    Assessment criteria
    1 Assesses the safety of the scene and the privacy and dignity of the patient.
    2 Cleans hands with alcohol hand rub, or washes with soap and water and dries with paper
    towels, following World Health (WHO) guidelines.
    3 Introduces self to person.
    4 Checks identity (ID) with the person (the person’s name is essential, and either their date
    of birth or hospital number) verbally, against wristband (where appropriate) and
    documentation.
    5 Checks for allergies verbally and on wrist band (where appropriate).
    6 Gains consent and explains reason for the assessment.
    7 Uses a calm voice, speech is clear, body language is open, personal space is
    appropriate.
    8a Airway: Clear; no visual obstructions.
    8b Breathing: Respiratory rate; rhythm; depth; oxygen saturation level; respiratory noises
    (rattle wheeze, stridor, coughing); unequal air entry; visual signs of respiratory distress
    (use of accessory respiratory muscles, sweating, cyanosis, ‘see-saw’ breathing).
    8c Circulation: Heart rate; rhythm; strength; blood pressure; capillary refill; pallor and
    perfusion.
    8d Disability: conscious level using ACVPU (alert, confusion, voice, pain, unresponsive);
    presence of pain; urine output; blood glucose.
    8e Exposure: Takes and records temperature; asks for the presence of bleeds, rashes,
    injuries and/or bruises; obtains a medical history.
    9 Accurately measures and documents the patient’s vital signs and specific assessment
    tools.
    10 Calculates National Early Warning Score (NEWS) or Glasgow coma scale accurately.
    11 Accurately completes document: signs, adds date and time on assessment charts.
    12 Conducts a holistic assessment relevant to the patient’s scenario.
    13 Disposes of equipment appropriately - verbalisation accepted.
    14 Cleans hands with alcohol hand rub, or washes with soap and water and dries with paper
    towels, following WHO guidelines - verbalisation accepted.
    15 Acts professionally throughout the procedure in accordance with NMC (2018) ‘The Code:
    Professional standards of practice and behaviour for nurses, midwives and nursing
    associates’.

    Thanks for the sharing! It's informative and helpful. But is it really allowed to have hand hygiene with the gloves on? It's known that's not a common practice.

      due to cvd you have to wear ppe all throughout the station until the actor is gone . The assessor will you ask you to demonstrate how would you normally perform hand hygiene but with gloves on for that reason. good luck

      Hi Kabita, some test centres are no longer requiring wearing of PPEs on both assessment and implementation stations. But you still need to wear a mask.

    Just curious about the BP cuff. I was always taught to palpate the pulse on the ACF and align the arrow on the cuff to where the pulse is.

      you will do that for manual BP- you will use a stethoscope to auscultate for the korotkoff sound. During the OSCE you will use a machine.

      You only need to wear ppe for assessment and implementation because there will be actors . Planning and implementation no need.
      Pens and fob watch are provided during the exams. Good luck

      Either a NEWS chart or a gcs chart depending on the scenario. With the addition of MUST for malnutrition/diabetes scenario and PHQ-9 for depression. Good luck

    Hi Emer! Is this the Assessment format na 20 minutes for the new osce? Ok lang pala na gawin na agad ang A-E, it will save so much time. Yung mga nagturo kasi sa amin, ang tinuro is get your obs muna then write them down then balik na nanaman kay patient para explain yung A-E ng buo.

      Follow whatever works for you best but if you ask me doing the obs first then the rest is not an a to e assessment , you are basically doing the old exam .Good luck!

      Hi Jim, it will be integrated on the scenario and conversation that you will be having with your patient. On the brief, it says there that you might focus on one aspect of care more than the other. The A to E assessment is the physical part of it, you might encounter a patient who is afraid of dying which might initiate a conversation about spirituality . You also might also get the scenario with anxiety and you will be focusing more on the patient’s psychological and social needs. Hope that helps and good luck

    I checked some of the comments and is terrifying. Am I the only one who is concern about not checking the pulse, rhythm, lack of 12-lead ECG with ongoing chest pain!? To all watching, please escalate! Golden standard is 12-lead ECG!

      Hi Van! During the Assessment station, you only have 20 minutes to finish your A to E, compute the NEWS chart and ask for any concerns.
      The next station will be PLANNING, wherein they can write two problems/care plans for the patient and write everything that you have mentioned in your comment.
      Lastly, the nurse mentioned she would escalate the patient to the doctor.

    Hi, i wanna ask , i had seen some of OSCE student holds kind of book which they refers to check about medicine side-effects and all so do they provide that kind of book during OSCE exam?

      Hi during implementation station the test centre will provide a tablet/Ipad which contains an app similar to a bnf. If you are unfamiliar with any medication on the drug chart, you can always access it. Best of luck

      We do apologise for a not so well written script as we filmed the video impromptu and with very little time, but I wanted to commend our small team (Jamie and Coco) for making the video possible and for helping other international nurses❤️
      Yup no need to expose the patient, that is correct ( see the video again for more details). Most of the actors are sitting on the exam and with their legs already exposed. We just forgot to remove the blanket at the very beginning 🤦‍♂️. That's why.

    Thank you for the video, is this assessment only for for NEWS-2 chart? And does it mean that the new OSCE is only having NEWS-2 assessment, no PHQ-9, MUST or GCS? Because this is the only assessment format on marking criteria.

      Hi renu, how many times would you suggest?
      In the current marking criteria, hand hygiene is only mentioned twice. Of course, you need to follow the five moments of hand hygiene. Attend the webinars so you will find out. Best of luck ❤️

    hi sir emer, what if the patient is not diabetic do we still need to include asking blood sugar for the assessment? and also can we ask the pain before going to A to E assessment????

      Whether the patient has diabetes or not, checking the BM is always part of the AE assessment. It is better to ask for pain during Disability but of course if the patient complains about pain at the very beginning you have to intervene. Just verbalise your findings when you reach disability.     Your teching is good to some extent. However, you don’t apply sanitiser on gloves for hand washing . You either take it off and use your bear hands, tell the examiner why you have not use gloves . No direct contact with bodily fluids.

      Hi Matvick, as you can see, this video was uploaded at the height of the pandemic. Regulations for OSCE are to wear PPE whilst doing the assessment station and perform hand hygiene while wearing gloves - and candidates/nurses are instructed to assume that they are sanitising their hands without the gloves.     Your training is fantastic but you are misleading student by applying sanitizers to your
    hand gloves during hand washing. It has been confirmed at my university and even NICE guidelines.

      Hi Matvick. This video was filmed during the height of the pandemic. During that time, the international nurses taking the OSCE were instructed to wear PPEs to protect themselves, the assessor and the actor during the assessment station. If you look closely, the video also shows how it is correctly done, without gloves. PPE is no longer required during this station and is now just optional. I understand that you are a nursing student, and I hope this video helped you understand the A to E assessment ❤️
