ok so basicly this is a sad story about the lady having cancer that makes her randomly turn into a dude. It alsocauses her to randomly punch people trying to interview her about her cancer. This has attracted many people to observe, so she has naturally become very smug about it. It's not shown but at the end of the video she and her interviewer collapses (from getting cancer too.)
