Phenobarbitone is the first-line treatment recommended by the WHO, IAP & NNF because it can not only control seizures but also reduce the brain metabolic rate.
It has been proven to be able to control neonatal seizures caused by various etiologies

Ma'am after one year recently my baby faced seizures.. she has HIE. Today she is being discharged by hospital. But in MRI all are normal except one thing.. In report, they are saying that " Mild T2 & FLAIR hyperintense signal intensity noted in supratentorial posterior periventricular bilateral region.
And in impression they have mentioned that " Mild HIE/dys : mylenting changes in supratentorial posterior periventricular bilateral region"

      Consult paediatrician or paediatric neurologist
Before stopping, neurological assessment, developmental assessment should be done.
If no episode of convulsions occurred after neonatal period. EEG normal, neurological assessment normal, then slowly decreasing the dose over 15-30 days. Then if no episode of convulsions, pheno can be stopped

Subtle seizure can present as apnea in FTNB.
In seizure, always tachycardia initially. So if you find tachycardia with apnea in FTNB first think seizure. You have to rule out cause of this eg hypoglycemia, hypocalcaemia etc.

    • @learningpediatrics5347
      @learningpediatrics5347  2 ปีที่แล้ว

      Dear Zayna,
Eye to eye contact normally develop at 4 weeks of age. So it's delayed.
As 90% of Brain growth occur in first two years of the life, so most of the time, if any brain insult occur during birth or neonatal period, can become totally normal.
You should consult paediatrician or neonatologist for the proper neurological assessment and some tests eg MRI brain, ROP, EEG, BERA
So you will be able to know what will be the chances of complete neurological recovery.
      God will bless your baby.

Is Jitteriness normal in new born does it go away automatically

    • @learningpediatrics5347

Jitteriness usually benign in 70-80% newborns but in 20-30% NB, it can be pathological as secondary to metabolic disturbance such as hypoglycemia.

You are right. Generally convulsions not seen in hie grade 3. HIE grade can be changed with treatment. Whenever newborn is admitted with severe birth asphyxia and requiring all vital supports (eg. Ventilator care for respiratory failure). Diagnosis is made as SBA with HIE grade 3. With treatment HIE grade will be changed. But final diagnosis will not be changed.
