I forgot my memory cards back when you couldn't get them except at specialty stores. That resulted in a canceled shoot--we scouted locations,, but couldn't actually shoot. I've also forgotten to bring the spare batteries for the flashes back when AA batteries were the norm, and my units took 6 each. On location, a couple of hours from nowhere (Death Valley). That didn't go well either. :(
Oh, and forgot the 'household' style sync cables for my (antique) strobes. That session was salvaged by some tiny speedlights.
I have a checklist now, and also keep a set of older cards in the camera bag Just In Case. So far, haven't forgotten camera batteries. (Jinx!)

I've made all of these mistakes, except for some reason, leaving the battery. I've left SD cards though and definitely have made the double booking studio mistake (was in the middle of shooting a boudoir set when the other team walked in.....yikes). I think my worst mistake was about 2 years ago. I was shooting a model but i took on an assistant and was teaching them my process. Well, i was shooting tethered into LR, and the assistant was a total beginner when it comes to shooting off camera flash. So, i did my first "black frame" shot to explain to them how to make sure your ambient is killed. I moved the exposure slider up 4 stops to show how far away we were from light pollution. Well, apparently i had the setting on to transfer settings from previous and didn't notice i had left the exposure slider up 4 stops. Finished the entire shoot before i realized what i had actually done was shoot all the images 4 stops underexposed and had a bunch of noise in all the images. That was a terrible mistake. Ugh.

I've left my entire camera behind before, the shoot was 40 mins away. I had 2 shoots on the day and photo dumped in between, grabbed my bag and left for the next shoot leaving my camera on the desk (big facepalm moment) thankfully I got away with using my phone and told the client I would shoot it for free when I arrived. The video actually turned out really good haha.

Oh my, if those happened to me I would surely flip! Luckily up until now, I have learned to prepare in advance in terms of gear and the people I work with. However... my one biggest mistake is to not scout / visit the location first that I will be shooting at. I do mostly outdoor shoots and while it may not be a big deal for many people, I like things to be planned out. This is because some locations here are rented on an hourly basis. So having a well planned set is a biggie for me . The clients love it too when the shoot is smooth and seamless as well as save a few bucks on renting the place.

Such a good tip Donnie! Planning the location is also so important, especially if like you said, it's a timed/paid location!
Plus location scouting can be super fun 😊

I forgot a battery ONCE. Didn’t matter though because the model cancelled last minute 😂
