There should be four little tabs on the end of the spindle were the blade goes, you have to make sure the blade is seated on those for the position you want it. Blades and accessory's can only be mounted at angles up to 90° left or right of straight ahead. Then add the flange washer and the bolt in and tighten it down. If it's still coming loose after that then there might be something wrong with your threads on the bolt or in the bolt hole. You might want to take it back for a replacement.
Can't keep the blade on mine no matter how much I tighten. Any ideas?

I tried to cut a toilet bolt with it and it just didn’t do squat. Apparently I need titanium blades.
I imagine this blade would be okay for wood products. To cut a metal bolt I would say you'd probably have to buy a more expensive one.
