If the caller computer is blank then the user stored the password in mobile devices /non-windows device. Ask the user to clear the password from mobile devices that he used. If the mobile devices moved to some other hand in your organisation track the phone and clear the stale password. If the user stored the password in application then caller computer will be blank. User knows the application name for clearing the stale password. Hope my suggestion helps to address the issue. Thanks for the post.

Please connect PDC emulator doma controller look for 4740 event id read the logs related your account. You can see the caller computer in the particular event. If caller computer is blank then the user request comes from non windows device. We need to check the non windows and clean the stale password.
As per my understanding security event log is not capable of capturing application details which causing frequent account lockout. In my experience Quest change auditor is capable of doing that. However it is a licensed tool. End user must aware of application names which they are using for business purpose, otherwise end user manger or colleagues knows application name.

Some case's CA display the DC name it means authentication request received by dc from non windows device and web url. We have to ask the user when and where he entered credentials. Based on user response we can address the AL issues.

End user never enter the credentials in domain controllers they don't know about DC details.so we can't blame user's. The DC which validates the users credentials,in case if it wrong then the dc lock the account. That's is the reason you are seeing caller computers as domain controller.

Service account is nothing but a normal Active directory domain account you can follow the same steps for (which I mentioned in the video) for removing the stale passwords. Prepare a spread sheet and document the application name with service account. Whenever you’re changing the password for your service account as application admin you might aware how to update the new password on your application. If your organization compliance permits, follow the below method for avoiding account lockout and password change for service accounts. 1.) Use “password never expires” option for your service accounts so that your service account password does not expires at any circumstance. 2.) Talk to your AD administration for configuring (FGPP)Fine grained password policy for service accounts. There is option available in FGPP to get exclusion from account lockout issue for your service accounts.
