Hi Salim, what is your Point on the Barcelona criteria? They are a Little bit more sensitive but less spezific. Some Kind of interesting approach…
No ECG rule will likely ever identify all AMI in either LBBB...My biggest issue with Barcelona criteria is that for discordant leads it limits itself to any lead where the R or S is ≤6mm (This seems to exclude some ECGs and therefore would miss some OMIs). My two cents.
A quick question. LITFL shows the 3rd criteria is discordant STE >25% (ST/S Ration). Example A shown at 1:25 & 6:15 shows discordant STD that is at 40%.
Are we supposed to be including BOTH discordant elevation / depression to the 3rd criteria, or only discordant elevation?
It’s either or on the 3rd criteria…it is only proportionality >25% that matters…not depression or elevation
The last time I saw a conference about this topic from Dr Amal Mattu where he stated one of 2 criterias is what we need: Concordant STE >=1 mm in any lead or concordant STD >=1 in V1, V2 or V3

Does the Modified Sgarbossa also include excessive disconcordant ST depressions >25% of the R wave or just excessive disconcordant ST elevation > 25% of the S wave...

Sgarbossa Criteria:
1. Concordant ST elevation in any lead ≥ 1mm
2. Concordant ST depression in leads V1-3 ≥ 1mm
3. Excessive discordance ≥ 5 mm
Modified Sgarbossa Criteria:
1. Concordant ST elevation in any lead ≥ 1mm
2. Concordant ST depression in leads V1-3 ≥ 1mm
3. Excessive discordance ST:S Ratio ≤ - 0.25
Barcelona Criteria:
1. Concordant ST elevation in any lead ≥ 1mm
2. Concordant ST depression in any lead (NOT JUST LEADS V1 - V3)
3. Discordant ST deviation ≥ 1mm in any lead where the R or S is ≤ 6mm.
