With all due respect to the people involved in this hard work and detailed art, this is not traditional Persian binding, it s modern bookbinding, casework to be more precise, done by hand bookbinders. That is, a very, very new technique. The traditional methods I believe are only done today by restorers and conservators. A modern binder would say he was taught this method by his master, who was taught by his master who was taught by his master and it would seem quite a long lineage, but it would be in reality no longer than 60-80 years. Check out the work Sherif Afifi does when it comes to traditional styles.
This may be modern but it isn't common anymore, this type of work is pretty expensive to be used broadly or commercially. Having done some book repair in the past, I wouldn't want to touch one of those books for fear of destroying it further.

If you're referring to the raised hubs, as they are called, just beneath the leather cover material -- these are in imitation of the heavy cords upon which earlier books were traditionally sewn. The cords were then integrated into the cover boards as a means of attachment. Lastly, the leather material would be pasted onto the spine (by means of a flattened tubular paper structure) and the cover boards, afterwards being "turned-in" around the inside of the boards and spine. Quite a lot of work compared to the case binding used here. What makes that feasible is modern adhesives and reinforcing materials.
When you sharpen your knife on the big stone, it that an abrasive kind of sharpening (removing metal, like a Norton stone) or a butcher's steel kind of sharpening (moving and manipulating metal).

@@Zeelian Of course you probably know that when sewing books on cords was the principal method used, materials and adhesives were very different from what is available today. Wheat paste and animal glue require a more integrated method because they have shortcomings that PVAs do not. So yes, it is possible to take shortcuts without losing durability and utility. And our lives are full of decorations and design elements with many different aims -- are books somehow apart?

Why fake the spine? I don't understand it.
To make pages flatter

Probably makes it faster and easier to craft the entire cover separately from the book instead of crafting it onto the book.
There are different thechniques.
