States like Virginia and Texas restrict them at intersections because of their DOT engineering judgment, not because of state law.

True, but LTOR isn't a thing outside of one way to one way in Indiana. In those cases, a red left turn arrow would be redundant. All traffic moves on all lanes of one street or the other at the same time regardless. We've moved more of our left turn lanes to turn red, then flash yellow after oncoming traffic is moving through the intersection, unless there's a safety reason not to do so. Most of the signals here in Indy like that are red ball and yellow/green arrow signals.

Right turn (or left oneway to oneway) on red arrow is only prohibitted in 15 states, including Ohio. Only 5 of those states (including Ohio) allow a sign to override that restriction. It is permitted on a red arrow in the other 35 states. Edit: 7 states do not allow left turn on red at all. Only two them prohibit right turn on red arrow.
Signaled intersection always takes that into account. That doesn't mean you can plow through that wayward pedestrian, just that the light should've told him not to be there.

That's fair, in many countries, red light with arrow simply means that you do not have right-of-way. The interpretation of this red arrow light in US is weird from my point of view. It's not a ball red light, which you must stop first, then turn right if it's safe to do so. But for red arrow light, it's pretty natural to stop until it turns green. Drove in China once and I agree with you, it's way better.
These are so dumb. We have these in Tampa at the end of offramps and they have two right turn lanes with two of those red arrow lights. You can run those lights right in front of a cop and they won't even stop you.

In most states, the national MUTCD guidelines are followed, and... as you state... you are not legally permitted to turn right on a steady red arrow. If Indiana DOT is going to allow right turns on red after stopping for red arrows, they need to allow that red arrow to flash... which is done in some other states, and is legally permitted, per the national MUTCD. People understand that flashing red means stop, and then proceed if safe to do so.
Don’t forget the yellow trap condition that can happen with this right turn signal paired with permissive/protected left turn signals (also prohibited by the national MUTCD).
