Shinobu's brave act only bought them a few minutes or so. If Kanao and Inosuke had not capitalised on that, Douma would have regenerated and killed them. 5. Fun fact, The top three upper moons cannot be killed even if their head is severed and the only way to kill them is sunlight or plot armour 😛.

gyutaro intentionally attacked inosuke's heart to kill him, he detected his attack and he shifted his heart to another place and he survived
