    The client record is the most important tool to ensure continuity of care
    Documentation contributes to service delivery by:
    Reducing replication of services
    Presenting a cohesive longitudinal record of clinically meaningful information
    Ensuring reimbursement for services
    Assists in guarding against malpractice

    By whom
    Were they adequately credentialed
    Records professional services

    Differential diagnosis
    Placement criteria used in decision making
    Treatment and other services provided
    Response to treatment interventions

    1. If a therapist leaves a practice, who is required to keep the clients' records, the therapist or the practice? 2. If the practice closes & the owner keeps the records for the specified amount of time, what is the procedure for informing former clients of how to access their records.

    I need to do may CAC supervision. Can you do individual supervision? I also need to do my CAC classroom time.

      Am I qualified supervisor, yes. Do I have time, no, sorry. I would check around at your local treatment centers.
