As you are talking about muscle tears: if you deal with deltoid and shoulder muscles in your practices, please consider making a video on when to see an orthopedic specialist if you're experiencing pain in the deltoid muscle. Thanks for all the very helpful videos. *thumbs up*
I have torn ACL as a result of assault. I was "dropped in a wrestling move" thrown across the room. Injury did not show immediately - only excruciating pain. A day later my knee had blown up to the size of a bowling ball. I had to wait a week to see the surgeon. I was in a leg brace for two years. Many MRIs to check on it. Tried to get surgery but I'm not a prof athlete so was dismissed. Since, bi-malleolar fracture with plate in ankle, ORIF on patella and soon hip replacement on that same leg. The ACL injury set off a series of further injuries. Now with arthritis - my beautiful legs are not the same as before.
I am 65 years old and had a complete tear of my ACL on my right knee .The doctor advised Physical therapy over surgery. In 2011 I had a complete tear of my left knee with no surgery. I don't run or do sports ,except I am a avid road cyclist. What do you think please?
Hi Greg. Good question. As we age, we usually develop some degree of arthritis in our knees which makes recovery from and success with ACL reconstruction more unpredictable. We would typically advise our middle aged patients like yourself that do not do a lot of plant and twist type activities to try and avoid surgery and work with physiotherapy. If you do find that instability is an issue you can always consider a brace which can be very effective. Hope that helps.
With an unreconstructed complete ACL tear, orthopedist advised no reconstruction (did do arthroscopic cleanup) because of damage already in done, and mostly torn and removed meniscus, would stem cell treatments to attempt to rebuild cartilage be worthwhile? Or is that just a futile attempt to hold off a knee replacement?
Hi mudbob1. At this point, the scientific evidence does not support the thought that injecting stem cells into an arthritic knee will regenerate or rebuild articular cartilage. Our best advice is to stay fit and lean for as long as you can controlling the pain non operatively and get a total knee replacement when your quality of life has deteriorated enough that it has become unacceptable to you. I have a high grade tear of my acl ( tennis). I am not having surgery. Can I swim front crawl for exercise with kicking? I have tried it and it makes the back of my knee a bit sore. I am one year post injury, have done pt, have a trainer for strength and conditioning and have a donjoy brace so I can continue my tennis. I want to swim. Would it be better to swim without kicking? I am 59 years old and have been very active my whole life.
How likely is it that i would injure it further by just lightly kicking up and down: low, medium or highly likely. I cannot use my Donjoy in the water. Perhaps i just have to get used to some low to medium soreness for the rest of my life?

I have alot of pain after work and it gets very stiff .and I find it hard to clime up stairs. Last sugrey was 15 years ago . When should I expect to get a total knee replacement ? Or is their something I need to look at ? Or is this arthritis ? What happened during football and I was just shot up with cortisone and some pain medication.
Hi Brian. So ACL injuries can lead to arthritis over time for sure. The best way to assess this would be with a weight bearing xray and that could help determine where you are on the spectrum. If your knee has been stiff for 15 years it would be hard to get it moving with physio at this point. Surgery can likely help this somewhat. Best of luck to you and thanks for sharing your story. Hi doc ...I 4 weeks ago tore my ACL ..m little overweight My MRI report says-. Mild degree partial tear of ACL with minimal joint effusion ( I'll defined increased signal intensity is detected along posterior fibres of mid/distal substance of ACL. Doctor here adviced me bracing and rest for 1.5 months and medications.. I m using hinged brace from past 8 days . I have little laxity in my knee when I try to flex my knee by bending forward. Minimal swelling . Pain was severe on 2nd and 3rd day only..after wards it subside. Now very minimal pain (probably centre/anteromedial side of patella) But my knees has definitely became weak. Have not tried climbing yet ..only walking . Do I need surgery for this?..will my knee never b same again as it was before?....it's mild partial tear ..so will it heal on it's own? ..am 24 years old .. And advice and help and suggestions would b appreciated doctors ...thanku so much

I re-tore my acl after falling. First doctor said I should get a surgery but I postponed it and decided to do p.t. Then I went back when I was available for a knee surgery, was assigned to another doctor that I had seen for my first surgery. He having only aquick time to see me said ACL surgery is not for reduction of pain as pain would go away but he said instability is a reason for surgery then told me to do p.t. Since then at 31 I have arthritis like symptoms and pain daily especially after doing stairs and occasional instability. Is the view that pain is not a reason for surgery a correct one?
Hi Kayla. So typically ACL reconstruction is performed for instability. The pain that often results from these injuries will typically subside unless it is due to a torn meniscus or arthritis and these require different treatments than ACL reconstruction. The arthritis can develop over time due to the instability. Hope this helps
Hi Ramiz. Without an MRI, you would have to base your decision on clinical assessment and other investigations. A complete ACL tear is often easy to diagnose with a physical examination. Diagnostic arthroscopy is not ideal but also another way to identify the problem.
I’ve recently torn my ACL for the third time. My doctor has recommended doing “bone injections” to strengthen the tunnels before the next reconstruction. And allow two months before the ACL is fixed. Does this sound correct? Thanks Alex-
Hi alex. It is possible that the bone quality has been compromised in the bone tunnels and they have slowly become bigger with your two prior procedures. Having treatment to increase your bone strength is not an unreasonable option. I suspect your surgeon is simply trying to increase the chances of success of your third procedure. Wow! People, go swim. Most of ur aches and pains will disappear. Has worked for me. My daughter too. Do swimonly with Dr's advice. Water therapy is also a good way to start. My problems have mostly become much better, from nit being able to hardly wLk to going up and down 48 steps daily.

My doc suspects I damaged both my meniscus and my ACL.
I go on the 25th for an MRI, and back to Ortho on the 1st.

i got this injury when i slipped, and hit my knee (in awkward angle) on the edge of a step...now it's painful to do morning runs and cycling...
