Yes. It traps bacteria, water, dirt, and loose hair in there with wax that should be able to migrate out on it's own. Sometimes something as simple as the water you wash your dog with or the water it jumped in while playing outside can become a huge problem if the water can't make it out. Then you have things like grass seeds, which can cling to the dog's hair and work it's way into it's ear canal by the ear hair. Those things are hell on anything, but the inside of a dog's ear very bad.
+My Favorite Groomer Dede , Way to go on the patience and reading the dog's pain like that and whispering so kindly to the dog because you limited the dog's stress and ouch signals in the process of cleaning their ears like that . Fortunately the dog has you to do what the owner will not do and that is help to cope and handle situations like this . It upsets me when I get the sense that there are owners who don't take the time to play with their pet and train their pet and it is always up to you the groomer or the vet to do some of that for them . This truly makes you one of a kind . - Dwight
Sedation can be dangerous, so they usually only do it when completely necessary. Besides, groomers usually do not have the ability to sedate their clients.

uhh some people like their dog with long hair. and at the beginning she says "yeah I know you don't like it that's why we do it every third time" they clearly love that dog. Also they clearly brush regularly or the hair would be matted and she'd have to clip/shave, not scissor.

Kind of a given, isn't it? Some dogs' ears can get so dense with hair it causes hearing loss.

If I were the owner I would get some sedative from my vet that we could really get in there and get those ears for every once in awhile

You could use some duck bill clamps on Max to hold the long stuff away from where you're working. You can find them at any beauty supply store.

Those are blunt-tipped hemostats, not scissors, and she has a special cleaning solution on the cotton.
Hair clips help to keep the rest out of the way for this maybe?
