Whatever's left of Isolde (if there is anything) in that mess is most certainly quite beyond recognition, hence the king's confusion.
The maiden is supposed to be a personification of the mist (apparently the words of a rioter, but the source got deleted since it was old). As the mist emits from Viego, maybe the maiden is a projection of how he saw Isolde, causing him to mistake her for his queen.
