This summary suggests that Malcador interviewed traitor individuals as potential Knights-Errant candidates. That is not accurate. Malcador drew individuals from traitor legions, but never traitor individuals. That's an extremely important distinction.

From what I understand, human souls are destined to end up in the warp after death, where they are quickly gobbled up by starved demons. The general belief for imperial citizens within the setting is that the Emperor protects their souls, but I've never read a first hand source from GW which suggests that this is the case unfortunately.
Despite how it seems, exterminatus of a whole planet's population is a relatively rare occurance. Especially somewhere where you need experienced and capable defences like Cadia. Imagine having to repopulate a whole planet of 850 million people, teach them how everything on the planet works, train them in fighting techniques and get them ready for the next incursion...
Fair question, I'm not sure that I've read a good account on why they haven't taken any action, but perhaps they saw Cadia as such a necessary defensive position that they chose to not respond? One of the other replies to you from EthanKristopherHartley is an accurate answer though, they wouldn't be throwing an exterminatus around willy nilly, and so the value of having a fully trained, and fully manned planet is certainly worth far more than a barren rock.
