You have to attend the both of you unless you had indicated on the ds260 not immigrating with them. If the derivative spouse fails the medical exam, he/she will be denied a visa but you will get your visa approved.
Nairobi embassy have been trying to register for interview for two weeks now but the response is always that my case is not scheduled,they gave me an email to send an email to informing them my problem qnd after I did the auto response was the email is no longer in use and they referred me to another email and when i emailed them they told me to email the body i have a mission with

Get a legal translation service from your registration office. E.g. for birth certificate, from registrar of birth
