    I had one for 9 months! Very painful...4th doctor, a surgeon did the sphnicterometey, which solved the issue in 1 week. 18 years later doing great!

    The pain is absolutely terrible but it gets extremely discouraging because it seems like it will never get better but then one day it just does. It took 8 weeks, both times.

    I have been suffering with anal fissue from past 1 year .. and slightest change in diet or holding up the urge to pass stool ..it starts the cycle of pain and bleeding and goes on for 2 weeks then gets healed. But keeps reoccurring. Try sitz bath 2 times a day, it really helps.
    God heal everyone going through this intolerable pain😢

      Use Huggies wipes, sometimes it's not the fiber but the issue is that toilet paper is made from many chemicals that can irritate the anus (fissure), and micro cut you. Wipes allow moisture so the skin doesn't get irritated, and apply ointment (Vaseline) after every use and shower. DONT use toilet paper it makes it worse and prevents it from healing.

    glycerine suppositories before you go are also great until it heals. Non Addictive and coats the fissure going in to lessen the likelihood of a tear. Fibre # 1

    I had two goes of EUA and Botox & the fissure reestablished itself after a few months both times. I was then prescribed Rectogesic cream (Glycerine tri-nitrate aka GTN). GTN is a vasodilator and increases blood flow. The Fissure was gone in a few weeks. The only thing is you get systemic side effects with GTN (mostly a headache) as it's rapidly absorbed in rectal tissue. The side effects only last a few minutes with no long-term issues.

    I suffered for a few years, then with to see a Surgeon. The surgeon was great, gave me the cream and told me to use fiber, water, clean with tucks pads, took another few months, but I'm healed. THANK GOD!!@

    I have had all of this, then got sent for more tests where they found that my sphincter muscles didnt work properly and surgery would likely not be successful and make me incontinent surgeon said it was too risky. Sometimes i would take that over the pain i still have.

    I get them once in a while, and they are the direct result of constipation or straining caused by certain foods. The key is to monitor what you eat so you learn to avoid those foods which cause constipation so that you don't have to strain so much. I try to avoid eating too much meat, coffee, and cheese too. Recovery time is about 3-4 weeks, and the way tell it is healing, is by noticing the bleeding begins to subside in that it becomes spotty until it eventually stops completely.

    My ordeal was a year long one. First because it wasn’t diagnosed second I was having heart issues simultaneously
    I had a Quadruple Bypass and everyone asks about the pain. The bypass was nothing pain wise but the anal fisher was months of hell. Finally got hooked up with a good Gastro surgeon who tried the Botox which helped but I had to have the sphincterotomy and now it’s great.
    Thanks to all the good doctors out there.

    My friend has had constipation too often and she changed her diet then regular movements are happening. A possible blocking caused her pain even though she eats enough fiber. Her doctor didn't have a reason but I knew she only drinks 1l of water a day. I say, now drink 3l+ of water as fiber and water seem equally important. She does and she's defacating a-ok now.

    I’ve had both hemorrhoids and fissures (and heart and cancer et al), but I thought the hemorrhoids were more painful. When the fissures bled, it actually relieved the pressure.     I use to have anal fissure(several years). I got rid of mine by doing intermittent fasting . It can't heal if you are always pooping . All fiber is going to do it keep you pooping. With fasting I would only poop every few days because I wasn't filling my intestines with food.

    I've been eating a mostly plant based, high fiber diet for years. My stools are very regular and soft, yet I've still had several of these. I thonk my sphincter doesn't relax properly during defecation. So even though I'm doing all the right things, I still experience misery about once a year.

      These guys left out anal spasm and pelvic floor therapy. Mine seems to be a combo of pelvic floor and fatty liver issues. So many possibilities for causes and surgeons are going to want to surgeon.

    I was using the fiber supplements for years (remember Per Diem?). Pain came and went, but when it came it was excruciating. After unrelated melanoma scares and surgeries, I was referred to a rectal surgeon. His surgery on me was the ticket. Aftercare was a year of monthly visits where he painted the affected area with silver. Twenty years later and I'm still healed and pain-free.

    I had an extremely painful, anal fissure the ointment, medication rectiv which is nitroglycerin that’s specifically for anal fissures worked amazingly. Only thing that gets hard to get it hard to find I had to drive far away to a pharmacy that had it

      It took 5 days to find the nitroglycerin and insurance doesn't cover it at least mine didn't they wanted $700!!! I was able to get it for $160

    I am 73. I am pretty well convinced that my anal fissures will not, and perhaps never will heal. I tried fiber and water, less fiber and more water, and various other combinations, all while using a topical, prescription ointment. Things improved and then after many months of using the ointment and then quitting its use, either the fissures immediately reopened or new fissures were created. In the past year I have used a Miralax store-brand once a day and since it allows more water to remain in the colon, it seems to solve my problem. Stools are soft and I am pain and bleeding-free. It is a relatively benign daily treatment in that the medication goes no farther than the colon. It does not enter the bloodstream. There are no other side-effects such as spontaneous bowel movements. When I miss a day, the pain returns the following day. I don't like the idea of taking a stool-softener daily, but I prefer it to pain and bleeding.

    I had a sphincterotomy many years ago that developed into a fissure, then had to have that repaired and then another sphincterotomy because the first doctor did it in the wrong place. It sucked and hurt like he'll, I was in nursing school and could not sit down in class, ugh.

    I got an anal fissure during childbirth. It seems that the area is weakened, as 30 years later it still raises its ugly head if I've eaten poorly and gotten constipated. Definitely very painful. Thanks for the video, docs. Botox - good to know. 🙂

    What is the difference between anal fissures and hemorrhoids? I have had problems with hemorrhoids back during the winter of 2020-2021. They came back a little bit over the past two months but not as bad as four years ago.

    It looks like I was waiting for this podcast . I do suffer with this problem for almost 3 months doing all the right things ( lots of fiber , fluids , ointment prescribed by the Dr. ) Nothings works ! I have scheduled appointment we will see what is next , maybe botox ? - and I said I will never use that staff 😉😂

    All the other doctors contradict what this doctor said. Even my own surgeon said do not add fiber and quit the vegetables. This doctor even says that it adds bulk. Several doctors including mine recommended the carnivore diet. Olive oil, avocado oil and fatty fish are beneficial. So far this is the only video I've watched that said to add vegetables and more fiber. Everybody agrees on more liquid. I'm not a doctor so I'm not going to argue with him but I will follow my doctor's instructions.

    Mine have gotten worse. They’ve gotten longer and now even if my stool is soft, it still is absolute agony. I had to leave work bc I had a bowel movement that morning and the pain was so bad I couldn’t sit. I had another bowel moment and it was a nightmare. I cried and my hands were shaking. 😢😢😢 I sent a message to my pcp for help.

    Thank you so much for this video! I didn't realize an anal fissure could be naturally occurring. I have had one since I add an episiotomy during the birth of my son almost 35 years ago. It has healed and flared up more times than I can count. It's brutal! I eat a very healthy diet and drink lots of fluids, fortunately. It's very sad that I was given no information or education on the subject by my doctor, but, I have been muddling along and managing it. I'll be asking my family doctor for a topical rx asap. Again, I thank you so much for this info!

    Too much insoluble fiber can make the problem worse. Fiber bulks up the stool. You don't want bulk.

    Yeah I’ve been dealing with this for a while now. As soon as I got on a fiber and water diet it seems to have gotten better. Sometimes it’s super painful to use the bathroom and other times it’s easy. Probably gonna go to the doctor soon just to be safe and make sure it heals fully cuz it’s on and off for me.

    Unfortunately for those of us with chronic diarrhea, who cannot tolerate fiber supplements, who already have urgency, who can't find a way to stop diarrhea that doesn't bring on constipation (that can be severe), and have had multiple anal fissures... and the pain is utterly intolerable.... 9/10 with bowel movements and for 30 minutes after... and those BM's are frequent.... This was my issue when I went to a colorectal surgeon. I had injections, creams, tried the cholestyramine (as I'd had a cholecystectomy years prior) and almost ended up in the ER, had to give myself an enema from tiniest dose of it...ended up having rectal surgery. Recovery was hell too. Was in so much pain for months. And can't take most painkillers.
    I was the patient from hell, because nothing worked for me! Any thoughts for me if this happens again? I still struggle with diarrhea most days. Still trying to find an answer for that.

    Botox can also cause temporary incontinence. Had it spring of last year, along with skin tag removal from previous fissures. It healed pretty quickly afterward. The Botox also caused a thrombosed hem. (Came home from procedure with it, so I had to heal from TWO things then. Ugh.) Feel like I have another one and do all I can to keep stools soft. And fiber doesn’t work for everyone, especially the supplements that GI docs harp on. Trying to balance fiber, magnesium, stool softeners to go ok and just once a day. Working with a dietician now and we STILL haven’t figured it out! :(

    How can we tell if they are hemorrhoids or anal fissures before being examined? Is there a difference in the feeling and pain?

      A fissure is a small tear on the lining inside of the anus. A hemorrhoid is usually less painful and is an issue with the blood vessels outside or just inside the anus.
      A doctor will tell you very quickly.

    My doctor said i had a fissure, ...that was over 20 years ago, i still have 'something' that pops out my hole when i pass a stool every single morning, i have to sit down on a hard surface and place pressure on my butt and it pops back in. ( i had a mishap many years ago where i bent down quickly to look at something under my vehicle and didnt notice a metal rod, it went straight up my butt hole and there was a sharp pain and blood, ever since then since the dostor said i had a fissure i just used some cream and it went away but then kept coming back, 20 years later stil in same situation), i really do not want surgery :(

    I don't know if I agree with you on fiber especially if people have IBS and chronic constipation it's not something they can just start immediately takes a long time to start feeling comfortable with this stuff.

    I had the surgery sphincterotomy, it was no fun, took 3 months to recover/heal from the surgery, the bleeding would not stop for 3 months, and all of a sudden it stopped and I did not know why. I don't feel any significant improvement after the surgery though. I really hope somebody could develop some artificial butt to replace the whole organ down there, they built the artificial heart many years ago, why nobody do something for the butt, how hard could it be?

    My physician prescribed me suppository for fissures. It started before three months , was better in the middle and relapsed again.

    In radiology, we use Botox injections for salivary gland dysfunction in stroke/ALS patients who have excessive drooling due to inability to swallow

      My husband died from ALS and I have never heard of that, it doesn't make sense to paralyze, what's already paralyzed. That would compromise swallowing more. Even with a gastro feeding tube my husband liked to try to drink things. Botox can cause ALS so this seems stupid!!!😮

      @@Mithras444 there’s not a single case described in literature of Botox causing ALS. Botox paralyzes muscle and salivary glands are not muscles. I’ve been involved in hundreds of cases of the Botox injections for drooling. It may not be life changing for the patients but is for the caregivers. You should research things before spreading misinformation. I’m very sorry you lost your husband, it is a horrible disease.

    It's horrible. You can't concetrate or sit down. You have to lay down all the time. The Sphincterotomy fixed it for me. This was due to Covid and a bad diet.

    Use Huggies wipes, sometimes it's not the fiber but the issue is that toilet paper is made from many chemicals that can irritate the anus (fissure), and micro cut you. Wipes allow moisture so the skin doesn't get irritated, and apply ointment (Vaseline) after every use and shower. DONT use toilet paper it makes it worse and prevents it from healing.

    I had anal fissures for 2 years, it lead to a fistula. It was very painful and distressing. And entailed 2 operations.
    I went to another hospital and they did a test and diagnosed rectal Gonorrhoea. I was given 2 tablets, 30 minutes after taking the tablets, I felt the symptoms start to subside and rapidly disappear.
    If someone has anal fissures that won't heal, it's a diagnosis that doctors should consider.

      Umm... Rectal gonorrhea is a sexually transmitted disease. Sex safe with condoms and regular sexual health clinic checks are most important. If you have anal sex, you need to have an anal swab regularly. Also your throat needs to swabbed if you do oral sex. Penile, vaginal, anal and oral gonorrhea are things that exist!

      @@marleyhill34 Thank you for the advice. Interestingly, I remember the doctor saying that gonorrhea can be transmitted through the use of fingers and I guess sex toys.
      Gonorrhoea is becoming more difficult to treat and some strains are antibiotic resistant. Untreated STD's can make you more vulnerable to HIV.
      People need to be educated and to stay safe.
