If you have never had Cialis (Tadalafil), don't bombard your body with 10mg-20mg of Cialis. Take as low as 2-3mg daily. After taking it for a few week or until you feel some side effect, stop taking it for a week to let the side effect subside. Then come back on the routine of 2-3mg daily. Repeat the cycling on/off for 2-3 times, until you don't feel any side effect for a whole week. Then you are pretty much safe to take is long term. For daily intake, it is recommended to keep the dosage under 5mg. By the way ,there is one VERY IMPORTANT thing is, DO NOT eat GRAPEFRUITS, since it would prolong the half life of the medication and enhance the side effects. It would be much better if you cut or reduce orange and lemon, too. Lime is OK.
My urologist gave 5mg daily to start. Minimal side effects, worked the first morning urination. The only problematic side effect was morning wood but that mostly subsided after about a week.
Im glad you said that, i was going to take some just so my partner and i can go all night like we used to years ago, problem is she loves swallowing uhmm milk? and in order to make the stuff smell and taste a little better i usually load up on fruits like citrus pineapple grapefruits etc etc. that would have been super embarrasing.

At 54 years old, I regained my strength after undergoing triple coronary bypass surgery in 2015. In 2018 I consulted my cardiologist and urologist who told me about the benefits of tadalafil for the prostate (despite having no problems) and for my heart, so in 2020, during the pandemic, I decided to give it a chance. Today my wife and I celebrate my 5 mg daily dose. On weekends I celebrate with wine or whiskey with no side issues. Tadalafil is wonderful!

There are many health benefits of Tadalifil (Cialis) like lowering blood pressure, increase blood flow for work outs at the gym and of course in the bed room, relaxing the blood vessels in the lungs to allow blood to flow more easily and so many more!. As a EMT, please use these medications as instructed by your Dr. If you need them, cool, just dont try to take extremely large doses. I have seen a pt try to get really ready for a date night by taking 3 of his 100 mg pills. It dropped his blood pressure to a dangerous level. No he had not taken Nitro. He was ok in the end, but it could have led to a really dangerous situation. So take the meds as prescribed please.

Great video! My doctor prescribed 5 MG of tadalafil per day to combat my BPH problems. I've been on it for about 4 years and it has worked great! Much better urine flow and I usually only get up once a night to urinate as versus 6 to 9 times before I started the treatment.

After radiation treatment for prostate cancer, although the clinicians I met provided excellent support and information ahead of time; still it was a wake-up call to gradually experience a loss of libido and functioning over a few years. As some of you might know - it's something that became normal at age 14-15 but now it is no longer the case. To put It bluntly, it's a bit shocking to have this be the 'new normal'. So, thanks for the info and the honest discussion. It helped me open a conversation with my doctor. So, my doc is aware and tuned in. And I am... better equipped.

I have been on Tadalifil for over one year. My prescription allows up to 4 5mg tablets daily! I am 78 years old with BPH and have had TURP surgery 2 years ago! My response to this drug (taking 1 tablet twice daily) has been amazing. I wish I could speak to the sexual aspects with corroborated corroboration from my wife of 57 years but it takes two to tango! Nonetheless I do have significantly more blood flow to my nether regions as exhibited by mornings like I was 1/2 my age! Firm erections with two pills daily are very sufficient for that result! But the biggest differences I have experienced are: 1) improvement in BPH (as indicated in the video) 2) more importantly my long term memory has definitely improved and 3) my balance is as good as it was 30+ years ago 4) during cardio workouts I have much better blood flow to heart and lungs (as a result of this and loss of body weight I have been taken of metropolol for the 1st time since my quintuple bypass surgery 21 years ago (my LAD was 100% blocked); The previous effects are priceless! The other thing I believe, based on a European study of Viagra, it is reported that long time users of that drug have virtually a 70% less chance of developing Dementia! I have to believe this same benefit would translate to Cialis!

I'm 64 and don't have any underlying health problems at all except a slight ED. Don't take any medication apart from the below. I take 50 mg of sildenafil and the effect last for 12-15 hours for me. The only side effect I get is a little flushing. Being slightly over weight I don't mind.

Im 63 , I have BPH and OAB .So life changes , I am caffeine and alcohol free . This helps OAB . I was on finistrade to shrink my prostrate for about three years . But it affected me long term , now I take 5mg tadafil, 1mg toltradene and tamulsin . I have sleep 7 hours no night peeing . I have asthma and CPAP as well . Fortunately I am in the UK so drug costs don’t affect me .. also get an annual psa blood check

DON"T be EMBARRASSED about anything you need to talk to your female doctor about. She has seen and heard it all! I am a straight, heterosexual man married to the my straight, heterosexual female wife for over 45 years after pursuing her for five years. I will be 72 years old in a few days. When I moved to to my current location five years ago I looked specifically for female doctors in the areas I need. Why? Based on my past history women doctors seem to be more open to answering questions I have than male doctors. And there is NOTHING SPECIAL about my body or any condition that they haven't seen before in their practice, i.e., other male patients. I have talked about several " intimate" items with my general/family physician, my dermatologist and my urologist. I have ED (and take 20 mg Tadalafil, effective for more than 36 hours for me and no side effects), I have urinary incontinence for which I take two medications which don't work 100% of the time so I wear a CVS Men,s Shield (cheaper than Depends and works just as well) every day. Most of the time I dispose it unused, but then there are those other days. I am obese, have varicose veins, have sebaceous adenoma all over my body, and do not look as handsome as I was when I got married (most of time *I* don't like to look at my naked body in the mirror after a shower). So I have a female general/family physician doctor, a female kidney doctor, a female gastroenterologist, a female ophthalmologist, and a female urologist although she is a PA-C which is a Physician Assistant with certification from a national organization and she works under a male M.D. urologist but I only saw him once. There is no female dermatologist where I live so I have a male dermatologist and my female dentist is retiring real soon so I will be changing to a highly recommended (by her and an orthodontist I know) male dentist. Again: DON"T be EMBARRASSED about anything you need to talk to your female doctor about. She has seen and heard it all!
Anthony how do you determine you have ED. Let say out of 10 times your sex didn't well as expected 5 times will you call it ED or you call it average and it is below that ?

Was prescribed tadalafil after IRE (the “NanoKnife”) treatment of Gleason 7 score for prostate cancer. I’m 90 days out now…no incontinence or ED…and found that the medication improved my urinary function markedly. For a 74 year old guy, that’s a nice win. My wife, of 52 years of marriage, probably finds my attentions rather annoying by now. Interesting problem to have, suddenly.
I take half a 20mg Cialis twice a week. I find my mental clarity improves as well as urinating is more comfortable and freeflowing. I sleep more easily and have better exercise tolerance. Also hanging more on the slack makes me feel more confident. Cialis is a lipophilic substance so absorbs better with fatty food to stimulate bile secretion. Tadalafil lasts 36 hours - thing is after 4 hours, she's sore and the cat's nervous...
I split a 20 mg into 4 parts, so I kinda take 5 mg daily when I am sexually active. The effects kinda builds up after each day, so I kinda take a dose one day prior or at least half day before. Good shit
@@itanasumerian7379 well when it was prescribed to me originally, the doctor suggested so because it has a much longer half-life invivo. So it's more convenient to take when need is anticipated. Both Viagra and Cialis are PDE5 inhibitors, an enzyme that reduces NO stimulation of blood vessels to expand.
I was prescribed 5mg/day TADAlafil (you see what I did there) and told to give it a week to start working. On day 5, I woke up and was damn near scared to death at the thing staring at me. I don't recall having erections like that as a teenager, and I have not seen such pronounced veins on anyone other than the arms of athletes. I went from just growing to near erect size when aroused and probably unable to push it through the center of a glazed donut to sporting a chubby that could win an arm wrestling match. Oh, the joy of modern medicine!

I am a physicist who analyzed data professionally for 40 years. I have also done the statistics portion of a Health Data Science course at the Masters level. Rena is the second YoutTube doctor who expresses a bias towards Meta-Analysis, or combining multiple studies. I have mis-givings about this and I would recommend health professionals treat meta studies with caution and read them very carefully. 1. In theory increasing the number of samples is good. However, this is only valid if the sample populations are very close to identical. Combining studies, this is highly unlikely. So is not a strong argument in favor of meta-studies. 2. Review studies may put good and bad studies on an equal footing. This is very bad, especially as Drug company papers tend to be heavily biased in favor of the drug under study. A possible tactic would be to flood the literature with favorable, but poorly done studies in the hope that the bulk of evidence in a meta study will still favor the drugs. As an example, many well done meta-analysis in the last five years have found Statins have a number-need-to-treat (NTT) between 42 and 500. This basically means they have no benefit, yet statins are one of the most prescribed drugs and the most profitable drugs in history. This is my professional opinion as an expert in statistics and data analysis.
I am only 40 and started to have ED at around 38. Urologist got me on both but i am still struggling- ugh. I am on high blood pressure med and i think thats whats started the whole thing.
At 1:42 you explain that once the penis ejaculates, the nervous system revs up causing the smooth muscles contract, forcing the blood out of the penis. However, the immediate loss of erection after ejaculation does not apply to all males. Some males are able to have multiple orgasms (>5) without losing their erection and having to stop coitus. In fact for some males, having 3 orgasms without stopping coitus is not an issue. In addition, their recovery time after having multiple orgasms and losing their erection can be as little as 3-5 mins (although each time they restart, the number of multiple orgasms drops). Of course, as these men age, the number of multiple orgasms they can achieve seems to begin to reduce in their mid 30's. I should also note, physical effort during sex has a strong effect in the number of multiple orgasms said males can achieve; i.e. if the partner is on top doing most of the work, the multi-orgasmic males can easily have many more orgasms, in contrast if they are excerting a lot of effort, like lifting their partner during coitus, then orgasms dramatically drop (even to just one orgasm, if the partner is providing no "lifting" support).
I would add the removal or major reduction in the consumption of many complex carbs as with cutting back on processed foods to aid in the increase of testosterone.
It all fell apart for me over the last couple of years. Now Im 50, over weight about 30-40 lbs, high BP, high Cholesterol, started TRT over 1 yr ago, have ED use Taladafil at times. I do work out lift weights and very strong but ive continued to decline in energy motivation every thing. Ive had untreated sleep apnea for a long time because I couldn’t do the mask thing. It finally got so bad I couldn’t stay awake during the day. I finally made myself use the CPAP and its been a life changer!! I sleep great feel great with lots of energy and i have erections at night all night long when i sleep! Everything is now getting better. I started Carnivore diet as well hoping to be meds free in 6 months.
I've tried both recreationally. Viagra doesn't do much for me. Cialis works great. Side effects are harder with Viagra. Headache and backache. Viagra seems to give you one big bang. Cialis seems to give you staying power multiple times for a couple hours.
Responses vary with the individual, along with age and underlying conditions. For me 5 mg daily Cialis supplemented with vardenafil "as needed" does the trick.
My doctor prescribed me sildenafil 50mg. I split it in half and took 25mg. Worked like a charm. I don’t have psychical ED but probably anxiety/performance is causing it. I asked my doctor what are the chances of priapism and he said 2% but I don’t think he really knew and gave out an answer. Is it really 2% that high?
I'm 75 yrs old, 'n have been taking Tadalafil 20mg tabs daily for 2 yrs now, it's a great 2-fold med, as I'd developed BPH when I was in my 40s, so they gave me an Alpha blocker for the urinary problem, but it's side effect was ED. Now that I've been on Tadalfil NO MORE PROBLEMS, as my sex-life improved dramatically. My last 2 GF's, one in her early 50s, 'n the other in her late 30s, said I was like a young man in bed. I also take an L-Arginine supplement cap every day to replenish Nitric Oxide.
I first took Tadalafil at age forty and had side-effects of acid reflux, ankle swelling, and a small degree of high frequency hearing loss, all temporary. Returning to it over ten years later and I don't seem to get any side-effects at all. One additional piece of info: Tadalafil has a half-life of seventeen hours, so after the thirty-six hours of supposed effectiveness, you roughly have a quarter dose left in your body. I find a full 20mg dose effective for three days and most often just take half a tablet. The only thing it can't help with is completely spontaneous sex with an ongoing partner, which is a shame but I don't see any way around that.
@@hukssumi Short answer is no. It used to be prescribed as a daily dose of 5mg for pulmonary hypertension but is not used for that purpose these days. As an ED med it's prescribed at a higher dose (10mg+), to be taken as needed and a dose is effective for two to three days.

I do have a question though, I take Tadalafil usually 10 milligrams every two days. I also take Tamsulosin HCL for Prostatitis and I know it can effect my erections and ejaculation. @@Femaqui07 benign prostatic hyperplasia … an increase of prostate size that is not dangerous but often uncomfortable . Cialis , in it’s 5 mg format , is , amongst other medicines , a possible solution . It helps me (I have BPH too) .

I'm a senior with reasonably controlled Diabetes and diminishing control over many years of having high blood pressure. I've been using the ED meds off and on as long as they've been available going back to before I had Diabetes and the hypertension was better controlled. My personal experience along with the experience of most other men I've discussed this with, is that Sildenafil is much more effective for ED. I'm surprised that most comments I've read here report opposite results. When I used Tadalafil for a couple of years I found it helpful, but it provided a significantly less vigorous erectile response than Sildenafil, and I never had much confidence in it on the 2nd day - and the extended period of effectiveness is really the entire point of using it (aside from the BHP issue Dr. Malik discussed). There were times when I felt the need to take 50mgs of Sildenafil to supplement the 36 hour Tadalafil I had taken the previous day when I anticipated sexual activity on that 2nd day. I am wondering if there is any significant risk to combining the 2 as I did during the last 12 hours of the 36 hour Tadalafil window? Btw, I never had any side effects from any ED medication other than some nasal congestion on some occasions, including the occasions when I combined the 2 meds.
Thank you for this info. I found as I approached my 60's my erections were not at full capacity all the time. I am a retired athlete and this bothered me. I experimented with low doses of L-Arginine and that did the trick. At 1 to 2 hours before intimacy (NO alcohol) my erections were as strong as ever. After a couple of years I discovered I was born with bicuspid heart valves (now corrected), that was a contributing factor according to my surgeon. In my opinion pills are a good gap filler for a more serious psychological/physiological issue that 'must' be addressed. Do you agree? Thanks again, I love your information.
these substances only inhibit the reduction of NO in your body... but if your Testosterone is low, not enough NO is produced... therefore it is smarter to start trt... On top of that you could take these pills...

My testosterone is normal now and on the high side for my age. Fitness and diet keep me at a peak performance. Thank you so much...
Citrulline may help too. Could try beet powder.It’s supposed to provide nitric oxide. I don’t much like the taste but if you mix it with juice, you can barely taste it.

I use Cialis. When I take it, it makes me feel terrible. Red face,. weirdness. pressure. Headaches.... Nothing worse for a guy to take this stuff. "hoping" his wife will be in the mood, and she isn't. A MORE PART of enduring taking this stuff is a MERE PREDICTION of when your wife, or girlfriend is in the mood. I had a major spinal injury in 2001 after which I spent years in recovery for numerous conditions in addition to the burst fractures of the vertebrae. One of these was ED and I was prescribed both these medications and both had really bad side effects for me (Viagra was the worst), migraines and severe lower back pain. I was already suffering chronic pain in the lower back but these meds just made it so much worse. I had to stop taking them.
I only take Cialis (5mg) three times a week. I don’t use it for ED, more for my pre work out! Never had side effects only good things in the gym and it helps in the bedroom lol!!!

On Dec. 11 a study was released showing that Viagra could be an effective treatment for Alzheimers disease. It was a small sample size but the results were impressive.
by the way for people that need those ,it can often be because of narcotics ,if you take heavy sleeping pills they affect testosterone so over a long period of time it'll make issue

I'm 77 yrs old, high blood pressure and only take one med for it but want to get off meds. because of side affects, always tired through the day. I'm going to ask my Urologist to put me on Tadalafil instead of my other medication to lower my BP and hopping to fix my ED problem at the same time.

I am now 62 years old and have had a 'fwb' with my sweet neighbor, a widow who is now 80 years old, for 15 years, according to mutual agreement. 2 years ago I had some ED problems and first tried sildenafil, then tadanafil (daily). This last one actually suits us better because we can have sex whenever we want, which is quite often. What an invention!
One side effect I noticed is aging quickly symptoms, if it was used by women they would noticed , but men do not care about aging they only care about the moment.
I didn't erectle dysfunction, but I ordered some cialis because I wasn't really physically attracted to my wife and just needed a little boost. Unfortunately, to this day, I swear that it caused me to randomly get low back pain that took a good week to go away. I just woke up in the middle of the night in incredible pain. Still, if I ever needed to use it again, I would opt for cialis.
Oh yea. Those muscle aches are no joke. It's like skipping excercise for 6 months and randomly running across the state. That's how sore it makes the muscles.

Cialis daily made my condition worse long term. Got to the point where I was completely non-functional. Took me over 6 months of not taking it to recover. Still not great but I'm not going back on that again. You also forgot one side effect (at least of Cialis): it makes sexual activity not feel good. Miserable even. Developed hypertension several years AFTER I started having ED, so that didn't cause it. Nobody ever did any investigation into the cause; just repeated a memorized explanation that sounded like a textbook. Just assumed hypertension figured in, ignoring that it post-dated the problem. And I've been to a dozen doctors in 4 different states over about 15 years. NOW they say, "well, you're in your late 50s ..." but they don't listen when I say it started in my early 40s! My sense is they're all flying blind and repeating theoretical stuff they read, then writing prescriptions. For me, I think I need to know what's causing it because the assumptions they're making have no basis in my particular case. But I've pretty much given up by now.
@@bgoodorhell4u Way past that. One of the first things I did 15 years ago. It was somewhat low, and so was vitamin D. 6 months of supplements and it was a bit below the center of the normal range. Also, testosterone affects libido but not ability.
I think you forgot to mention that regardless of which you take, you still have to have desire to trigger the outcome. If you are with some one you don't have a desire for then these meds may not help.

If you take 10 Viagra and a girl walks in that mentally and physically turns you off then there's nothing going to rise for that situation. The studies she referred to all included men that were satisfied with their partners. Not men that started out horny then found something about their prospective partner that was a real turn off. There has to be a trigger.

If you have never had Cialis (Tadalafil), don't bombard your body with 10mg-20mg of Cialis. Take as low as 2-3mg daily. After taking it for a few week or until you feel some side effect, stop taking it for a week to let the side effect subside. Then come back on the routine of 2-3mg daily. Repeat the cycling on/off for 2-3 times, until you don't feel any side effect for a whole week. Then you are pretty much safe to take is long term. For daily intake, it is recommended to keep the dosage under 5mg. By the way ,there is one VERY IMPORTANT thing is, DO NOT eat GRAPEFRUITS, since it would prolong the half life of the medication and enhance the side effects. It would be much better if you cut or reduce orange and lemon, too. Lime is OK.
My urologist gave 5mg daily to start. Minimal side effects, worked the first morning urination. The only problematic side effect was morning wood but that mostly subsided after about a week.

At 54 years old, I regained my strength after undergoing triple coronary bypass surgery in 2015. In 2018 I consulted my cardiologist and urologist who told me about the benefits of tadalafil for the prostate (despite having no problems) and for my heart, so in 2020, during the pandemic, I decided to give it a chance. Today my wife and I celebrate my 5 mg daily dose. On weekends I celebrate with wine or whiskey with no side issues. Tadalafil is wonderful!

As a EMT, please use these medications as instructed by your Dr. If you need them, cool, just dont try to take extremely large doses. I have seen a pt try to get really ready for a date night by taking 3 of his 100 mg pills. It dropped his blood pressure to a dangerous level. No he had not taken Nitro. He was ok in the end, but it could have led to a really dangerous situation. So take the meds as prescribed please.

Great video! My doctor prescribed 5 MG of tadalafil per day to combat my BPH problems. I've been on it for about 4 years and it has worked great! Much better urine flow and I usually only get up once a night to urinate as versus 6 to 9 times before I started the treatment.

This is really helpful (and normalizing). After radiation treatment for prostate cancer, although the clinicians I met provided excellent support and information ahead of time; still it was a wake-up call to gradually experience a loss of libido and functioning over a few years. As some of you might know - it's something that became normal at age 14-15 but now it is no longer the case. To put It bluntly, it's a bit shocking to have this be the 'new normal'. So, thanks for the info and the honest discussion. It helped me open a conversation with my doctor. So, my doc is aware and tuned in. And I am... better equipped.

I have been on Tadalifil for over one year. My prescription allows up to 4 5mg tablets daily! I am 78 years old with BPH and have had TURP surgery 2 years ago!
My response to this drug (taking 1 tablet twice daily) has been amazing. I wish I could speak to the sexual aspects with corroborated corroboration from my wife of 57 years but it takes two to tango! Nonetheless I do have significantly more blood flow to my nether regions as exhibited by mornings like I was 1/2 my age! Firm erections with two pills daily are very sufficient for that result!
But the biggest differences I have experienced are:
1) improvement in BPH (as indicated in the video)
2) more importantly my long term memory has definitely improved and
3) my balance is as good as it was 30+ years ago
4) during cardio workouts I have much better blood flow to heart and lungs (as a result of this and loss of body weight I have been taken of metropolol for the 1st time since my quintuple bypass surgery 21 years ago (my LAD was 100% blocked);

I'm 64 and don't have any underlying health problems at all except a slight ED. Don't take any medication apart from the below. I take 50 mg of sildenafil and the effect last for 12-15 hours for me. The only side effect I get is a little flushing. Being slightly over weight I don't mind.

Im 63 , I have BPH and OAB .So life changes , I am caffeine and alcohol free . This helps OAB . I was on finistrade to shrink my prostrate for about three years . But it affected me long term , now I take 5mg tadafil, 1mg toltradene and tamulsin . I have sleep 7 hours no night peeing . I have asthma and CPAP as well . Fortunately I am in the UK so drug costs don’t affect me .. also get an annual psa blood check

DON"T be EMBARRASSED about anything you need to talk to your female doctor about. She has seen and heard it all! I am a straight, heterosexual man married to the my straight, heterosexual female wife for over 45 years after pursuing her for five years. I will be 72 years old in a few days. When I moved to to my current location five years ago I looked specifically for female doctors in the areas I need. Why? Based on my past history women doctors seem to be more open to answering questions I have than male doctors. And there is NOTHING SPECIAL about my body or any condition that they haven't seen before in their practice, i.e., other male patients. I have talked about several " intimate" items with my general/family physician, my dermatologist and my urologist. I have ED (and take 20 mg Tadalafil, effective for more than 36 hours for me and no side effects), I have urinary incontinence for which I take two medications which don't work 100% of the time so I wear a CVS Men,s Shield (cheaper than Depends and works just as well) every day. Most of the time I dispose it unused, but then there are those other days. I am obese, have varicose veins, have sebaceous adenoma all over my body, and do not look as handsome as I was when I got married (most of time *I* don't like to look at my naked body in the mirror after a shower). So I have a female general/family physician doctor, a female kidney doctor, a female gastroenterologist, a female ophthalmologist, and a female urologist although she is a PA-C which is a Physician Assistant with certification from a national organization and she works under a male M.D. urologist but I only saw him once. There is no female dermatologist where I live so I have a male dermatologist and my female dentist is retiring real soon so I will be changing to a highly recommended (by her and an orthodontist I know) male dentist. Again: DON"T be EMBARRASSED about anything you need to talk to your female doctor about. She has seen and heard it all!

Was prescribed tadalafil after IRE (the “NanoKnife”) treatment of Gleason 7 score for prostate cancer. I’m 90 days out now…no incontinence or ED…and found that the medication improved my urinary function markedly. For a 74 year old guy, that’s a nice win. My wife, of 52 years of marriage, probably finds my attentions rather annoying by now. Interesting problem to have, suddenly.

I take half a 20mg Cialis twice a week. I find my mental clarity improves as well as urinating is more comfortable and freeflowing. I sleep more easily and have better exercise tolerance. Also hanging more on the slack makes me feel more confident.
Cialis is a lipophilic substance so absorbs better with fatty food to stimulate bile secretion.
Tadalafil lasts 36 hours - thing is after 4 hours, she's sore and the cat's nervous...

Both Viagra and Cialis are PDE5 inhibitors, an enzyme that reduces NO stimulation of blood vessels to expand.

I was prescribed 5mg/day TADAlafil (you see what I did there) and told to give it a week to start working. On day 5, I woke up and was damn near scared to death at the thing staring at me. I don't recall having erections like that as a teenager, and I have not seen such pronounced veins on anyone other than the arms of athletes. I went from just growing to near erect size when aroused and probably unable to push it through the center of a glazed donut to sporting a chubby that could win an arm wrestling match. Oh, the joy of modern medicine!

I am a physicist who analyzed data professionally for 40 years. I have also done the statistics portion of a Health Data Science course at the Masters level. Rena is the second YoutTube doctor who expresses a bias towards Meta-Analysis, or combining multiple studies. I have mis-givings about this and I would recommend health professionals treat meta studies with caution and read them very carefully.
1. In theory increasing the number of samples is good. However, this is only valid if the sample populations are very close to identical. Combining studies, this is highly unlikely. So is not a strong argument in favor of meta-studies.
2. Review studies may put good and bad studies on an equal footing. This is very bad, especially as Drug company papers tend to be heavily biased in favor of the drug under study. A possible tactic would be to flood the literature with favorable, but poorly done studies in the hope that the bulk of evidence in a meta study will still favor the drugs.
As an example, many well done meta-analysis in the last five years have found Statins have a number-need-to-treat (NTT) between 42 and 500. This basically means they have no benefit, yet statins are one of the most prescribed drugs and the most profitable drugs in history.
This is my professional opinion as an expert in statistics and data analysis.
I am only 40 and started to have ED at around 38. Urologist got me on both but i am still struggling- ugh. I am on high blood pressure med and i think thats whats started the whole thing.
At 1:42 you explain that once the penis ejaculates, the nervous system revs up causing the smooth muscles contract, forcing the blood out of the penis. However, the immediate loss of erection after ejaculation does not apply to all males. Some males are able to have multiple orgasms (>5) without losing their erection and having to stop coitus. In fact for some males, having 3 orgasms without stopping coitus is not an issue. In addition, their recovery time after having multiple orgasms and losing their erection can be as little as 3-5 mins (although each time they restart, the number of multiple orgasms drops). Of course, as these men age, the number of multiple orgasms they can achieve seems to begin to reduce in their mid 30's. I should also note, physical effort during sex has a strong effect in the number of multiple orgasms said males can achieve; i.e. if the partner is on top doing most of the work, the multi-orgasmic males can easily have many more orgasms, in contrast if they are excerting a lot of effort, like lifting their partner during coitus, then orgasms dramatically drop (even to just one orgasm, if the partner is providing no "lifting" support).
For ED in men, try L-Arginine that is a natural amino acid before getting on The Sildenfil drug. Only a suggestion.

It all fell apart for me over the last couple of years. Now Im 50, over weight about 30-40 lbs, high BP, high Cholesterol, started TRT over 1 yr ago, have ED use Taladafil at times. I've tried both recreationally. Viagra doesn't do much for me. Cialis works great. Side effects are harder with Viagra. Headache and backache. Viagra seems to give you one big bang. Cialis seems to give you staying power multiple times for a couple hours.

My doctor prescribed me sildenafil 50mg. I split it in half and took 25mg. Worked like a charm. I don’t have psychical ED but probably anxiety/performance is causing it. I asked my doctor what are the chances of priapism and he said 2% but I don’t think he really knew and gave out an answer. Is it really 2% that high?

I'm 75 yrs old, 'n have been taking Tadalafil 20mg tabs daily for 2 yrs now, it's a great 2-fold med, as I'd developed BPH when I was in my 40s, so they gave me an Alpha blocker for the urinary problem, but it's side effect was ED. Now that I've been on Tadalfil NO MORE PROBLEMS, as my sex-life improved dramatically. My last 2 GF's, one in her early 50s, 'n the other in her late 30s, said I was like a young man in bed. I also take an L-Arginine supplement cap every day to replenish Nitric Oxide.
I first took Tadalafil at age forty and had side-effects of acid reflux, ankle swelling, and a small degree of high frequency hearing loss, all temporary. Returning to it over ten years later and I don't seem to get any side-effects at all. One additional piece of info: Tadalafil has a half-life of seventeen hours, so after the thirty-six hours of supposed effectiveness, you roughly have a quarter dose left in your body. I find a full 20mg dose effective for three days and most often just take half a tablet. The only thing it can't help with is completely spontaneous sex with an ongoing partner, which is a shame but I don't see any way around that.
Use the waiting time for foreplay.

@@hukssumi Short answer is no. It used to be prescribed as a daily dose of 5mg for pulmonary hypertension but is not used for that purpose these days. As an ED med it's prescribed at a higher dose (10mg+), to be taken as needed and a dose is effective for two to three days.

I do have a question though, I take Tadalafil usually 10 milligrams every two days. I also take Tamsulosin HCL for Prostatitis and I know it can effect my erections and ejaculation. But do I need to continue the Tamsulosin or would the Tadalafil ease the prostatitis? Any information would be very appreciated Rena. Thank you!

I'm a senior with reasonably controlled Diabetes and diminishing control over many years of having high blood pressure. I've been using the ED meds off and on as long as they've been available going back to before I had Diabetes and the hypertension was better controlled. My personal experience along with the experience of most other men I've discussed this with, is that Sildenafil is much more effective for ED. I'm surprised that most comments I've read here report opposite results.
When I used Tadalafil for a couple of years I found it helpful, but it provided a significantly less vigorous erectile response than Sildenafil, and I never had much confidence in it on the 2nd day - and the extended period of effectiveness is really the entire point of using it (aside from the BHP issue Dr. Malik discussed). There were times when I felt the need to take 50mgs of Sildenafil to supplement the 36 hour Tadalafil I had taken the previous day when I anticipated sexual activity on that 2nd day. I am wondering if there is any significant risk to combining the 2 as I did during the last 12 hours of the 36 hour Tadalafil window? Btw, I never had any side effects from any ED medication other than some nasal congestion on some occasions, including the occasions when I combined the 2 meds.

Thank you for this info. I found as I approached my 60's my erections were not at full capacity all the time. I am a retired athlete and this bothered me. I experimented with low doses of L-Arginine and that did the trick. At 1 to 2 hours before intimacy (NO alcohol) my erections were as strong as ever. After a couple of years I discovered I was born with bicuspid heart valves (now corrected), that was a contributing factor according to my surgeon. I had a major spinal injury in 2001 after which I spent years in recovery for numerous conditions in addition to the burst fractures of the vertebrae. One of these was ED and I was prescribed both these medications and both had really bad side effects for me (Viagra was the worst), migraines and severe lower back pain. I was already suffering chronic pain in the lower back but these meds just made it so much worse. I had to stop taking them.
I have torn vertebrae in my L1&2.I have ED so I want to be careful. I just turned 70 yesterday.

I am now 62 years old and have had a 'fwb' with my sweet neighbor, a widow who is now 80 years old, for 15 years, according to mutual agreement. 2 years ago I had some ED problems and first tried sildenafil, then tadanafil (daily). This last one actually suits us better because we can have sex whenever we want, which is quite often. What an invention!

I didn't erectle dysfunction, but I ordered some cialis because I wasn't really physically attracted to my wife and just needed a little boost. Unfortunately, to this day, I swear that it caused me to randomly get low back pain that took a good week to go away. I just woke up in the middle of the night in incredible pain. Still, if I ever needed to use it again, I would opt for cialis.
Oh yea. Those muscle aches are no joke. It's like skipping excercise for 6 months and randomly running across the state. That's how sore it makes the muscles.

There are all-natural ways to treat ed issues. One is genital massages called Karsai Nei Tsang. The other one is penis exercises!
I have some advice. If you have never had Cialis (Tadalafil), don't bombard your body with 10mg-20mg of Cialis. Take as low as 2-3mg daily. After taking it for a few week or until you feel some side effect, stop taking it for a week to let the side effect subside. Then come back on the routine of 2-3mg daily. Repeat the cycling on/off for 2-3 times, until you don't feel any side effect for a whole week. Then you are pretty much safe to take is long term. For daily intake, it is recommended to keep the dosage under 5mg. By the way ,there is one VERY IMPORTANT thing, DO NOT eat GRAPEFRUITS, since it would prolong the half life of the medication and enhance the side effects. It would be much better if you cut or reduce orange and lemon, too. Lime is OK.
The best review I ever heard
Cialis daily made my condition worse long term. Got to the point where I was completely non-functional. Took me over 6 months of not taking it to recover. Still not great but I'm not going back on that again. You also forgot one side effect (at least of Cialis): it makes sexual activity not feel good. Miserable even.
Developed hypertension several years AFTER I started having ED, so that didn't cause it. Nobody ever did any investigation into the cause; just repeated a memorized explanation that sounded like a textbook. Just assumed hypertension figured in, ignoring that it post-dated the problem. And I've been to a dozen doctors in 4 different states over about 15 years. NOW they say, "well, you're in your late 50s ..." but they don't listen when I say it started in my early 40s!
My sense is they're all flying blind and repeating theoretical stuff they read, then writing prescriptions. For me, I think I need to know what's causing it because the assumptions they're making have no basis in my particular case. But I've pretty much given up by now.

@@bgoodorhell4u Way past that. One of the first things I did 15 years ago. It was somewhat low, and so was vitamin D. 6 months of supplements and it was a bit below the center of the normal range.
Also, testosterone affects libido but not ability.

I think you forgot to mention that regardless of which you take, you still have to have desire to trigger the outcome. If you are with some one you don't have a desire for then these meds may not help.
