I have watched a lot of baseball clips and yes I don't understand what in the hell is going on however I did notice that was a deliberate throw he threw it so hard it threw him off balance.
Even if it was unintentional, if the pitch is near the head: If the umpires don't eject the pitcher, then they are being really careless about the batter's safety.

They know how to have a good argument but I read lips and I’ve never seen them utter a wrong word. People don’t believe there are good umpires in baseball, but there definitely are.
