Henry didn't get one because he wasn't exploring "his" silent hill like everyone else was. I'm not against PH only being exclusive to James being in SH. I'd think otherworld would "save" PH manifestation even if James died or got the good ending and left SH.
It can be both. It's implied James, in the past, saw the painting of Pyramid Head in the Silent Hill Historical Society and his manifestation is based off of that.
Hello! I just finished SH2 and I got questions. So, PH is not a manifestation of James's sexual frustations but a punisher for himself. That's why it kills some monsters of the town? It is not doing sexual things in the first encounter. So what is Maria and why she is killed three times for PH?
1. Maria is what James want Mary to be. You can empathy with this situation since living with an illness person is very frustrating, not just physically but also mentality. So James want Mary to be more sexual, a kind of woman that talks sweet things to man, or at least, James is sexually frustrated because Mary is ill. 2. The Pyramid Head is the darkside of James. That's why it did some *sexual* action when James first see it in the kitchen. His head is the Pyramid, implies that James is trapped with Mary in the hospital room, can't move on on his life, but at the same time he can't leave Mary alone behind just so he can live his own life, James's life is being trapped as well. 3. Beside from the Pyramid Head, all the monsters are Female. Why? Well, it's implies that of Mary's illness, like in the game, when you kill too much monsters you get the bad ending, whereas you could get the good end. 4. The Pyramid Head can't be defeated unless James admit his guilty. Later on when he admit, the Pyramid Head kill itself.
The creator of Pyramid Head said the helmet is not his actual head and would like to leave whatever is under the helmet ambiguous. Comparing it to a magician revealing his trick.

People whining in the comments as if James cannot be and will be the only human being to have murdered his terminally ill wife so he could go meet women again to be called to Silent Hill.
