In regards to 18:30, if you're curious, most likely the reason it bugged out is you managed to get aligned off to the left of the note. Specifically, Mario 3 defines "junctions" (whether by pipe, note block, doorway, etc.) by the "screen." A "screen" is 16 columns worth of tiles (256 pixels wide, i.e. the horizontal resolution of the NES) in a "non-vertical" level (where "vertical" think Pipe Maze.)
On a "non-vertical" level, every 16 columns (height irrelevant) has one junction point defined. What "screen" the player is on is determined by their horizontal position (as you'd imagine.) So, most likely, this note block was placed on the fringe of a "screen" (the leftmost edge) and you managed to slip just out of the "screen" by bouncing off the left side.
At this point, an improper junction was used, probably not defined by the level, and you just sort of ended up in the void there.
