If you use a cipher on a secret console, you won't get the apex version of the tank, you must complete both secret consoles manually, but can use ciphers on marked ones

Also if I am correct I think you can use Cipher with the consoles or is that limited to not SP. And while it's RNG the autobreach parazon mod do work, it worked only once to me but it did work. Nova is only used to skip past the gates. This is the Apex version of the tank, to spawn it you need to hack 2 unmarked terminals, which can not be ciphered, can not be autobreached and can not be hacked with perspiacity.

Worm hole allows for the fast travel + parkour velocity mod. Invisibility comes from a parazon mod after hacking. Armor strip comes from latron incarnon with puncture procs. Not sure if I missed anything.
Something has to be bugged in my game I don't see how you are doing this. My incarnon are not charing, my hammer will not get any combo in the fight I bash the ground hitting groups of enemies and I'm never at x6 to activate. After 20 mins I got a healt bar gone and he heals it right back up, the enemies don't die there are pylons shooting missiles. I been doing this day after day week after week and nothing I'm ready to break everything. How are you guys just jumping and smashing him and he dies? Wtf

Organ Shatter. But sadly a semi decent riven is kinda required if you want to consistently one shot. Still does a lot of damage without, just can't guarantee it to be a oneshot from my expirience.

Nova's portals are known for getting into unreachable places.
Notice in this vid the portal's beam is aimed through the crack in the door.
