This video also brings some viewpoints for discussion: From the practical perspective, the most challenging part is the "reaction plan". who is responsible to stop the production? who is responsible to adjust the process? who is responsible to contain? In reality, can an operator do these things? ... Meanwhile, if the product spec is used to control the "product characters", then is it better to intervene in the process by studying the process variabilities and using a control chart instead? In manufacturing plants, contain activities are ALWAYS complex because it's hard to know the impact scope, and the production resources and work orders are tight. A general statement of containing material leave too much space for execution which usually leads to insufficient containment actions.
I fully agree with your frame, just adding some complementary views based on my decades of manufacturing quality experience.

1/ For each measurement do you need to have overall instruction of how to measure? For example - with caliper 0-150 mm. And files showing proving that training was done by workers ?

Hello @blueberry11051, there is no definite answer as for who has to fill out the document, it depends how it is organized in your company and if it is not clearly defined maybe you could create a RASI matrix to define all related tasks.
In general, it is important to review it in a multi-disciplinary team and consider the FMEA. (Not one person alone can know all the possible failure modes and risks related to a process or product)
For an in-house production the lead could be with a quality planner (those who bring the new projects) and be reviewed by the user (those who receive the project). In an SQE supplier relationship, here the supplier creates the CP and the SQE reviews it and gives input.
