I hope one day I could find a dedicated professional such as you, Sir. I have a root canal treatment on the same teeth your patient in the video has it; it was because of a trauma. The thing is the doctor I went to did not even took an x-ray or covered the teeth around during the process. Now after 2 years it has been lightly discolored again and I'm too afraid to loose my teeth or receving a poor treatment without the correct hygiene. This has affected my self-esteem tbh. Unfortunately I'm located in Puerto Rico.

Of all the internal bleachings I have performed over the years, I have had one tooth darken one time following endodontics and internal beaching. The tooth initially bleached whiter than the adjacent central incisor, then darkened a bit over a year or so. I do not know the reason it happened. You could probably leave the bleach mixture in the tooth for several weeks with no problem. If you were leaving it that long, my understanding is you would want to change it every 2-3 days because it looses its potency. I have found the tooth is bleached as much as it will bleach within 3 days to 1 week. It is interesting that I have had a few internally bleached teeth that actually bleached lighter than the adjacent teeth, then 6 months later turned darker again. I am not sure what the reason. I always tell patients that dark teeth may end up needing veneers in addition to the internal bleach, just in case that redarkening phenomena occurs. Take your practice to the Top Tier. Internal bleaching usually lasts many years. Occasionally, the tooth will initially internally bleach ideally, and even bleach lighter than the adjacent teeth, but some darkness will return. I have asked several endodontists why some of the darkness occasionally returns after several months, and still do not know the answer. Luckily, normally the tooth remains bleached indefinitely. I prefer home bleaching with trays to in-office bleaching, like Zoom, because the in office bleach is so strong patients with gum recession and any root surface exposure have hypersensitivity. The at home bleach gets patients to the same degree of bleaching, it just takes a few days vs. one appointment. With the at home, patients can bleach once or twice a day for 30 minutes if they have no root surface exposure. If they do have recession and root surface exposure, then they may bleach only 30 minutes every other day, take 2 Advil prior to bleaching and use the desensitizing gel following each bleaching session.

No, you want the gutta percha or cement a bit coronal to the CEJ so the bleach solution does not blow out through the cementum into the periodontal ligament space and soft tissue. Keep the bleach solution above the CEJ, in enamel.
hi i have a dead front tooth in which I got a root canal done for a couple of years ago, since then they recommended internal bleaching as well as veneer bc the tooth has become darker over the years and i had had braces too (I’m 20 now, had braces which caused dead tooth and had root canal done when I was 17) so my teeth were all good until that. my tooth shape is good so that’s what they’re first recommendation was internal bleaching. i just want to be able to smile confidentially again with normal colored teeth, what would you recommend is best? Definitely, I would recommend internal bleaching initially then, if the tooth is not the ideal shade, consider veneering. Internal bleaching is not as predictable as veneering, but it usually works well.

Dr Steven, how long is the internal bleaching good for if kept up on good hygiene etc? I'm going to get tooth #9 done with some more sessions to get a better result. I was looking into a veneer, but going to try this first. Your help is much appreciated.
The bleaching may last indefinitely. I have had one case the turned a bit dark again after about 6 months post op., but all others have remained approximately the same level of bleaching years later. I do not know why the one case turned slightly darker after 6 months and have not come across any research or any expert who knows that answer. Over all, very stable and long lasting procedure.
You must remove the sclerotic dentin with a slow speed round burr from inside the pulp chamber prior to placing the bleaching mixture. Internal bleaching is not a precise procedure. Litterature tells us to wait a week after removing perborate to prevent the remaining oxygen from hampering composite curing. Did you place the composite immediately after?

@@centerforard maybe this is a European difference but over here, you can also have the opening at the back of the tooth left open, and with this you’ll inject bleaching gel before putting the tray over the top.
Interesting. I have never heard of that technique. If I understand what your are describing, it would seem debris would pack into the "hole" in the back of the tooth if it was left open.
What brand sodium perborate do you use? and if it matters? Is clinical 3% Hydrogen Peroxide mouthwash sufficient to blot the chamber or does it need to be 30%? TIA!
Dear Dr ... Till I know the newest research contraindicates internal bleaching because they causes external root resorption... Dr Pascal magne ( University of southern California) and Dr belser ( University of Geneva) conducted this research. Now what is indicated is to perform an external bleaching to bleach as much it can be. Please read the book and articles from PHD.
It is very important to place crown and bridge cement at the CEJ at the apical extent of the pulp chamber so the bleach remains in the pulp chamber, surrounded by dentin and enamel, and does not extend down the root of the tooth. No problem with external root resorption so long as the bleach does not extend past the pulp chamber into the root.
this will go dark again in 1 year or two. I had been bleaching my front tooth 7 times it always go dark eventually. imo root canal is old way to treat dead tooth. I have replaced all my root canals with ceramic implants in europe im super happy. thoughts?
Glad you got a good result. Occasionally teeth will darken a bit following internal bleaching. Extracting the teeth and placing implants is a big job! Why not veneers if you wanted to lighten the shade of the teeth vs. extraction and implants?
@@centerforard my implantologist explained to me that root of my tooth that had root canal done had small inflammation that trigger immune system at alert state all the time. thats the major concern because when you really need your immune system to react it wont be as effective. Doing veneer is just hiding it.
Interesting. How does he feel the inflammation at the apex (tip) of the tooth gets to the facial surface of the tooth that has had endodontics? The canal is filled with gutta percha and sealer.

I have no pain whatsoever, the tooth doesn't react on cold or hot touch, and I have no darkening spots around the tooth. It doesn't show any change on Xray and it seems to be vital on the scale of 7, after performing a vitalization test on the tooth. What do you recommend to be done in this case? Should I get a RCT and then internal bleach, or veneers after? I'm so confused! Would appreciate a reposnd! Thank you in advance!
Normally, if the tooth is dark it has a dead nerve. The most conservative treatment is a root canal and internal bleaching. If the color looks good after the bleaching, you probably will not need a veneer.
Yes, the nerve of the tooth is necrotic (dead), normally from a blow to the tooth, and blood has gone into the dentinal tubules and turned the tooth dark.

@@centerforard thank you for your replies. I have been experiencing pain few hours after the treatment for about 12 hours. Now that the instant pain is gone, when I bite, there is a feeling of discomfort. Is there any chance my tooth is fractured? The tooth had root canal about 14 years ago.
I am not sure what the problem is. Internal bleaching should not be uncomfortable, during or after the procedure. Perhaps you had endodontics at the same procedure. From the endo. you might experience a bit of post operative discomfort from the injections and filing. I suggest you see your dentist and have an examination.

Many times it is not necessary to veneer the tooth following internal bleaching. The IB is often all that is needed.

What kind of mirror are you using , my mirror is always fogging , is it recommended to use photography mirrors and easy to use ?
Your assistant must blow air on the mirror with the air/water syringe to prevent fogging. I use large or medium occlusal mirrors.

I do not know why the one case turned slightly darker after 6 months and have not come across any research or any expert who knows that answer. Over all, very stable and long lasting procedure.

How do you manage sclerotic dentin from years of a necrotic pulp? I'm having a hard time penetrating this dentin with the internal bleaching.
You must remove the sclerotic dentin with a slow speed round burr from inside the pulp chamber prior to placing the bleaching mixture. Internal bleaching is not a precise procedure. It sometimes works better than other times.

Litterature tells us to wait a week after removing perborate to prevent the remaining oxygen from hampering composite curing. Did you place the composite immediately after?
Yes, I placed the composite immediately after thoroughly rinsing the orafice. I have never had a problem with the composite post op doing it this way.

Depends on the structure of the tooth and the technique utilized. In my office we normally see a significant change within 24 hours.
Dr Cutbirth, thanks so much for this video. I'm just about to have this procedure carried out and thought that I would research it beforehand. I've been given an internal bleaching option or, the option of injecting and topping up the bleach myself. From your experience, which do you think is the better approach?
I have no idea what you are referring to when you say "injecting and topping up the bleach myself."
@@centerforard maybe this is a European difference but over here, you can also have the opening at the back of the tooth left open, and with this you’ll inject bleaching gel before putting the tray over the top.
Interesting. I have never heard of that technique. If I understand what your are describing, it would seem debris would pack into the "hole" in the back of the tooth if it was left open.
What brand sodium perborate do you use? and if it matters? Is clinical 3% Hydrogen Peroxide mouthwash sufficient to blot the chamber or does it need to be 30%? TIA!
Any brand of Sodium Perborate. 30 - 35% Hydrogen Peroxide must be used.
Dear Dr ... Till I know the newest research contraindicates internal bleaching because they causes external root resorption... Dr Pascal magne ( University of southern California) and Dr belser ( University of Geneva) conducted this research. Now what is indicated is to perform an external bleaching to bleach as much it can be. Please read the book and articles from PHD.
It is very important to place crown and bridge cement at the CEJ at the apical extent of the pulp chamber so the bleach remains in the pulp chamber, surrounded by dentin and enamel, and does not extend down the root of the tooth. No problem with external root resorption so long as the bleach does not extend past the pulp chamber into the root.
Hi Dr, can I know the reason why the discolored tooth not turn white after 3 session of internal bleaching? I used hydrogen peroxide 30% only.
Mix the sodium perborate with the hydrogen peroxide. Bleaching requires both. Follow the procedure.
this will go dark again in 1 year or two. I had been bleaching my front tooth 7 times it always go dark eventually. imo root canal is old way to treat dead tooth. I have replaced all my root canals with ceramic implants in europe im super happy.

Glad you got a good result. Occasionally teeth will darken a bit following internal bleaching. Extracting the teeth and placing implants is a big job! Why not veneers if you wanted to lighten the shade of the teeth vs. extraction and implants?
@@centerforard my implantologist explained to me that root of my tooth that had root canal done had small inflammation that trigger immune system at alert state all the time. thats the major concern because when you really need your immune system to react it wont be as effective. Doing veneer is just hiding it.
Interesting. How does he feel the inflammation at the apex (tip) of the tooth gets to the facial surface of the tooth that has had endodontics? The canal is filled with gutta percha and sealer.

Recently I got a discoloured front tooth due to a trauma! I have no pain whatsoever, the tooth doesn't react on cold or hot touch, and I have no darkening spots around the tooth. It doesn't show any change on Xray and it seems to be vital on the scale of 7, after performing a vitalization test on the tooth.
What do you recommend to be done in this case? Should I get a RCT and then internal bleach, or veneers after? I'm so confused! Would appreciate a reposnd! Thank you in advance!
Normally, if the tooth is dark it has a dead nerve. The most conservative treatment is a root canal and internal bleaching. If the color looks good after the bleaching, you probably will not need a veneer.

Yes, the nerve of the tooth is necrotic (dead), normally from a blow to the tooth, and blood has gone into the dentinal tubules and turned the tooth dark.

How long does an internal bleaching last?
I have had some cases where the internal bleach lasted indefinitely and in a few cases some of the darkness returned. In those cases, I restored the teeth with veneers.

I have been experiencing pain few hours after the treatment for about 12 hours. Now that the instant pain is gone, when I bite, there is a feeling of discomfort. Is there any chance my tooth is fractured? The tooth had root canal about 14 years ago.
I am not sure what the problem is. Internal bleaching should not be uncomfortable, during or after the procedure. Perhaps you had endodontics at the same procedure. From the endo. you might experience a bit of post operative discomfort from the injections and filing. I suggest you see your dentist and have an examination.
As I reread your comment, I see you had a root canal 14 days ago. Definitely go see your dentist.
