In a young woman, acute gynecological disease processes, such as ovarian torsion, hemorrhagic ovarian cyst, pelvic inflammatory disease, and ectopic pregnancy, should also be considered within the differential diagnosis. A stone in Rt. ureter and enlarged mesenteric lymph nodes should also be ruled out. But moreover, there may be two different conditions present simultaneously. Here in the case you are watching, a bilateral ovarian cyst is seen with Rt. ovary having a chocolate cyst. Even then the patient was examined for appendicitis and an inflamed appendix with multiple faecolith was found. So the examiner should be prepared to see all the possibilities so that no finding should be missed.
Other conditions that can mimic appendicitis include celiac disease Meckel's diverticulitis,, inflammatory diseases of the right upper abdomen (gallbladder disease, liver disease, or perforated duodenal ulcer), right-sided diverticulitis, ectopic pregnancy, and Crohn's disease.
Dunphy's sign of acute appendicitis. Dunphy's sign is a medical sign characterized by increased abdominal pain with coughing. It may be an indicator of appendicitis.
When appendicitis is misdiagnosed, the true culprit for the pain varies. In men, the cause may be an inflamed lymph node or a viral infection of the intestinal tract. In women, the problem may be an infection of the ovary or uterus or ectopic pregnancy.
CT scans are considered to be 98 percent accurate in diagnosing acute appendicitis when read by an experienced radiologist. Before a CT is even considered, history, physical exam, and simple laboratory tests should point to appendicitis as the most likely diagnosis.

Screen rt side is pt lft side. If seen on rt side of screen its a lft ovarian cyst and if it shows on lft side of screen its rt ovarian cyst
