When I was a kid and my father took me to record stores, the cover of Funkadelic’s Maggot Brain used to terrify me. That woman, stuck in the ground up to her neck, screaming, I found it so unsettling. Even worse, the backside showed a skull instead. Now I have a t-shirt featuring that cover. I call that conquering your fears.
Def Leppard - Retroactive - Cover depicts a woman sitting at her dresser table with a mirror but if you look at the image slightly out of focus, you can clearly see the skull formation.
When i was five years old i didn't really have a clue who 'Iron Maiden' were but i know that seeing the album covers to their debut album and 'Killers' in the local supermarket scared me to death, it was the banal backgrounds with streetlights,houses and everyday things with the terrifying undead psychopath 'Eddie' front and centre, especially that bloody axe!!
I'm throwing in a vote for Brujeria's Matando Güeros. 90s death metal/grindcore that featured a genuine severed head on the cover, which subsequently led to the album being banned in many places. An original vinyl (Roadrunner) is now around $150.

I think the "Seventh Son of a Seventh Son" album is pretty creepy, with the icy/watery background, the disintegrating floating Eddie with his head on fire, holding his "heart" which also looks like something trying to burst out of it, the floating lights, and even at the back with a ghostly Eddie from "Number Of The Beast".

TNOTB is easily the scariest and most unsettling album art I've ever seen in my life. That album also has the song "Hallowed Be Thy Name", which is quite possibly the most disgusting and dreaded chart in the game "Beatstar". That song will haunt me as long as I live.
I have always had an affinity to the art work of Iron Maiden. At the time it was ground breaking and you wondered what was next for eddie. I must confess I never realised there were hands on eddie where he had killed someone.The creepeist for me is " Piece of Mind" coz it just looks like eddie had lost his mind in an asylum. The UK Black Sabbath Greatest Hits from 1977. Featuring the classic painting The Triumph Of The Dead. Which is 462 years old. Kinda ties in with the first album when I was a kid and thinking it was from 1871. But that's just the catalog number. And to this day, I still believe this album could be from 1871, just because of how dark it is. Definitely one of the first albums i remember as a kid. Which I guess is different seeing an album cover back then, because it was so huge compared to 3 year old me. Or maybe I was 2. Either way, scary shit. Kinda like News Of The World by Queen. That skit from Family Guy makes me laugh.

I’ve always loved the Black Sabbath debut cover. While most of the others on the list fall under the slash/shock/schlock/almost comic book horror for me, the Sabbath cover is truly haunting and creepy without being over the top. I could stare at it for hours.
Honestly Cannibal Corpse's Butchered at Birth and The Wretched Spawn (uncensored cover) are absolutely terrifying. Their album cover arts I have always LOVED though. The brutality of the cover arts is actually what drew attention to me when I was younger. Love Cannibal Corpse. Honorable mentions I'd say is Cattle Decapitation's Monolith of Inhumanity and The Anthropocene Extinction, both EXCELLENT albums as well.
Venom had a pentagram with a face inside on their debut akbum in 1981. The Motley Crue album was possibly inspiref by it as their album you showed came out in 1983. In The Court Of The Crimson King by King Crimson from 1969 is very iconic and I feel quite disturbing for some people. Barry Godber a friend of Peter Sinfield the Lyricist painted the cover based kn his own face in a mirror. Barry had not heard any of the music and the band thought the art captured ghe essence of their music especially 21st Century Schizoid Man. Robert Fripp saw the painting at the record label. It had bleached due to the sun. He now owns it at his home. God knows what people thought in 1969 looking at a display of the cover in the record shop window.
@@Channel33RPM Yea Ian Mcdonald was a great talent on this album. He was also a member of Foreigner for their first two albums. He did help with the arrangements kn the songs on the first album. You can tell by the vocal arrangements. Sadly Mick Jones didn't allow Ian to have his name on the credits.

I'm not easily creeped out by anything, but a couple of albums by Rammstein have some seriously odd imagery: 'Mutter' has a preserved fœtus on the front, and images within of the band as if they too, have been pickled in formaldehyde. 'Sehnsucht' has all the band members looking like corpses, some with sharp implements on their faces. The most disturbing, though - and it was packed in a black slipcase to hide the images, is 'Liebe Ist Für Alle Da' which show the band in period costume, with some partially clad young women. The pictures imply that these women are killed and possibly eaten. It's a great album, but man, those graphics are uncomfortable to think about. Yes, it's Rammstein, and not fluffy bunny territory, but... 😆😆😆

I think last year you had a guy on from Australia that was on your vinyl dens and he has his own TH-cam channel called explosive action and he does metal album reviews and he always make sure to talk about the cover of it because the cover is what catches your eye first.
Those were all great choices. Even the Eagles. Sabbath has had some of the worst covers around, but they nailed the creepy factor on that first cover. It almost makes up for some of the lame ones that followed.

It's been a while Frank. I will say there is one CD I won't even mention. Simply because the cover art is too terrible for words. Yea. I can go along with you on METALLICA's "Kill Em All" (1983). 📀 "Hell Awaits" (1985) from SLAYER 📀 is a CD I choose not to listen to, at least at this point in time. After being in a coma for a while and having a near death experience, I just don't have the desire to hear it. Strangely enough though, "Tempo of the Damned" (2004) from EXODUS don't seem to upset me.
