It looks like the pilot of the helo was the proximate cause, but the real issue is the system permitted this to happen -- it was an accident waiting happen and almost guaranteed to happen sooner or later that it would!
The Helicopter flew above 200 feet! They are restricted from flying above 200 feet in the area they were in. Radar shows the helicopter reached as high as 350 feet. They knew the rules. The only question is why did they not adhere?
Same reason there's a 250 knot speed limit under 10,0000 feet that routinely gets ignored/busted. Same reason you do 70 in a 55 zone in your car. We all break rules, and pilots do too...even military pilots. What needs to be looked at closely is the safely culture of that unit and how often rules are bent and broken to complete the training/mission. From an aviation safety standpoint, the heli passing 0-100 feet below the jet is still a major "near miss" incident that will make the news. 500 feet is the standard for vertical separation in air traffic control. Anything less its going to be in a report somewhere. Bottom line: the two aircraft should have never been occupying that small patch of sky at the same time.

At the point of impact the CRJ, on path to the runway was at about 370 feet. The CRJ is 24.5 feet tall and the helo is 16.5 feet tall meaning that even if both crews were where they were supposed to be the separation was actually permitted, PERMITTED to be 129 feet. What is the expected deviation of height for each AC, it isn't zero as nothing is perfect and neither are the pilots, if we put that at, say, 50 feet then the procedure allows them to be not 129 feet apart but 29 feet. INSANITY!

It only takes a second or two for it to happen on accident, but, a commercial rated pilot is required to be able to hold altitude within 50 ft at all times, and you should be able to hold closer than that if you are experienced and talented, even hand flying. There is NO excuse to break a max altitude by 200 feet on a VFR corridor under a final approach path next to a major airport, while you KNOW there is an aircraft on short final.

Watching the video and hearing the ATC AND aircraft radio transmissions, it’s so obvious he didn’t see the plane and thought the aircraft stated by the ATC was the plane in front of the plane that was hit. They were the exact same make & model plane as well

That is a busy airway - - there were planes in front of and behind the one that was hit and taken down by the helicopter. There weren't shortage of air traffic communication - helicopter was told of approaching planes - asked if they had them in sight - helicopter said yes - and obviously was looking beyond immediately in front of view to other approaching planes - - THEY WERE IN THE WAY OF THE LANDING APPROACHING PLANE AND DID NOT SEE IT - - Horrible Tragic Result From Copter Error - Air Traffic was Guiding SEVERAL Planes All In A Row In Their Approaches - - That Copter Should Not Have Been In the Midst Of That and They Did Not See The Plane Coming - - They Were On Visual Responsibility and Failed - - Not RADAR / Air Traffic Responsibility at time of collision.
