The lesson I’m taking from these two replies is that, in Spelunky, hearing an offscreen explosion is equivalent to hearing glass shatter somewhere in your house at midnight. You have no idea what just happened, but there’s a very fair chance that you’re about to die.

I've gotten into the ice caves, and the moment I enter the level an enemy has immediately blown up a mine, which then launched another mine in my direction, that blew up before it hit the ground, killing me instantly. I still stand by that there was nothing i could have done to reasonably avoid that.

The only time when a death in spelunky is not your fault is when you spawn in ice caverns, and a yeti off screen is on a trampoline, and it bumps into an alien space ship, causing the spaceship to come crashing down from off screen and blowing you up. I can’t think of any other time it isn’t your fault though.
An example of insta death that isn't your fault. The yetis in ice caves. If they throw at a certain wall formation and you bounce back to them, he keeps throwing you until you die. No escape either.

Sometimes you get less than a second from spawning to death in spelunky. As in you don't get enough time to react, you spawn and immediately die.
I like how we had to keep running back things about the game that aren't so good because we needed to stick to the claim that it's perfect.
