In India we Have Meropenem Sulbactum combination at the place of Varbobactum. How much effective this combination Meropenem Sulbactum is? same is with Cephalosporins
@appdoctor good question - any advantage of mero- sulb comb. sulbactam is a beta lactamase which confers protection against certain beta lactamases ( ambler class A ) , it does not protect against carbapenemases , however at times certain beta lactamases can cause resistance against mero and here the combination will be helpful. however this is not common as the main resistance comes from carbapenemases and mero- sulb does not protect from that. to the best of my knowledge there is no data published comparing the two. Varbobactum is on the other hand active against carbapenemases ( mainly kpc and some oxa which are common in the west) and is fda approved. its spectrum is like cetazidime -avi. for india cef- avi with aztreonam is the most potent antibiotic for cre , however it should be used as a last resort to prevent resistance.
