I’m a PRP proponent first. But I have received epidurals as well. They are definitely just bandaids, to help you get to the next steps in recovery/healing i.e. PT etc.
It is so frustrating that my surgeons first line is still cortisone injection. I won’t let it happen. His hands are tied by insurance coverage. When will this change?
I hear you. Much of this research is very recent. And it takes a long time to change medical practices and insurance coverages. All we can do is support more research and spread the word! Thanks for your comment
Please check in TH-cam this: The knees over toes guy, this person had some serious knees problems and he fixed all of them based on exercises, I'm doing them almost every day and I fixed some serious pain I had, just research and give it a try.
Thank you for a really interesting and informative video. I have two questions. 1. How do you know the quality of the PRP injection you will be getting? Should the doctors office provide that data before the injection? 2. Do you know of any studies comparing PRP to Mensenchymal stem cells derived from umbilical cord tissue?
Great video. You have a gift for explaining things so well. I just had my first PRP injection to the right first MTP joint. Tried Prolozone first and that didn't work for me. PRP was the next step. Thanks for all the great information you are providing.
We're still not entirely sure what the "best" is. More research is being done on this topic. What is clear -- too little and it's not good enough. Too much could also be detrimental. The big question is where is the sweet spot... to be determined!
thank you Dr. Peng , thoroughly enjoyed your video re: Trigger Points & Therapy also ; this video just changed my mind about cortisone inj vs PRP for neck pain ….
I have craniocervical instability and occipital neuralgia and pain management is willing to try PRP injections for my lax ligaments as well as wanting to target c3 facet joint. I had whiplash injury from car accident in 2019 and have Ehlers-Danlos syndrome. He told me there are no idea of outcome but since I had success with PRP in my shoulder for rotator cuff tear he wants to be hopeful.
Unfortunately PRP has not been extensively studied for peripheral neuropathy. There are case reports that it works for conditions such as carpal tunnel syndrome, but no larger trials (to my knowledge).
Just found your TH-cam channel. Excellent information and I've had great success with PRP and Prolo therapy for several injuries with doctor Brian Shiple in Glen Mills PA.
I am currently trying to decide where to go to get a PRP injection. We have all these QC Kinetics clinics popping up all over my area, but I don't know how to tell if they are a legit place for this procedure. I have always used TRIA sports clinics for my injury's and they offer PRP injections at 900 per shot. I am also located near the Mayo clinic as they also offer the PRP. It is hard to determine where I should go and after watching this great video, I really want to make sure to get the most platelets in the injection for my knee. Meniscus tear and start of some arthritis. Runner, racer, and cyclist btw.
I just started working with them for shoulder. I am not sure why all the hate on QC Kenetics I wish someone would review them that is legit. They work with ultrasound to find the right places to put the plasma and I have a 5 session program including stem cells ahead. I am very hopeful
Over the years, I've had a number of small injuries to the left shoulder & bicep tendons. Doctor insisted that Cortisone was the only way to fix the issues at hand. I had 2 rounds of Cortisone injection in the shoulder & tendons at upper bicep. There was no pain relief & approximately 9 months later, the long head tendon tore from anchor point. It was 1.5 years before I got in for surgery to pull bicep back into place, which could not be pulled fully back. I will never accept Cortisone injections ever again.
what about the Trapezium? If a patient is told that the diagnosis is arthritis of the thumb/trapezium joint. Musicians around the world need better options
Got a PRP injection w/ hyalruronic acid 3 weeks ago for cartilage lesion/ "chondral defect of lateral trochlea." Was this the right decision? Previously got a PRP injection for partial patellar tendon tear and next MRI showed the tendon healed.
MRI CONCLUSION, 1 month ago before PRP; -Full-thickness full-thickness chondral defect of the lateral trochlea with subchondral reactive marrow change of the underlying bone. -Small knee effusion. -Tiny Baker cyst.
I have two artificial knees, but still have chronic pain. In addition I am loosing bone in my right knee and have been told that i will need a revision to my right prosthesis. I have found no Physician-that will inject my knee with PRP because of possible infection. What can I do??

I cover that at the end of this video: th-cam.com/video/wexBm-ag-g8/w-d-xo.html You can technically get PRP as often as you need it. It's your own body!
Thanks for this video. Do you think there is a difference in the efficacy of stem cell vs prp based on the age of the recipient (eg 20s, 30s, 40s, 50s, 60s)?
I had a mojor stroke 8years ago and I lost my left side I have been following stem cell treatment I believe it’s my only hope to get my life back I am 62 years old
It's important to point out that this discussion is for the use of mesenchymal stem cells for the treatment of orthopedic conditions and not neurological sequelae. Good luck
Thanks for your question. Much less data on that because they aren't as common as arthritis in the knee or the hip. With that said, most doctors make the logical leap that if orthobiologics work for arthritis in one joint, it should work for arthritis in another joint.
You would think the bone spurs have to be removed. But that's not what the studies suggest. PRP / MSCs are effective without surgery. In fact, probably better for the treatment of osteoarthritis. See here for more info: th-cam.com/video/FAsnQtIKN6g/w-d-xo.html
@@JeffreyPengMD Thanks. So, am I correct to assume the spurs generally aren't the cause of the inflammation/pain unless they are large enough to impinge on a nerve?
Osteophytes are a consequence of underlying osteoarthritis and inflammation. Their presence is part of the grading system for OA. radiopaedia.org/articles/kellgren-and-lawrence-system-for-classification-of-osteoarthritis?lang=us
Thank you very much for this information! I have a degenerative complex meniscus tear (happened 5 weeks ago) and I am considering the PRP injections. I know you mentioned that PRP does not heal the meniscus tears, but what outcome should I expect after the PRP treatments? Is it even worth it for my situation?
Thank a good presentation with video. Could you make a comparison of the Expensive PRP with the cheap PRP? What is the main difference? I also plan to have knee PRP
Thanks for the great and thorough review. If one uses their own blood for the PRP injection, any way of knowing platelet concentration? Where does the “cheap” PRP come from?
This usually has to do with 1) how much blood is drawn and 2) how good the kit is at retaining platelets and growth factors. Most kits do a good job at retaining platelets and growth factors, so it really seems like the amount of blood taken is one of the key things to look for. It seems like around 20-30cc is a minimum for small areas and around 50-60cc for larger areas. Definitely an area of active research!
@76sherie Hello lady, get well soon, I think you had arthritis in your knee. How long has it been since you had stem cell treatment? Have you fully recovered? For example, can you walk comfortably, walk quickly, go up and down stairs comfortably? We would be very happy if you could inform us, we are also in search of something.I'm sorry, but how old are you?
I had people tell prolotherapy is better then prp injections and I also had people tell me prp is better so which is it by the way I have 3 heirnated discs both sides of the back so I'm the future I gotta do a litt more research and I'm going with one of those it's either prp prolotherapy stem cells or ozone therapy I have heirnated discs both sides of the lower back and I was already told surgery ain't a option unless pain comes back to my leg so these other choices sound safer for me in the future what's your opinion on this ?
Both are potentially good options, but PRP seems to be MUCH better. Not a lot of head to head trials. This is one example, however, they used low dose inferior quality PRP and found that PRP is still favored. High dose PRP would result in better outcomes. pubmed.ncbi.nlm.nih.gov/36166187/
Unfortunately head to head studies have not been done to compare the two treatments. Both seem like great options but please discuss with your health care provider, good luck
PRP and biologics have not been studied for retrocalcaneal or retro achilles bursitis. In theory it will work. But I cannot back that statement up with clinical trial data.
This is still a topic that is being actively investigated. It seems leukocyte rich MIGHT be better for soft tissue and leukocyte poor MIGHT be better for joints. But this is subject to change with more clinical trials.
Thanks for your support! Unfortunately there are not a lot of studies looking at PRP for fibromyalgia or myofascial pain. Though feel free to check out my video on how to take a multimodal approach to treating trigger points! th-cam.com/video/VlpcqFqrQgQ/w-d-xo.html
Unfortunately umbilical MSC treatments are still VERY new and there is no robust head to head comparison data between them and PRP. Hopefully they will get done one day, but until then, PRP is still my gold standard for orthobiologic therapy.
Thank you for sharing this! In regards to hip labral tears I see this 2019 study (ref below) concluding with: "Ultrasound-guided injection of platelet-rich plasma holds promise as an emerging, minimally invasive technique toward symptom relief, reducing pain, and improving function in patients with hip labral tears." Question: Would you say there's a possibility that PRP might work for labral tears / instability in the shoulder as well? I'm asking because so far I've only found anecdotal stories supporting this. --- Am J Phys Med Rehabil. 2019 Nov; Use of Platelet-Rich Plasma for the Treatment of Acetabular Labral Tear of the Hip: A Pilot Study Arthur Jason De Luigi 1, Daniel Blatz, Christopher Karam, Zachary Gustin, Andrew H Gordon
Thanks for your comment. As with everything, it depends. Are labral tears isolated or in the presence of hip osteoarthritis? If there is underlying OA, chances are yes, the PRP will work. I don't believe I have seen any studies looking at PRP in the use of shoulder instability (I could be wrong, just nothing in recent memory).
Great Video Jeff, like it when these types of videos are backed up with evidence-based peer reviewed research papers. I have mod/severe knee OA in both knees. How often would you recommend PRP injections? I know you said it takes time for effects to happen. I guess the gaol of PRP is to slow down the OA progression and delay the inevitable TKR. Are there any side effects to PRP, I guess there is the same infection risk as a cortisone injection, which is about 1 in 1000. I think?
The beauty of PRP is that it's you!! No risk of allergic reaction. No toxicity. The only risk is infection, but if done with sterile technique, the risk is really minimal and close to zero.
Yes, thank you for the great videos on PRP and Stem Cells Therapy. I especially liked your info @ 21:10, "...Bone Marrow Stem Cells was not superior to PRP." This is great news for me because I already took PRP for a mild case of Osteoarthritis for both knees and I was considering stem cell therapy. One quote here gave me $6000 for Stem Cell Therapy (not sure if that was adipose fat tissue or bone marrow) and one quoted me $1000 for 2-3 joints using PRP. I think I'll go with the PRP. I did notice that the first time I took PRP the effects only lasted about six months, but that could just be the concentration of platelets for the first attempt. This time the machine that they're using concentrates platelets at 5-10x baseline. Hope this one lasts longer. Thanks again for the videos. Keep them coming, and Happy New Year!
Hello doctor (I am from Asia) I want to ask : Is it possible to heal piriformis syndrome by injecting PRP? (my right piriformis is manifested inflame under ultrasound scan) I took tons of treatments, e.g. steroid, shockwave, ultrasound but it seems like these therapies did not work for me. I just got my first dextrose prolotherapy injection about 10 days ago, I still didn’t have a noticeable sciatic pain relief. This pain really hurts my routine life for half year🥲 I just hope PRP will work(only a bit search I could find, so I couldn’t tell weather PRP works or not for bad piriformis syndrome)
Do you have a listing of doctors who practice PRP? I live in Lancaster, PA. And was wondering if any physicians have trained on this procedure near me? I so want this PRP. I have a lot of pain in my right knee doctor said I should have surgery. But I have A-Fib in which I take a blood thinner. He’s not sure I can have replacement surgery with the A-Fib and blood thinner that none of my doctors will agree to have it done. Other option would be PRP. I do have tremendous pain and not very active. Is there any way you can help me? Thank you

Many tennis elbow patients report immediate pain relief after treatment with cortisone injection. But microscopic analysis of tennis elbow tissues suggests there is no inflammatory process with tennis elbow. If that is true, what is the possible mechanism that steroid injections would quickly improve tennis elbow pain? It is rather confusing...
Steroid injections have pain relieving effects in addition to its anti-inflammatory effects. It's just you are trading short term gain for long term damage. Each person needs to assess risk and benefit on their own (assuming their doctor does a proper informed consent). Thanks for your comment.
Hey Dr. Peng, thank you for the wonderful lecture. What do you think about that recent JAMA article where they said there was no difference between PRP and saline? Effect of Intra-articular Platelet-Rich Plasma vs Placebo Injection on Pain and Medial Tibial Cartilage Volume in Patients With Knee Osteoarthritis I think it’s BS because they didn’t even use PRP. And I don’t even think they know what the actual cell count was, or even have a way to verify it. Anyway, just annoys me because these therapies are the future.

Excellent video , the dose of medication is always important consideration when making an analysus in medical studies in order to get a good result this was not considered before making a bad analysis recomendation that a medication does not work, because of this poor study insurance companies can now use the excuse for not paying for medical care that is needed causing millions of people to suffer from artritic conditions just like in the past they showed studies that it was the fat and meat products that were killing people not the sugars and starches breads and recommended people to eat mainly carbohydrates instead of permitting meats and fats ,now millions of people are dying from obesity and heart disease and cancer because of the sugars breads and carbohydrates led to multiple diseases this concept was accepted by the medical establishment as true and recommended could there have been also some type of incentive from insurance companies for this study to have shown that prp did not work? Now people are dying because they cant move they cant exercise they cant enjoy the rest of their lives with their families becausevthey cant .move
Will have to say this does not work if you are completely bone on bone. You need cartilage to grow. 8K wasted for me. I know it helps with some though.
"The results of this study demonstrate that PRP therapy delivers ng/mL-range concentrations of catabolic proteases, which could perpetuate inflammation and inhibit tissue healing." ref: pubmed.ncbi.nlm.nih.gov/24627579/ This statement seems contradictory to everything else that I've heard about PRP Can you pls clarify?
This is a great example of why in vitro studies are not the same as in vivo studies. MMPs are associated with worsening cartilage destruction and osteoarthritis. So you would think injecting any solution with MMPs would be counterproductive. But in actuality, synovial fluid analysis after PRP injections show DECREASED inflammatory cytokines (TNFa, IL1b). PRP injections have been shown to decrease the progression of osteoarthritis. See here for more info: th-cam.com/video/_7HU_-YDfQY/w-d-xo.html
First of all, I want to thank you for all this information. I have watched tons of videos, and I just hate when people say “studies have found”, I’m a chemist, so proofs and information is really important for me, and I love the fact you do show the publications, and I have looked for some. That’s very profesional Now, I have a couple of question. I have had 2 knee surgeries, and a few weeks ago, my orthopedist asked for an MRI to confirm if my meniscus is torn, I haven’t gotten the scan, because i don’t want a 3rd surgery (ignorance sometimes is better 🥲😅) I found very interesting what you explained in another video, that maybe the cause of a tear is the OA. I know there is no way to regenerate cartilage, my surgeon suggested I should start taking glucosamine with Chondroitin sulfate, Is there evidence about the effectiveness of this? Also, do you know if this condition (OA) can be potentiated somehow with flat feet, or bad posture? And about plasma, what my surgeons explained is that, the growth factors are the ones that will be involved in the “healing” process, now the concentration or presence of those growth factors, could vary? Could they be affected by the eating style of a person? And last question. As I understand, the weight plays a big role in all this, I’m 5’11” and 160 pounds, Would you say is better to lose more weight, the less the better?
Thanks for your support! You have excellent questions that likely should be addressed with your trusted health care provider. I'll do my best, but will be brief. 1. Mixed studies for glucosamine, likely little to no harm in trying. 2. Yes, biomechanical problems can lead to worsening arthritis. 3. Yes, type and preparation of PRP matters... a LOT. 4. Diet matters significantly. Aim for more fruits and vegetables (ie. anti-inflammatory diet) 5. Weight is critical. Adipose tissue release inflammatory adipokines which are problematic in arthritis.

What about stem cells from the (1) placenta + (2) chord blood? What is your opinion on them (and what's the "technical name" for them).
I’m a PRP proponent first. But I have received epidurals as well. They are definitely just bandaids, to help you get to the next steps in recovery/healing i.e. PT etc.
Yup, 100%. They are a bridging treatment
How much is the cost for these PRP & steem cell in back L4-S1 anterolesthesis and arthritis?
It is so frustrating that my surgeons first line is still cortisone injection. I won’t let it happen. His hands are tied by insurance coverage. When will this change?
I hear you. Much of this research is very recent. And it takes a long time to change medical practices and insurance coverages. All we can do is support more research and spread the word! Thanks for your comment
I had a better reaction from cortisone than I did from PRP , sometimes simplest is best
Please check in TH-cam this: The knees over toes guy, this person had some serious knees problems and he fixed all of them based on exercises, I'm doing them almost every day and I fixed some serious pain I had, just research and give it a try.

Is there any specific research on bicep tendinitis with PRP or stem cell?
Can you please explain how PRP can help TMJ? Does PRP help for TMJ osteoarthritis?
Thank you for a really interesting and informative video. I have two questions.
1. How do you know the quality of the PRP injection you will be getting? Should the doctors office provide that data before the injection?
2. Do you know of any studies comparing PRP to Mensenchymal stem cells derived from umbilical cord tissue?

Great video. You have a gift for explaining things so well. I just had my first PRP injection to the right first MTP joint. Tried Prolozone first and that didn't work for me. PRP was the next step. Thanks for all the great information you are providing.

so the cheaper PRP doesn't do quite as well, what is a good concentration for effective PRP treatment then?
We're still not entirely sure what the "best" is. More research is being done on this topic. What is clear -- too little and it's not good enough. Too much could also be detrimental. The big question is where is the sweet spot... to be determined!
thank you Dr. Peng , thoroughly enjoyed your video re: Trigger Points & Therapy also ; this video just changed my mind about cortisone inj vs PRP for neck pain ….

Can PRP be used for slip disc healing?
PRP has not been studied extensively for the spine. However, there is some limited evidence.

PRP and idiopathic peripheral neuropathy? Does it work does it restore feeling?
Unfortunately PRP has not been extensively studied for peripheral neuropathy. There are case reports that it works for conditions such as carpal tunnel syndrome, but no larger trials (to my knowledge).
If I am on meds , what do you think about prp injections?
Best to talk with your docs
Just found your TH-cam channel. Excellent information and I've had great success with PRP and Prolo therapy for several injuries with doctor Brian Shiple in Glen Mills PA.
Thanks!
I don't know anything about QC Kinetics, but I've heard others warn to stay far away from those. On the other hand, Mayo clinic is great!!
I just started working with them for shoulder. I am not sure why all the hate on QC Kenetics I wish someone would review them that is legit. They work with ultrasound to find the right places to put the plasma and I have a 5 session program including stem cells ahead. I am very hopeful
Can we use prp injection in 3 rd metatarsal arthritis.
Yes, we really need to move away from cortisone shots

Does stem cell therapy work for rheumatoid arthritis, please guide
Orthobiologic therapy has not been studied for the use of rheumatoid arthritis.
Good luck
Is there any evidence that the stem cells and/or the PRP injections help in other areas of the body that have OA, like the hands or the feet?
Thanks for your question. Much less data on that because they aren't as common as arthritis in the knee or the hip. With that said, most doctors make the logical leap that if orthobiologics work for arthritis in one joint, it should work for arthritis in another joint.
Is PRP or MSCs effective if there are bone spurs in the joint? Don't the spurs need to be removed first? This is for CMC joint OA.
Here you go! th-cam.com/video/wexBm-ag-g8/w-d-xo.html
High dose PRP would result in better outcomes. pubmed.ncbi.nlm.nih.gov/36166187/

shock wave therapy.....which one has more benefits?
Unfortunately head to head studies have not been done to compare the two treatments. Both seem like great options but please discuss with your health care provider, good luck

So, are you saying that PRP is not relevant for bursa issues, specifically Achilles bursitis ? Can you briefly explain?
PRP and biologics have not been studied for retrocalcaneal or retro achilles bursitis. In theory it will work. But I cannot back that statement up with clinical trial data.
Should the PRP be Leukocytes rich or leukocytes poor. Please answer.
This is still a topic that is being actively investigated. It seems leukocyte rich MIGHT be better for soft tissue and leukocyte poor MIGHT be better for joints. But this is subject to change with more clinical trials.
Your videos are useful. 👍👍
Could you explain if PRP can help patients with fibromyalgia or myofascial pain ? 🙏🙏
Thanks for your support! Unfortunately there are not a lot of studies looking at PRP for fibromyalgia or myofascial pain. Though feel free to check out my video on how to take a multimodal approach to treating trigger points!

Creatine works miraculously for my fibromyalgia. Pain disappeared in 5 minutes. I take a teaspoon daily.
Good work Jeff. Just had my first PRP injection. No benefits yet.
Good luck! PRP takes time to work and may take up to 3 injections. As always, discuss with your health care provider.
could you go through some studies of PRP vs umbilical MSCs?
Unfortunately umbilical MSC treatments are still VERY new and there is no robust head to head comparison data between them and PRP. Hopefully they will get done one day, but until then, PRP is still my gold standard for orthobiologic therapy.

In regards to hip labral tears I see this 2019 study (ref below) concluding with:
"Ultrasound-guided injection of platelet-rich plasma holds promise as an emerging, minimally invasive technique toward symptom relief, reducing pain, and improving function in patients with hip labral tears."
Question:
Would you say there's a possibility that PRP might work for labral tears / instability in the shoulder as well? I'm asking because so far I've only found anecdotal stories supporting this.

Am J Phys Med Rehabil. 2019 Nov;
Use of Platelet-Rich Plasma for the Treatment of Acetabular Labral Tear of the Hip: A Pilot Study
Arthur Jason De Luigi 1, Daniel Blatz, Christopher Karam, Zachary Gustin, Andrew H Gordon

I think it’s BS because they didn’t even use PRP. And I don’t even think they know what the actual cell count was, or even have a way to verify it. Anyway, just annoys me because these therapies are the future.

Do you mean varicose veins? Stem cell injections have not been studied for that.
You’re mis pronouncing Mesenchymal every-time you say it ;)
Ha! What is your preferred way of pronouncing it?
Will have to say this does not work if you are completely bone on bone. You need cartilage to grow. 8K wasted for me. I know it helps with some though.
You are correct. People with grade 3 and 4 osteoarthritis do not get as good of outcomes as those with grade 1 and 2 arthritis.

"The results of this study demonstrate that PRP therapy delivers ng/mL-range concentrations of catabolic proteases, which could perpetuate inflammation and inhibit tissue healing." ref:
pubmed.ncbi.nlm.nih.gov/24627579/
This statement seems contradictory to everything else that I've heard about PRP Can you pls clarify?
This is a great example of why in vitro studies are not the same as in vivo studies. MMPs are associated with worsening cartilage destruction and osteoarthritis. So you would think injecting any solution with MMPs would be counterproductive. But in actuality, synovial fluid analysis after PRP injections show DECREASED inflammatory cytokines (TNFa, IL1b). PRP injections have been shown to decrease the progression of osteoarthritis. See here for more info:

First of all, I want to thank you for all this information. I have watched tons of videos, and I just hate when people say “studies have found”, I’m a chemist, so proofs and information is really important for me, and I love the fact you do show the publications, and I have looked for some. That’s very profesional
Now, I have a couple of question.
Aim for more fruits and vegetables (ie. anti-inflammatory diet)
5. Weight is critical. Adipose tissue release inflammatory adipokines which are problematic in arthritis.
