Derek is definitely the hardest working saxophonist (physically) on the planet! HAHA!! Now this is a TRUE duet!!!
If any saxophonist could be more physically intense than Derek, it's Colin Stetson.
OH YEAH!!! Forgot about how he does that on Bass Sax!!!! Knew him WAAAYYYYYYYY back in the day (Senior year high school University of Michigan Honors Jazz Band). Great player then too! Got to witness the beginning stages of his saxophone physicality! :)
Definitely its Leo P, hands down, no competition, the guy flails that bari around like its a piccolo, watch him at BBC Prom Night or Too Many Zoos

Looks like he has a ring on his thumb and he's hitting the thumb holder for his right hand.
Yeah, Jonathan is right, he uses a ring and hits the thumb hook to make a click sound
