i have a doubt so if in long face internal rotation was in backward direction and u said internal and external rotation are always opposite so will the external rotaion in this cases be in forward direction.?or if the inernal and the xternal rotation both are in backward rotaion in high angle cases? and how do we measure the total rotation here do we add both internal and external or we substract the external from internal rotation? please help.
Yes you are absolutely right. If internal rotation is clockwise then external rotation tries to compensate and its forward. In growing age it compensates.
But there's always variation. If internal and external both are down and back. It will result in severe kind of long face. Total rotation is always measured by angle between lower border of mandible and cranial base like SNMP. In this type of case when both are clockwise then angle between internal and extremal rotation might be zero depending on the magnitude of both rotations

Matrix part of internal counter clockwise rotation is possible with any appliance/ surgery which moves posterior teeth apically (intrusion). Posterior bite plane, implant supported, maxillary impaction etc
sir during growth development what cause the resorption in one side and apposition in the opposite side ? ( surface apposition and resorption )
There's some role of genes. other fact is that Bone responds to external pressures. Each bone is lined with periosteum outside and endosteum inside both are rich in cells. When bone is under the force of surrounding soft tissue, this layer is pressed and bone is reabsorbed. When there's stretch then osteoblasts are activated and bone is deposited. This is a general rule. Rest can be understood when regional growth is dealt separately.

1. Can the act of mewing (pushing the tongue onto the maxilla throughout the whole palate) reverse the downward growth of maxilla (in 10:35) and hence causing the mandibular to rotate ccw. Also, if it's possilbe, how long do you think it takes to reverse the effects?
2. Is there any way to tackle gummy smile?
3. Does palatal expanders also cause the expansion of the mandibular? And what are the other no surgical ways for mandibular expansion?
Unfortunately once growth has occurred then it can't be reversed.
Treatment of gummy smile depends on the severity and the cause. Microimplants sometimes do miracles.
In Mandible dental expansion is the only possibility
Mewing can reverse some downward growth bro but it takes a while, best bet for us is surgery

hello sir i have a theory where proclining the upper incisors and inclining the lower incisors may help the mandible slide forward and chin to appear more prominent..is this reasonable?
Nice thought. In growing age it's important that mandible doesn't get locked due to any malocclusion. Class 2 div 2 may lock the mandible so your theory is correct in that case. But in class II div 1 lower Incisors are sometimes proclined and doesn't allow mandible to grow
does downward growth have any effect on the cheekbones as well?
Over all long face. Not directly related to cheek bones

Yes. (Differential) impaction of maxilla and auto rotation of mandible is possible

Yes. In severe cases we need surgical impaction of maxilla.
Sometimes intrusion of molars give autorotation. This can rotate mandible counter clockwise

@@irfansinstituteofclinicalortho yes, I was thinking about if your maxilla grow downward your mandible also grow downward. Downward growth maxilla=long midface, nose, low zygomatic bones and negative orbital vector. I formed my question wrong. I should Ask if its possible to move maxilla upward to shorten it. If maxilla is shortened mandible will rotate and follow it, also it will fix zygomatic bones, shorten nose and midface and fix under eye support.
Edit: if such surgey exist IT would so beneficial. Typical ccw rotation surgery only rotate lower maxilla(under nose) and rotate mandible body, but what will ramus and upper maxilla?
@@Hhjhfu247 now that totally depends on case. In most of the cases maxilla is affected at Lefort 1 level. Signs are gummy smile, open bite, overjet, rotated mandible etc. This can be treated with intrusion of posterior teeth without effecting other facial bones. But if other changes are needed then can be planned accordingly. Lefort 1 impaction can elevate nasal tip but it doesn't affect other facial bones much.
So what is a good option for people with long face syndrome
Depends on severity, age and causes. In growing age, control the mouth breathing habits. High pull headgear may work. Afterwards maxillary impaction in severe cases and intrusion in mild cases

@@irfansinstituteofclinicalortho bro plz reply me don't say in person is consultation is needed.so question my face length is 12 cm and male from eyebrow to chin and want to have 14 cm ie 2 cm long.so by doing CW rotation thinking to get 8 mm length on upper and 12 on lower so by lengthening face my nose will be short so does downgrafting automatically increase nose length or rhinoplasty ise need if If I m increase face by 2 cm so by nose should be about 7 to 8 mm length give me idea

Normal values are mentioned in videos. Apart from normality In my clinical experience, I have seen SNMP values from 10 to 50ish.
