i believe there's a couple of forced long rests but you can complete the game in 3 days game time if you rush

It's the diseases themselves! Though you can still add them to your character as a paladin, In the instance of this mod

Various enemies can inflict you sometimes though mostly temporary. A majority of the diseases are from the spell "Contagion"

You can either play in "dishonor mode" or get a mod that allows you to keep other saves I'm honour mode
