To me, fast reaction levels are the hardest to make. Normally, a level is supposed to be designed so players generally know what they're getting into but a fast reaction level is the total opposite until the last moment. The problem is that you can't do much to test it yourself because you already know what's gonna happen.
So you just have to try to guess how much time it'll take for players to process all of the important things on screen. And then, if you watch other people failing to beat your level, it's like "Is it badly designed or just hard? How can you tell?"
