    Your recommendations make very good sense. LOT OF docs SAY nothing about high arch feet. They seems to approach these issues as’ one size fits all’ to address all plantar fasciitis issues. I appreciate your diligence in approaching the solution for we ‘high arch’ people. Thank you so much.

    Easily the best high arch vid I've found on TH-cam, thanks for the information & tips. Mine are very high & I only really realised it was a thing after recieving physio in the (Royal) Air Force aged 30 (I'm 49 now!) for knee & back issues, the PTI was amazed I'd gotten that far in life just putting up with all the issues they were causing, I suppose it was just a case of coping especially when I didn't know there was even a problem.
    Thanks again, great stuff 😊

    Love your videos and this one was super informative. I just found I have super high arches, and watching this one was an eye opener. I have an ASD kid who is still a toe walker at 10. Do you have any videos on toe walking that cover the damage he's doing to his feet (like this one) that don't dwell on surgery? 99% of what I've found would freak him out so bad he wouldn't watch.

    This blew my mind. I’ve had knee pain, low back pain, and problems with my leg muscles my whole life. I’ve been pushing through trying to stretch out my hamstrings, and calves. Literally, I’ve never been able to do a hamstring curl with more than 10lbs without a tearing sensation. I’ve lost over 40lbs last year, I’m committed to living a healthy lifestyle, but leg day has been demoralizing. It’s not a feeling of muscle fatigue, but muscle tearing. I have to do body weight exercises and even then it’s brutal.
    I love running, but I can’t do it. I started running, and I needed 5 days to recover. I was limping around everywhere. It honestly is just embarrassing. I know I can do more but my legs won’t let me.
    I’ve always known my feet are weird but I never made the connection.
    It still might be something else but at least I have a better idea now.
    I’m going to be seeing a specialist soon.

      I’m in the exact same boat! I’m 28 and have been in the military for 10 years. With rucking, running, and working out, I’ve always pushed through the pain. But over the last couple of years, it got to the point where I couldn’t walk 100yards without having to stop. The leg pain you’re talking about is exactly my case too: a tearing, burning sensation. I did have Chronic Exertional Compartment Syndrome and had a fasciotomy on each leg to release that pressure. It only helped about 20%.
      With my extremely high arches and rigid foot, I’ve worn orthotics so thick that I can’t wear normal shoes. Fortunately, my surgeon recommended the Dwyer Osteotomy. Cut my heel bone and shifted it out 1.2cm. It is to make my foot more aligned as it should be. I’m hoping this fixes the root cause… 5 weeks after the surgery at this point, in a cast, and in a lot of pain. I underestimated this surgery…

    I have over supination issues, to the point where on my right foot, I am almost walking on the side of my foot. So I get the chronic ankle pain / swelling and soreness all down the side of that right foot.
    All this time, I thought my ankle was the issue, but now I am thinking it is perhaps the over supination as wearing a supination OTC orthotic made a world of difference to prevent the foot and ankle rolling out.
    Going to go see a professional now for an evaluation. My whole right side (leg) is also tight due to a past herniated disc on that side and subsequent surgeries due to severe s1 nerve impingement.
    I work from home so perhaps “house shoes” with a good custom orthotic are in order.
    Very informative video. Many thanks!

    Great video. I’m an elite athlete and have issues with a high arch. But the major issue is probably that I have been putting a lot of stress on my 5th metatarsal and have developed a stress fracture. What’s the best thing besides stretching calves, etc that I can do to protect the 5th metatarsal getting too much impact? Cushioning and special orthotics or wedge-ish orthotic?

    Loved the video, very informative, I suffer with CMT. I only wish you could give recommendations on athletic shoes. I tried the Hoka clifton shoe but my instep is so high, it hurt to wear them, couldn't wait to take them off they were so tight. Also, they did not feel to have much cushion at all. I love the cushion of the ON cloudstratus but found that I was very off balance and stumbly with those. I'm not sure what else to try.

    Wow 47 seconds into the video and I'm already feeling attacked 😂😂 I get really bad pain in my arches if I'm stood too long, my heels are constantly in agony and if there was an award in school from the about of sprained ankles I'd win every time! Every other day I'd twist my ankle or I'd describe it as "rolling over" like I walk on the outside of my feet and I have penguin feet like they naturally turn out! Standing with my feet facing forward is like asking someone with parallel feet to point their toes in xxx

    I use to have lots of pain in my forefoot from walking and have had hammer toes. But I associated it with flat feet, even though I actually don't have flat feet, but maybe low arches. I do foot exercises, so my true arch is somewhat hidden a bit by muscle. Unfortunately, after wearing ill-fitting steel toe boots on my job for six years, I have developed hallux rigidus in both great toes, but the left one is much worse. Weight gain is also an issue, but I'm not too far gone. It seems as if I have some high arch foot issues, but I don't have high arches. I wear Birkenstocks and barefoot shoes to try to retrain my feet into the correct mechanical structure.

    I'm just wondering about bespoke orthotics for patients with high archs. The high arch of the orthotic is used for better distribution of force into the medial side of the foot so that the lateral side of the foot won't need to take in all the force? am i correct in saying that

    I have calluses on my feet in places ive never seen on anyone else. When i was a kid i used to complain about shoes not fitting me properly and tightness of the laces, i wouldn't run because of discomfort and when i would walk or run without shoes on i could feel the bones of my heels hit the pavement. Like... zero shock absorption. I have intense hip and pelvic floor pain and my posture is really bad. I still struggle to find shoes that feel comfortable i tend to stick with adidas or shoes that are two sizes too big.

    I have super high instep and arches, to the extent that when weight bearing standing barefoot no part of my arch touches the ground, and vast majority of orthotics provide no contact through the arch either, my feet ar super high volume and pretty rigid but no hammer toes, no bunions. In most shops it is EXTREMELY hard to find shoes I can get my feet into - straps too short to do up, sides would not meet tongue in lace-ups or trainers, boots impossible to get my foot in I used to get pain on top of foot through simply having shoes crush top of foot - but cured that by finding super deep super wide shoes designed for diabetics. Am not diabetic and my feet are not swollen - but simply getting a low-rise, high volume shoe with cushioning at the front made a world of difference. Brookes walker in width 8E provides good cushioning, decent neutral support and wonderful high volume comfort. btw they neglect women so had to go to a mens US 6.5 8E - am a uk size 5 or EU 39. In DB shoes, many styles in width 6V or width 8E - some admittedly a little grannyish - but omg the COMFORT and support !!!!!! Can walk and stand all day without pain! No orthotics needed - just room enough not to crush my foot!!! If you like hiking Merrell has some good options or ( with strap extenders ) the Ecco off-road range is tough, supportive and above all comfortable. I get the leather+velcro strap extenders at DB shoes.

      I have a trick for this, it's to skip the lace holes in the middle, this is something I commonly do for people with high arches or top of the foot surgery/incisions as they get out of their boot. Basically just laces at around the ankle and non in the middle of the foot.

    My feet are very cavus especially my left foot. This causes my right hip lots of pain, and im prone to ankle sprains as well. Wide width shoes and work boots definitely helped my problems a lot... 8 or 10 inch tall work boots are good for my ankles and give me more confidence on uneven terrain.

    I’ve been dealing with Plantor since last summer. First Dr. suggested doing steroid shots which I stopped after 4x within about 8 months. Next Dr. said I have Cavus foot and asked if I wanted it fixed, obviously I said whatever it takes. He then put me in a boot after taking a custom mould of it. 5 weeks later I go in for my mould and I guess that’s it. 2-3 weeks in and still pain, nothing is fixed and I’m out $1800 for something I could of gotten myself on Amazon for $150. That’s 3 different doctors now that’s done nothing but collect money. I need a real doctor and don’t understand how anyone can call themselves a professional and be this poor at their ineffective profession. This doctor sounds like he knows what’s going on. Too bad he’s not closer

    Do you think for people with high arches it is possible to strengthen the feet and the arches to then be able to go without orthotics and a heel lift?
    My thinking is that after strengthening the feet and learning to walk without pronating or supinating I could transition to barefoot shoes.
    I did it for a week or 2, then after a few days (only wearing the barefoot shoes for 1-2 hours a day without long walks, mainly at home) my right knee started to hurt so I put my orthotic in, then after a few days, and practicing completely barefoot to be aware of my arch, I put the barefoot shoes back on without the orthotics and even a few days of walking my knees didn't hurt.
    I then overdid it and my achilles has been hurting ever since and I am back to my asics with a 8mm heel lift and my orthotics until I recover.
    I wonder if my injury is connected to my high arch and regardless, if you think or know that high arched people can move to barefoot shoes and say goodbye to their orthotics. Doesn't have to be a barefoot shoe, even altras, I just think it would be easier to manage without orthotics with a very thin sole.

    As a kid I was told I had high arches and a bone out of place in my feet. The Podiatrist built supports into my shoes and that helped. Question- the soles of my shoes wear out first on the outside because I seem to walk on the outsides of my feet. What causes that? Thank you for such good teaching. You are really blessing many people.

    I have a family member, with sciatica nerves damage, that goes down into her legs. When she sits down, the pain goes down into her legs, causing nerve pains down into one of her legs. I always believed that, a lot of her leg pains, are coming from my lower back. This pain causes her, not to be able to just stand right up and be able to just immediately walk. It always takes her a minutes, for the feeling to set in, then she's able to take steps. She says, ice is her best friend, laying it on her problem areas. friend It always seems to soothe those nerves. Hot then cold water, when showering, seems too also soothe those nerves. Now one of her ankles, is turning black, Soaking it in Epsom salt and vinegar, seems to help and putting ice on it at night. She did something very stupid and tried to shave that leg a few days ago. Causing a small nick of her skin to fall off, which caused a burst of blood to spray out of her leg. She ok, had to be rushed to the hospital, because the leg was pouring blood out of it. No one advised her, that she couldn't shave her legs and this could happen. She's getting shocking pains, from the ankle, halfway up the leg. She's seeing her primary physician twice about this and no one, seems to not help her. She looked on TH-cam, to answers and help. The exercise you gave her, it helped right alway. But every time she sits down, the pain kinda start back. Shocking pains, near the ankle and the side of her lower leg. I believe this problem is coming from the scientific, causing leg deficiencies. I believe This problem came from my, varicose vein surgery surgery. Their were two doctors from the same practice, that preform my surgeries. Doctor Gregory, from Empire medical, in Maplewood New Jersey. When he performed my left leg Varicose procedure, everything was fine. But then doctor Malovin, from the same medical group. Suddenly he wanted to do my right leg, during his procedure, It was very painful. I have feeling pains in this area and on the front bone every since. Even my knee replacement, that didn't turn out right. Which caused me surgery twice now and looking at a third surgery as well. I believe, this problem started from the varicose bad surgery. The seond knee surgery caused more nerve damage, up in the thigh area. After the second knee surgery, after waking up, the thigh was very swollen and hurting. After the knee replacement, every sense, I have no feeling in my knee area and half of my lower leg and ankle. I had my varicose surgeries in 2018, into 2019, with my problems worsening. Then the knee replacement, in 2019 and the second surgery to that knee in 2020. My leg won't straighten out all the way or bend all the way and still lost of feelings. Every sentence the varicose vein surgery was done, I feel like something was left in my leg and it hasn't healed right. The way Doctor Malavin was shoving that probe up into my leg and this leg hasn't been right every sense. It seems that if I don't think for myself, I will not get any relief with this problems. I have been coming up with some good exercises, that has been helping a lot. But after finding your TH-cam page, thank you for helping, with the suggestion on leg and ankle nerve pain. Using it helped right away with that sharp pains in the lower leg area.

    I found that addressing supination (in my case) would have made a big difference in the long run, and it's not easy to do it by yourself. After too many episodes of stress fractures in my metatarsals, I had custom insoles made 15 years ago, but they were too bulky and uncomfortable. I stopped using them but always made sure to wear shoes and insoles with good arch support. The thing is that most of them were just not suitable for correcting supination! Tbh I had no idea that many people with high arches had pronated feet instead. Anyway, I didn't get very good advice about this and after years, the supination took its toll on my ankles - if only I knew before. If anyone has good insoles for high arches and supination to recommend, I'm all ears

    • @Leo-nc3yx

      I suffered from high arch feet before I even noticed it.
      It's actually very weird, I used to go hiking and play many kinds of sports back in the day, until one time when I was stretching to save a ball(tennis), my right ankle
      just went numb and throbbing pain came right after. Probably shouldn't go hiking the day after, it nearly killed me in terms of pain.
      And then I suffered for 2 years, went through a lot of rehabs,pain-killer both pills and injections, to the point that I was frustrated since it feels like the pain can never go away. I'm not going to say my life is miserable after that, but it does take away a lot of fun from me, I was injured when I turned 23, and now I'm 25, can't
      go on a hike anymore, can't do an extreme sport, can't run like I used to.
      Then one day I go to the hospital for an MRI test on my sore knee, the doctor said
      it was just Tendinitis, then I told him I have been through this on both my knee and ankles. He took a look at my feet, and said something like "You got one of the most high-arching feet that she had ever seen".
      After that, I have been wearing some sort of custom insoles like you, should probably go to the capital for a made-to-offer, but I got a cheaper one and settle with it. Can't say the pain really goes away, it still hurts at times, especially after I do some running.
      The doctor's explanation to why I seemed to have no problem before, it's because when I exercised often I built a strong muscle in my lower body, but once I got injured I have to stop running, hiking, training, and all that stuff to gain or maintain my strength. Now my lower body isn't as strong as before,my high arch feet became a problem for me since then.

    • @anaserta
      @anaserta 4 หลายเดือนก่อน

      Exercising makes absolute sense but after going through lots of pain, physio, injections etc I realised that for me no relief was comparable to the right insole. This is all so tricky specially because many doctors don't really seem know the issue that well and we end up suffering with it longer than needed. I've been to two sports physios in London who had no idea of what was causing my ankle issue. Then my previous dr back in Brazil told me this is an expected "phase 2" of having very high arches - which according to him tends to create friction in the ankle over time so these tendinitis start showing up. I had new techy insoles made recently and they really changed my life. They now address high arches + correct supination + have a little bit of heel to create space in the ankles. Anyway, it makes total sense to strengthen these areas but if there's still pain I'd highly recommend getting the best insoles you can for your feet. I spent years thinking I couldn't walk a lot or hike anymore but that's possible now!@@Leo-nc3yx

    Alright people! In his demonstrations he was using a skeletal model of the foot that doesn't include the tendons, ligaments, and muscles in your feet. If you know anything about engineering, then you would know how structurally sound the arch is. When you support the arches of your feet, you are not allowing the full function of the tendons, ligaments, and muscles that support the arch. I was a long distance runner for years and I was always told to wear support shoes with Powerstep brand orthotics in them to support my high arches. I suffered from ITB Syndrome chronically because of the misinformation I was given. The over supporting shoes caused my feet over supinate and derailed my running career. I switched to barefoot shoes and have noticed a vast difference in my foot health, the stability of my foot, and better forefoot striking when running. His videos showed a lot of heel strikers and that is not how the running gait is supposed to work. I highly recommend you to conduct your own research, but I believe you will find that a wide toe box, zero drop from heal to toe, and minimal cushioning are best for your feet. Whether you have high, neutral, or low arches it is the way were meant to walk or run. I suggest starting with Altra shoes first, and then progressing to VivoBarefoot, or another minimalist brand.

    Hi Dr. Tom, quick question. When you say "high-arched feet are associated with neurological conditions 85% of the time," do you mean if you have high-arched feet you have an 85% chance of having a neurological condition? Or the other way around? I'm curious because I do have high-arched feet but have no diagnosed condition and didn't know if it was something concerning enough to have checked out by a doctor. Thank you!

    • @DrTomBiernacki
      @DrTomBiernacki  ปีที่แล้ว

      The thing is they are not premade, it can cost base $125 to have them printed, then consider scanning/plaster, mailing, time and visit. For most people not worth making if less than $2-300 minimum as they would be made at a loss. But that being said I do sell my premade ones for way less than superfeet in clinic ($20) and think they work much better. But for the 30% or so that need custom, it can make all the difference.

    You said for high arched feet you want a flexible, high cushioned shoe, but that is an oxymoron. Especially with the shoe that you showed us as an example. Try and twist and bend that shoe. It doesn’t. The higher the cushion in a shoe, the less bendable it gets. I’m curious, which is it: do you want a flexible shoe or a cushioned shoe?

    • @DrTomBiernacki
      @DrTomBiernacki  2 ปีที่แล้ว +2

      I should be more clear, high arched people generally when younger and healthier can tolerate and enjoy less rigidity. On the other hand the Hoka is well loved because even though it is supportive, it has an extreme amount of cushion that provides the best of both worlds. A rigid uncushioned shoe like a dress should might be very uncomfortable or a lower quality running shoe.

    Wait so how do you actually fix the high arch?

    • @DrTomBiernacki

      Not so much fixing the high arch but fixing the pain and problems associated with it, I guess I should make that more clear looking back at the title

    🔴🔵🟢 any shoe brands that have nice looking shoes I can wear at church? I don’t mind wearing ugly shoes but do need nicer ones for church. I think I have a pretty high arch

    Hoka has a VERY low heel to toe drop, 4mm or less. Not good for people who need 8-12mm as you mentioned.

      @DrTomBiernacki  ปีที่แล้ว +1

      You're absolutely right, I just checked and was surprised to see they average about 5mm,

      Maybe I had it confused with the Brooks beast which I believe is 10-12mm range heel drop

      @@DrTomBiernacki I'm not sure that I have a high arch. I think I have a medium arch. But lately I've developed terrible Peroneal Tendonitis, lateral foot pain and Insertional Achilles Tendonitis with a retrocalcaneal bone spur. I think I've tried them every recommended shoe there is in hopes of finding a shoe that does not cause excruciating Insertional AT pain and that relieves my Peroneal Tendonitis. The only shoes I've found so far that are both comfortable (no pressure on the Insertional Achilles area, no heel pain) and supportive, are the New Balance 990v5. They have a 12mm heel to toe drop. I've not gotten any help from the podiatrists and ortho/sports drs I've seen. None suggested orthotics or much of anything else to help me. When I asked the Ortho about the lateral foot pain I have in the Peroneal Tendon area, around and under my ankle and under my foot, he just brushed it off with a few words "that's coming from the Insertional AT". That's it. Nothing about how to treat it. I am really receiving sub-par, inadequate care from these doctors and it's very discouraging. But they are in my health plan's network. I'll be changing that during open enrollment in Oct. and I can't wait to move on, hopefully, to quality doctors who actually care. One question I have for you: Is peroneal tendonitis relieved by a high-arch insole? Does it need a valgus post? I've been wearing Powerstep Pinnacle Blue inserts. I tried Powerstep Pinnacle Maxx but they seemed to put more pressure on the outside of my foot where the pain is. Is that because the Pinnacle Maxx inserts have a heel post that tilts the foot outward? Thanks for your help!

    I picked up running 2 months aggo after 20 years of inactivity. I average roughly 40 miles a week at this point. When i push my big toes downward the center of my arch gets a sharp pain. It's beginning to affect my runs. Eventually id like to crank it up to 50 miles a week. Anyone have any suggestions on a good long distance shoe for a high arch but also a shoe that doesn't hinder speed?
